Development of in vivo tissue-engineered microvascular grafts with an ultra small diameter of 0.6 mm (MicroBiotubes): acute phase evaluation by optical coherence tomography and magnetic resonance angiography.

Biotubes, i.e., in vivo tissue-engineered connective tubular tissues, are known to be effective as vascular replacement grafts with a diameter greater than several millimeters. However, the performance of biotubes with smaller diameters is less clear. In this study, MicroBiotubes with diameters <1 mm were prepared, and their patency was evaluated noninvasively by optical coherence tomography (OCT) and magnetic resonance angiography (MRA). MicroBiotube molds, containing seven stainless wires (diameter 0.5 mm) covered with silicone tubes (outer diameter 0.6 mm) per mold, were embedded into the d

Management of pelvic venous stenosis with intravascular stainless steel stents.

Patients with gynecologic malignancies may develop stenosis of the large pelvic veins as a result of their disease or its treatment. The percutaneous insertion of a stainless steel vascular stent is a novel approach to the management of an extrinsically compressed vein. The objective of this study was to review the results of treating lower extremity edema secondary to a pelvic venous stenosis through the percutaneous insertion of a stainless steel vascular stent. A retrospective review was performed on gynecologic oncology patients who presented with an edematous lower extremity and underwent

Long-Term Management of Vascular Access Ports in Nonhuman Primates Used in Preclinical Efficacy and Tolerability Studies.

Vascular access ports (VAPs) are an essential tool for long-term vascular access in preclinical studies and disease modeling in non-human primates (NHPs). We retrospectively reviewed central (inferior vena cava, IVC) and portal VAP implantation with the maintenance at our center from 15 January 2010 to 31 January 2018. In total, 209 VAPs were implanted for long-term drug administration and sampling. Patency was >95% at 6 months and >80% at 1 year for IVC VAPs and >90% at 6 months and >85% at 1 year for portal VAPs. The majority of animals had no complications and access was generally durable w

Six years of clinical follow-up with endothelial cell-seeded small-diameter vascular grafts during coronary bypass surgery.

This clinical study was performed to investigate the patency rate of endothelial cell-seeded small-diameter expanded polytetrafluoroethylene grafts during coronary artery bypass surgery. Between September 1995 and December 1998, 14 patients (median age: 71 years, range: 61-79 years) received 21 endothelial cell-seeded small-diameter grafts. In all, 43% of the performed implantations were reoperations. Endothelial cells were harvested from a forearm vein, cultured and characterized in the laboratory until a sufficient number was available. After in vitro seeding, the grafts were allowed to matu

A single-center experience in the management of combat-related extremity vascular injuries.

OBJECTIVE: Combat-related vascular injuries account for up to 12% of trauma cases, with peripheral vessel injuries being the most prevalent among casualties. The ongoing military conflict in the Middle East has led to the widespread use of explosive devices and missiles, resulting in a high incidence of peripheral vascular trauma. METHODS: This study presents a retrospective single-center analysis of all patients with peripheral vascular trauma admitted to our facility between October 2023 and November 2024. Data on patient demographics, injury patterns, surgical interventions, and outcomes w

Midterm Outcomes and Predictors of Failure of Lower Extremity Bypass to Para-Malleolar and Pedal Targets.

BACKGROUND: The frequency of distal lower extremity bypass (LEB) for infrapopliteal critical limb threatening ischemia (IP-CLTI) has significantly decreased. Our goal was to analyze the contemporary outcomes and factors associated with failure of LEB to para-malleolar and pedal targets. METHODS: We queried the Vascular Quality Initiative infrainguinal database from 2003 to 2021 to identify LEB to para-malleolar or pedal/plantar targets. Primary outcomes were graft patency, major adverse limb events [vascular reintervention, above ankle amputation] (MALE), and amputation-free survival at 2 yea

Ten-year experience using cryopreserved arterial allografts for vascular reconstruction during major oncologic surgery.

OBJECTIVE: Resection of vessel-encasing tumors has been historically associated with high morbidity and mortality. However, advances in surgical techniques and cancer treatments have improved outcomes for patients undergoing resection with vascular reconstruction. In specialized centers, cryopreserved arterial allografts (CAAs) are increasingly used when autologous conduits are unavailable, offering superior anatomical compliance and resistance to infection compared with prosthetic conduits. This study aimed to evaluate the outcomes of CAAs as a conduit for vascular reconstruction during major

Evaluating the Safety and Efficacy of Humacyte Acellular Tissue-Engineered Vessel in a Real-World Combat Setting: A Retrospective Observational Multicenter Study.

INTRODUCTION: Peripheral vascular trauma occurs as 10% of trauma injuries in combat settings. Its treatment in a combat setting is associated with challenges such as increased risk of infection, increased time to treatment, and lack of availability of medical personnel to perform vascular surgery. Currently, the autologous great saphenous vein or synthetic vessels such as Dacron or Gore-Tex (Teflon) are used to treat peripheral vascular trauma. Unfortunately, these options may neither be available nor advisable because of injury to the native vasculature or the increased risk of infection. Thu

Optimizing Dialysis Vascular Access: Moving beyond Fistula First.

For many years, the vascular access guidelines recommended placement of arteriovenous fistulas (AVFs) in preference to arteriovenous grafts (AVGs) because AVFs had superior long-term patency, required fewer interventions to maintain patency, and were associated with lower costs of vascular access management. However, subsequent research has questioned the Fistula First strategy (placing an AVF whenever the vascular anatomy is suitable). First, AVF nonmaturation is substantial (30%-40%) and even higher among women, older patients, and those with peripheral vascular disease. Second, about half o

Light-Activated Vascular Anastomosis.

Background. There have been few advances in technique since vascular anastomosis was performed with silk suture on a curved needle in 1902. This technique results in disruption of the endothelium with exposed intraluminal suture, both of which may lead to thrombocyte aggregation, intimal hyperplasia, and vascular stenosis. A variety of alternative techniques have been explored, with limited success. Photochemical tissue bonding (PTB) is a light-activated methodology of rapidly cross-linking tissue interfaces at the molecular level. Herein, we describe a new technique for anastomosis of venous

Uncooled infrared camera for the noninvasive visualization of the vascular flow in an anastomotic vessel during neurological surgery: technical note.

We herein present our experience to assess intraoperative confirmation of vascular patency with an uncooled infrared camera in extracranial-intracranial (EC-IC) bypass surgery. This camera had distinguishing characteristics, including its small size, light weight, and adequate temperature resolution (< 0.022 degrees). We used a simplified zoom germanium lens as a preliminary study to verify the potential of using this camera to assess the vascular flow of the end-to-side anastomosis model in rats. In addition, we evaluated the vascular flow in continuous clinical series using this infrared cam

Influence of balloon type and lesion location on outcomes following fistuloplasty.

BackgroundThere is limited evidence supporting the optimal use of fistuloplasty to maintain vascular access at various lesion sites, despite its critical role in facilitating renal replacement therapy and the overall high failure rates of arteriovenous fistulas (AVFs). This study aims to identify covariates affecting primary and secondary patency following fistuloplasty of native upper limb vascular access AVFs.MethodsThis retrospective study included all patients who underwent fistuloplasty at a tertiary vascular centre over 4 years.
