Methods used to develop clinical guidelines in France. The example of common lumbosciatic syndrome.

A number of Regulatory Medical References, or criteria for quality health care, have been worked out in France by a committee of physicians' union and national health insurance agency representatives, based on clinical guidelines developed using predefined methods. For each guideline, the level of evidence was evaluated, and when this level was inadequate a strong professional consensus was sought. For common lumbosciatic syndrome, the ANDEM (Agence Nationale pour le Développment de l'Evaluation Médicale, or national agency for the development of medical evaluation) worked with a multidiscipli

From Tanzania to Washington, DC: Application of the Consolidated Framework for Implementation Research and ADAPT-ITT Model to Guide the Development of a Community-Based PrEP Intervention for Black Adults.

While HIV incidence in Washington, DC has declined overall, significant racial disparities persist. Pre-exposure prophylaxis (PrEP) remains underutilized among Black adults due to multiple barriers including medical mistrust, stigma, and structural barriers to access. Using the Consolidated Framework for Implementation Research (CFIR) and ADAPT-ITT model, we conducted in-depth interviews with opinion leaders (n = 28) including community-based organization leaders, health department representatives, healthcare providers, and current/potential PrEP users to guide the adaptation of a community-ba

Psychotropic medication non-adherence and its determinants among adult psychiatric patients in Ethiopia.

BACKGROUND: Mental illness remains a significant global public health concern, yet it is largely manageable through psychotropic medications and psychotherapy. However, non-adherence to prescribed psychotropic medications is a major barrier to effective treatment and long-term symptom control. This challenge compromises clinical outcomes, increases relapse rates, and places a burden on healthcare systems. Understanding the extent and contributing factors of medication non-adherence is essential for designing targeted interventions at both clinical and community levels. This study aims to asses

Chronic obstructive pulmonary disease as a systemic risk factor for cancer: a nationwide cohort study in Korea.

BACKGROUND: Chronic obstructive pulmonary disease (COPD) is increasingly recognised as a systemic condition with extrapulmonary comorbidities including malignancy. While the association between COPD and lung cancer is well established, evidence for extrapulmonary cancers, particularly after accounting for lifestyle, demographic and socioeconomic factors, remains limited. METHODS: We conducted a nationwide population-based cohort study using the Korean National Health Insurance Service-Health Screening Cohort (2002-2019). After exclusions and propensity score overlap weighting, 532 174 patient

Machine learning algorithm for early detection of end-stage renal disease.

BACKGROUND: End stage renal disease (ESRD) describes the most severe stage of chronic kidney disease (CKD), when patients need dialysis or renal transplant. There is often a delay in recognizing, diagnosing, and treating the various etiologies of CKD. The objective of the present study was to employ machine learning algorithms to develop a prediction model for progression to ESRD based on a large-scale multidimensional database. METHODS: This study analyzed 10,000,000 medical insurance claims from 550,000 patient records using a commercial health insurance database. Inclusion criteria were pa

Lyme borreliosis in Finland: a register-based linkage study.

BACKGROUND: In Finland, the routine surveillance of Lyme borreliosis (LB) is laboratory-based. In addition, we have well established national health care registers where countrywide data from patient visits in public health care units are collected. In our previous study based on these registers, we reported an increasing incidence of both microbiologically confirmed and clinically diagnosed LB cases in Finland during the past years. Here, we evaluated our register data, refined LB incidence estimates provided in our previous study, and evaluated treatment practices considering LB in the prima

Association between insurance status and patient safety in the lumbar spine fusion population.

BACKGROUND CONTEXT: Lumbar fusion is a common and costly procedure in the United States. Reimbursement for surgical procedures is increasingly tied to care quality and patient safety as part of value-based reimbursement programs. The incidence of adverse quality events among lumbar fusion patients is unknown using the definition of care quality (patient safety indicators [PSI]) used by the Centers for Medicare and Medicaid Services (CMS). The association between insurance status and the incidence of PSI is similarly unknown in lumbar fusion patients. PURPOSE: This study sought to determine th

Profile and interplay of mental health as well as physical health symptoms in post-COVID patients.

Currently, no clinically relevant physiological markers of post COVID-19 condition exist, which stymies treatment development and rehabilitation efforts. Therefore, a monocentric cross-sectional study was conducted to characterize the long-term mental and physical health consequences of infection with SARS-COV-2. 156 participants were recruited as part of the voluntary post COVID-19 condition consultation hours at the BG Hospital Hamburg, Germany, after the insured persons were invited by the relevant statutory accident insurance providers. Overall, 34% of participants were unable to work. Bas

Design of a randomized clinical trial of a colorectal cancer screening decision aid to promote appropriate screening in community-dwelling older adults.

BACKGROUND: Appropriate colorectal cancer screening in older adults should be aligned with the likelihood of net benefit. In general, patient decision aids improve knowledge and values clarity, but in older adults, they may also help patients identify their individual likelihood of benefit and foster individualized decision-making. We report on the design of a randomized clinical trial to understand the effects of a patient decision aid on appropriate colorectal cancer screening. This report includes a description of the baseline characteristics of participants. METHODS: English-speaking prim

Patient-reported disability progression outcomes among patients with multiple sclerosis: Results of an outcomes-based agreement.

BackgroundOutcomes-based agreements (OBAs) are agreements between payers and manufacturers in which payment for medications is tied to patient outcomes. These contracts aim to measure the value of prescription medications on predefined clinical indicators in real-world patient populations. OBAs are gaining traction in the United States as the health care industry shifts from volume-based to value-based care. Multiple sclerosis (MS) is an appealing therapeutic area for OBAs because of its prevalence, high cost of medications, and multiple effective therapeutic options.ObjectiveTo describe findi
