Performance and usability testing of an automated tool for detection of peripheral artery disease using electronic health records.

Peripheral artery disease (PAD) is a common cardiovascular disorder that is frequently underdiagnosed, which can lead to poorer outcomes due to lower rates of medical optimization. We aimed to develop an automated tool to identify undiagnosed PAD and evaluate physician acceptance of a dashboard representation of risk assessment. Data were derived from electronic health records (EHR). We developed and compared traditional risk score models to novel machine learning models. For usability testing, primary and specialty care physicians were recruited and interviewed until thematic saturation. Data

Digital Health Solutions for Type 2 Diabetes and Prediabetes: Systematic Review of Engagement Barriers, Facilitators, and Outcomes.

BACKGROUND: Digital health interventions, including artificial intelligence (AI)-driven solutions, offer promise for type 2 diabetes mellitus (T2DM) and prediabetes management through enhanced self-management, adherence, and personalization. However, engagement challenges and barriers, particularly among young adults and diverse populations, persist. Existing reviews emphasize clinical outcomes while neglecting engagement factors crucial to intervention success. This review highlights engagement barriers and facilitators, offering insights into improving digital health solutions for diabetes m

A Bayesian Network-Based Browsing Model for Patients Seeking Radiology-Related Information on Hospital Websites: Development and Usability Study.

BACKGROUND: An increasing number of people are visiting hospital websites to seek better services and treatments compared to the past. It is therefore important for hospitals to develop websites to meet the needs of their patients. However, few studies have investigated whether and how the current hospital websites meet the patient's needs. Above all, in radiation departments, it may be difficult for patients to obtain the desired information regarding modality and diagnosis because such information is subdivided when described on a website. OBJECTIVE: The purpose of this study is to suggest

Low-Income Older Adults' User Challenges and Preferences Related to Voice Interface Design on Smart Speakers.

PURPOSE: Artificial intelligence-based voice assistants offer diverse functionalities to support aging in place. The current study explored usability challenges and design recommendations for voice user interfaces (VUIs) of smart speakers among low-income older adults. METHOD: Results from two user-centered design studies involving residents in low-income housing (n = 27 and n = 11, respectively), including current smart speaker users and nonusers living alone, were synthesized. Using a qualitative descriptive approach, we explored participants' experiences, challenges, and design preferences

Improving Pelvic Floor Muscle Training Adherence Among Pregnant Women: Validation Study.

BACKGROUND: Mobile health apps, for example, the Tät, have been shown to be potentially effective in improving pelvic floor muscle training (PFMT) among women, but they have not yet been studied among pregnant women. Adherence to daily PFMT will improve pelvic floor muscle strength leading to urinary incontinence (UI) improvement during the pregnancy. OBJECTIVE: This study aims to document the validation process in developing the Kegel Exercise Pregnancy Training app, which was designed to improve the PFMT adherence among pregnant women. METHODS: We utilized an intervention mapping approach

A web-based personalized decision support tool for patients diagnosed with ductal carcinoma in situ: development, content evaluation, and usability testing.

PURPOSE: Patients diagnosed with ductal carcinoma in situ (DCIS) face trade-offs when deciding among different treatments, including surgery, radiation, and endocrine therapy. A less chosen option is active monitoring. While evidence from clinical trials is not yet available, observational studies show comparable results for active monitoring and immediate treatment on cancer outcomes in select subgroups of patients. We developed and tested a web-based decision support tool (DST) to help patients explore current knowledge about DCIS and make an informed choice. METHODS: The DST, an interactiv

Evaluating the Usability of a Mobile Health Technology to Enhance Medication Adherence in People with HIV.

Antiretroviral therapy adherence remains challenging for many people with HIV. To overcome this challenge, we created and tested CHAMPS, a combination intervention comprised of a novel mobile health application (app) and community health worker sessions that monitor and manage HIV medication adherence. This study assessed the usability of the CHAMPS app. Results showed user satisfaction and perceived usability of the CHAMPS app, suggesting that the rigorous user-centered design supported a usable app.

Improving Tobacco Cessation Rates Using Inline Clinical Decision Support.

BACKGROUND: Tobacco use is a significant cause of morbidity and mortality in the United States. Even brief advice from a clinician can significantly influence cessation rates among tobacco users, but clinicians often miss opportunities to provide this simple intervention. OBJECTIVES: The intent of this quality improvement project was to increase tobacco cessation among tobacco users by nudging clinicians using a clinical decision support (CDS) tool. METHODS: We developed a CDS tool using principles of user-centered design and the CDS Five Rights to dynamically insert actionable information a

Design of Personal Health Libraries for People Returning from Incarceration in the United States.

Individuals with a history of incarceration face many barriers to accessing resources to meet their basic needs when returning to community settings. Digital health tools have potential to reduce health inequities by facilitating connections to health and social services, and peer support. This study aimed to employ a user-centered design approach to create a digital Personal Health Library (PerHL) for previously incarcerated individuals. The design process included in-depth interviews followed by rapid analysis, interpretation sessions, and user experience/user interface (UX/UI) design of a h

Requirements engineering-based mixed-methods needs assessment for a telemedicine breast cancer follow-up system.

INTRODUCTION: Breast cancer is one of the leading causes of mortality among women worldwide. Telemedicine presents a promising pathway to improve cancer care delivery; however, structured approaches to systematically eliciting and integrating stakeholder requirements remain scarce. This study aimed to conduct a comprehensive needs assessment using requirements engineering principles to inform the design of a telemedicine-based follow-up system for breast cancer patients. METHODS: An exploratory sequential mixed-methods design was employed, encompassing five stages: requirements elicitation, d

Clinical needs assessment for non-invasive intraoperative bladder volume monitoring devices: a cross-sectional study based on the Kano model.

Intraoperative urine output is a key indicator of fluid balance and renal function. With the rise of enhanced recovery after surgery (ERAS) protocols, there is a growing demand for non-invasive bladder monitoring alternatives to traditional catheterization, which poses risks such as infection and urethral injury. This study aimed to identify user-centered requirements for a non-invasive intraoperative bladder volume monitoring device using the Kano model. A cross-sectional survey was conducted among 388 surgical professionals, including anesthesiologists, surgeons, and nurses from tertiary hos

A Framework for the Human-Centered Design of Service Processes Enabled by Medical Devices: A Case Study of Wearable Devices for Parkinson's Disease.

The successful introduction of medical devices (MDs) in real-world settings hinges on designing service processes that cater to stakeholders' needs. While human-centered design (HCD) approaches have been widely applied to service process innovation, the literature lacks a methodology that leverages MDs' key features to design service processes that meet stakeholders' needs. This study aims to fill this gap by developing a framework for the HCD of service processes enabled by MDs. The proposed framework mixes and adapts methodological elements from HCD and technology-enabled design approaches a

Development of a Group Psychotherapy for Combat Veterans With Moral Injury: Protocol for a User-Centered Study.

BACKGROUND: Approximately 25% of combat veterans with posttraumatic stress disorder (PTSD) seek treatment for traumas involving potentially morally injurious events (PMIEs), which involve acts of commission, omission, or betrayal that deeply transgress one's sense of right and wrong. The sequelae of exposure to PMIEs, called "moral injury," are associated with functional and psychiatric impairment and disrupt veterans' sense of identity and meaning, ability to connect with and trust others, and engender disturbing guilt, shame, rage, and disgust. Currently, no first-line treatments directly ad
