Tropical leg ulcers in children: more than yaws.

The management of yaws has changed in recent years. Mass treatment with oral azithromycin has replaced intramuscular benzathine benzylpenicillin. Treponemal and non-treponemal serology (equivalent to TPHA and RPR) point-of-care blood testing is now available. In addition, recent studies in yaws endemic regions have shown that a significant number of leg ulcers in children which are clinically suggestive of yaws are caused by Haemophilus ducreyi. It is noteworthy that the World Health Organization has also set the ambitious goal to eliminate yaws by 2020.

alpha+-Thalassemia protects children against disease caused by other infections as well as malaria.

In the South West Pacific region, the striking geographical correlation between the frequency of alpha+-thalassemia and the endemicity of Plasmodium falciparum suggests that this hemoglobinopathy provides a selective advantage against malaria. In Vanuatu, paradoxically, alpha+-thalassemia increases the incidence of contracting mild malaria in the first 2 years of life, but severe disease was too uncommon to assess adequately. Therefore, we undertook a prospective case-control study of children with severe malaria on the north coast of Papua New Guinea, where malaria transmission is intense and

The Influence of Parental Gender-Based Violence and Discrimination on Children's Socioemotional Development: Evidence From Low- and Middle-Income Countries.

This study investigates the intergenerational impacts of gender-based violence and discrimination (GBVD) experienced by mothers on the socioemotional development of young children using microdata from over 6,000 mother-child pairs drawn from the Multiple Indicator Cluster Surveys (MICS6) across five LMICs: Comoros, Eswatini, Fiji, Vanuatu, and Vietnam. Using household fixed effects and instrumental variable approaches, we examine how various dimensions of maternal GBVD, including feeling unsafe at home, gender-based and sexual orientation discrimination, and unsafe community environments, affe

A decade of global volcanic SO2 emissions measured from space.

The global flux of sulfur dioxide (SO2) emitted by passive volcanic degassing is a key parameter that constrains the fluxes of other volcanic gases (including carbon dioxide, CO2) and toxic trace metals (e.g., mercury). It is also a required input for atmospheric chemistry and climate models, since it impacts the tropospheric burden of sulfate aerosol, a major climate-forcing species. Despite its significance, an inventory of passive volcanic degassing is very difficult to produce, due largely to the patchy spatial and temporal coverage of ground-based SO2 measurements. We report here the firs

Ciguatera poisoning: a global issue with common management problems.

Ciguatera poisoning, a toxinological syndrome comprising an enigmatic mixture of gastrointestinal, neurocutaneous and constitutional symptoms, is a common food-borne illness related to contaminated fish consumption. As many as 50000 cases worldwide are reported annually, and the condition is endemic in tropical and subtropical regions of the Pacific Basin, Indian Ocean and Caribbean. Isolated outbreaks occur sporadically but with increasing frequency in temperate areas such as Europe and North America. Increase in travel between temperate countries and endemic areas and importation of suscepti

Plasmodium vivax: a cause of malnutrition in young children.

We studied the aetiology of malnutrition in a cohort of 1511 children < 10 years old in Espiritu Santo, Vanuatu. Malnutrition was categorized using standard anthropometric criteria as: underweight [weight-for-age (WA) Z score < -2], wasting [weight-for-height (WH) Z < -2], or stunting [height-for-age (HA) Z < -2]. On multiple logistic regression analysis, the only factors significantly associated with wasting were age < 5 years [OR (95% CI) 1.8 (1.2-2.9), p = 0.01] and having suffered one or more episodes of clinical P. vivax malaria in the 6 months preceding nutritional assessment [OR 2.4 (1.

Red blood cell phenotypes in the alpha + thalassaemias from early childhood to maturity.

The alpha+ thalassaemias are the most common single gene disorders of humans, yet little is known about their haematological characteristics in childhood. Blood samples have been collected randomly from more than 2000 individuals in village communities in Vanuatu in the South West Pacific and analysed for alpha thalassaemia and associated haematological changes. Here we describe the haematological effects of the alpha+ thalassaemias from early childhood through to maturity in this population. Mean cell volume (MCV) and mean cell haemoglobin (MCH) levels in individuals of normal, heterozygous a

Evaluation of Multiplex-Based Antibody Testing for Use in Large-Scale Surveillance for Yaws: a Comparative Study.

WHO has targeted yaws for global eradication by 2020. The program goals are to interrupt the transmission in countries where yaws is endemic and to certify countries as yaws free where yaws was endemic in the past. No new rapid plasmin reagin (RPR) seroreactivity in young children is required for certification of elimination at a country level. We sought to evaluate whether antibody responses to specific treponemal antigens measured in a high-throughput multiplex bead array (MBA) assay differentiate past versus current infection and whether a nontreponemal lipoidal antigen test can be incorpor

Progress in the molecular biology of malaria and the immunology of nematode infections.

Japan is one of a small number of countries to have successfully controlled or eliminated major parasitic diseases, including malaria, filariasis, schistosomiasis and enteric parasitoses. Of particular importance in this success was a close collaboration between primary research and public health efforts. Many Japanese researchers continue to study malaria, particularly the areas of genetics and immunology, and this should contribute to global parasite eradication strategies. Furthermore, studies of immunoregulation of nematode infection using the interleukin-18 pathway, most of which have bee

Asia-Pacific analysis of ESMO study on availability, out-of-pocket costs (OPC) and accessibility of antineoplastic medicines.

BACKGROUND: The European Society for Medical Oncology (ESMO) conducted a global survey on the availability, out-of-pocket costs (OPCs), and accessibility of cancer medicines in 2015, with an updated survey in 2023. The Asia-Pacific (APAC) region accounts for approximately half of global cancer-related deaths, yet substantial disparities persist in access to essential cancer medicines. A focused analysis of the 2023 ESMO global survey was undertaken to evaluate access trends specific to the APAC region. PATIENTS AND METHODS: Data from the 2023 ESMO global survey were analysed to assess the ava

Across six societies children engage in costly third-party punishment of unfair sharing.

Third-party punishment of unfairness shows striking cross-societal variation in adults, yet we know little about where and when in development this variation starts to emerge. When do children across societies begin to pay a cost to prevent unfair sharing? We present an experimental study of third-party punishment of unfair sharing across N = 535 children aged 5-15 from communities in six diverse countries: Canada, India, Peru, Uganda, USA, and Vanuatu. We tested whether children were more likely to punish equal or selfish (maximally unequal) distributions between two absent peers. We also tes

Trachoma, Anti-Pgp3 Serology, and Ocular Chlamydia trachomatis Infection in Papua New Guinea.

BACKGROUND: In Melanesia, the prevalence of trachomatous inflammation-follicular (TF) suggests that public health-level interventions against active trachoma are needed. However, the prevalence of trachomatous trichiasis is below the threshold for elimination as a public health problem and evidence of conjunctival infection with trachoma's causative organism (Chlamydia trachomatis [CT]) is rare. Here, we examine the prevalence of ocular infection with CT and previous exposure to CT in three evaluation units (EUs) of Papua New Guinea. METHODS: All individuals aged 1-9 years who were examined f

Mortality and cause-of-death reporting and analysis systems in seven Pacific Island countries.

BACKGROUND: Mortality statistics are essential for population health assessment. Despite limitations in data availability, Pacific Island Countries are considered to be in epidemiological transition, with non-communicable diseases increasingly contributing to premature adult mortality. To address rapidly changing health profiles, countries would require mortality statistics from routine death registration given their relatively small population sizes. METHODS: This paper uses a standard analytical framework to examine death registration systems in Fiji, Kiribati, Nauru, Palau, Solomon Islands

Human T-cell lymphotropic virus type I infection and disease in the Pacific basin.

Human T-cell lymphotropic virus type I (HTLV-I), the cause of adult T-cell leukemia/lymphoma (ATLL) and HTLV-I-associated myelopathy/tropical spastic paraparesis (HAM/TSP), is widespread in the Pacific basin. Modes of virus transmission include blood transfusion (and intravenous drug use), breast milk, and sexual intercourse. High prevalences of HTLV-I infection and disease occur among the inhabitants of southwestern Japan and among first- and second-generation (issei and nisei) Japanese-Americans in the Hawaiian Islands. Other Pacific populations with high prevalences of HTLV-I infection incl

A systematic review of historical and contemporary evidence of trachoma endemicity in the Pacific Islands.

INTRODUCTION: Trachoma is endemic in several Pacific Island countries. The aims of this study were to (a) identify future trachoma mapping needs in the Pacific and (b) to examine whether any temporal trends in trachoma prevalence could be ascertained from the historical literature on trachoma in the Pacific Islands. METHODS: Human studies of trachoma and eye care in the Pacific Islands were identified from a systematic search of PubMed, EMbase, Scopus and Web of Science databases. A published quality assessment system for disease prevalence studies was modified to assess studies for quality a
