Vasa Previa: Prenatal Diagnosis and the Rationale Behind Using a 5 cm Distance from Internal Os.

In pregnancies with vasa previa, prenatal diagnosis and pre-labor cesarean delivery are associated with significantly improved perinatal outcomes compared to undetected cases. However, a universally accepted ultrasonographic definition of vasa previa is lacking. Specifically, the distance from the cervical internal os beyond which vaginal delivery can be safely recommended remains to be determined. Field experts and recently published societal guidelines agree that a 2 cm cut-off is suboptimal, given that complete cervical dilation during labor risks unprotected fetal vessels within a 5 cm rad

Incidence and causes of perinatal death in prenatally diagnosed vasa previa: a systematic review and meta-analysis.

OBJECTIVE: This study aimed to estimate the perinatal mortality associated with prenatally diagnosed vasa previa and to determine what proportion of those perinatal deaths are directly attributable to vasa previa. DATA SOURCES: The following databases have been searched from January 1, 1987, to January 1, 2023: PubMed, Scopus, Web of Science, and Embase. STUDY ELIGIBILITY CRITERIA: Our study included all studies (cohort studies and case series or reports) that had patients in which a prenatal diagnosis of vasa previa was made. Case series or reports were excluded from the meta-analysis. All

Can we perform a prenatal diagnosis of vasa previa to improve its obstetrical and neonatal outcomes?

INTRODUCTION: Vasa previa (VP) is defined as a condition in which the fetal blood vessels, unsupported by the placenta or the umbilical cord, run through the membranes of the lower uterine segment. It is associated with a high risk of stillbirth by exsanguination. This study aimed to assess the clinical context of diagnosis of VP in order to elaborate a strategy for its prenatal diagnosis and to improve its obstetrical and neonatal outcomes. MATERIAL AND METHODS: This historical cohort study covered the period from January 1, 2011 to December 31, 2015. All women who gave birth at our obstetri

Clinical significances of magnetic resonance imaging in prenatal diagnosis of vasa previa in a woman with bilobed placentas.

We report a case of a pregnant woman diagnosed as having vasa previa by magnetic resonance imaging (MRI). A parous woman was referred to our hospital at 31 weeks of gestation due to suspicion of placenta previa. Transvaginal ultrasound examination together with the Doppler techniques showed a fetal vessel on a lesion of low and high mixed echogenecities over the internal os, but could not confirm whether it was placental tissue or not. MRI demonstrated that it was not placenta but a hemorrhage between bilobed placentas and that the vessel was running over the internal os freely from the placen

An easy-to-use method for detecting fetal hemoglobin--a test to identify bleeding from vasa previa.

BACKGROUND: Vasa previa is a rare but potentially dangerous fetal condition that may occur during pregnancy. Ideally, all cases such cases are detected antenatally, but many present as late vaginal hemorrhaging. At the current time, there is no test for fetal hemoglobin (HbF) in general use. METHODS: A modified method of identifying HbF is presented. Five milliliters of 0.14 M NaOH was combined with 50 microl of a mixture of fetal and maternal blood. After 2 min, it was judged if the solution still had a red tone or not. The sensitivity of this method for detecting HbF was assessed. RESULTS:

Type III Vasa Previa Associated with Resolution of a Low-Lying Placenta: Case Report and Literature Review.

Vasa previa occurs when fetal vessels lie above the cervical os. A novel type of vasa previa, known as type III, is characterized by an abnormal branching of fetal vessels from the placenta in the absence of velamentous cord insertion (as seen in type I) or multilobed placenta (as seen in type II). Here, we present a case of a type III vasa previa after a resolution of a low-lying placenta. The presence of any known risk factors of vasa previa, including low-lying placenta, should prompt screening for vasa previa in the third trimester. Accurate and timely diagnosis of vasa previa will confer

Double fatal outcome after ruptured vasa previa in monochorionic twins: case report and review of the literature.

Vasa previa is a condition in which one or more fetal blood vessels run through the amniotic membranes and cross or run near the external orifice of the uterus. Rupture of membranes can lead to tearing of these vessels and cause acute fetal exsanguination. In monochorionic twin (MC) pregnancies, acute exsanguination in one twin can lead to severe complications in the co-twin due to the presence of inter-twin placental vascular connections. We report a MC pair with severe perinatal asphyxia due to acute exsanguination after prenatally undetected ruptured vasa previa. This resulted in severe hem

Early Detection of Vasa Previa in a Low-Risk Pregnancy Leading to Favourable Perinatal Outcome: A Case Report.

Vasa previa is a condition with a high mortality rate where the foetal blood vessels traverse, or lie in proximity to, the internal cervical os and are typically not supported by the umbilical cord or placenta. Recognised risk factors of vasa previa include velamentous umbilical cord insertion, assisted reproductive technologies, multiple pregnancies, bilobed or succenturiate placentas, and low-lying placental insertion. Perinatal mortality rate remains high when vasa previa is not detected antenatally, although current evidence does not support universal screening for vasa previa in the gener

Fetoscopic laser photocoagulation: a medically reasonable treatment option in the management of types II and III vasa previa.

Vasa previa is a condition where unprotected fetal vessels cross the cervix within the membranes, posing a considerable risk of fetal death or severe morbidity if the membranes rupture before or during delivery. There has not been a definitive in utero treatment for this condition. Patients are typically closely monitored and hospitalized in the early third trimester and scheduled for cesarean delivery before term. This approach poses considerable physical, social, psychological, and financial challenges for pregnant patients and their families. Furthermore, fetal vessel rupture may lead to se

Impact of targeted scanning protocols on perinatal outcomes in pregnancies at risk of placenta accreta spectrum or vasa previa.

BACKGROUND: Placenta accreta spectrum and vasa previa (VP) are congenital disorders of placentation associated with high morbidity and mortality for both mothers and newborns when undiagnosed before delivery. Prenatal diagnosis of these conditions is essential to allow multidisciplinary management and thus improve perinatal outcomes. OBJECTIVE: The objective of the study was to compare perinatal outcome in women with placenta accreta spectrum or vasa previa before and after implementation of targeted scanning protocols. STUDY DESIGN: This retrospective study included 2 nonconcurrent cohorts

A close call: does the location of incision at cesarean delivery matter in patients with vasa previa? A case report.

We present here a case of vasa previa in a multipara, diagnosed at the time of her late second trimester ultrasonogram. The patient subsequently underwent an elective cesarean section after 37 weeks gestation, giving birth to a healthy child with an uneventful post-partum, neonatal and infant course. At the time of cesarean section, the incision was gradually deepened in layers through the myometrium by utmost care allowing the amniotic sac to protrude through the uterine incision hereby avoiding laceration of the vasa previa and its branches. Fetal exsanguination and a need for blood transfus

Mental and emotional health impacts of vasa previa on birthing individuals and health care providers: a scoping review protocol.

OBJECTIVE: This scoping review will assess mental and emotional outcomes of birthing individuals diagnosed with vasa previa (VP) in any birthing context, including evidence that captures the perspectives of health care providers involved in their care. INTRODUCTION: VP is a rare obstetric complication that poses a significant risk of perinatal morbidity and mortality. There is a limited understanding of the emotional and psychological impacts on birthing individuals diagnosed with VP. The review will explore mental or emotional health outcomes related to the diagnosis of VP and the care for b

The natural history and management of vasa previa: a single institution's 15-year experience managing patients remote from labor and delivery.

INTRODUCTION: Vasa previa (VP) is a diagnosis with potential catastrophic obstetric outcomes. We describe the outcomes of VP managed at our institution, which uniquely provides inpatient monitoring on an antepartum unit located remotely from labor and delivery (L&D). METHODS: Retrospective study of singleton pregnancies diagnosed with VP at a single institution. VP was diagnosed prenatally by ultrasound if one or more fetal vessels unsupported by underlying placenta were coursing within 2 cm of the internal os. Most cases were admitted for inpatient monitoring; however, patients were counsele
