Sevuparin for the treatment of acute pain crisis in patients with sickle cell disease: a multicentre, randomised, double-blind, placebo-controlled, phase 2 trial.

BACKGROUND: There are no approved treatments for vaso-occlusive crises in sickle cell disease. Sevuparin is a novel non-anticoagulant low molecular weight heparinoid, with anti-adhesive properties. In this study, we tested whether sevuparin could shorten vaso-occlusive crisis duration in hospitalised patients with sickle cell disease. METHODS: We did a multicentre, double-blinded, placebo-controlled, phase 2 study in 16 public access clinical hospitals in the Netherlands, Lebanon, Turkey, Bahrain, Oman, Saudi Arabia, and Jamaica. Patients aged 12-50 years with a diagnosis of sickle cell disea

Assessing and managing people with sickle cell disease presenting with vaso-occlusive crisis in the emergency department.

Sickle cell disease is a group of inherited blood disorders characterised by atypical haemoglobin molecule structures (haemoglobin S) which can manifest as painful vaso-occlusive crises, chronic anaemia and progressive organ damage. This article aims to raise greater awareness of sickle cell disease, its acute manifestations and the potential for rapid clinical deterioration that can occur in patients with this condition. The article outlines the main aspects of the assessment and management of patients presenting to the emergency department (ED) with vaso-occlusive crisis, describes the barri

Plasma concentrations of asymmetric dimethylarginine, an endogenous nitric oxide synthase inhibitor, are elevated in sickle cell patients but do not increase further during painful crisis.

Plasma concentrations of asymmetric dimethylarginine (ADMA), an endogenous nitric oxide synthase inhibitor, are elevated in the clinically asymptomatic state of sickle cell disease (SCD). However, the role of ADMA during vaso-occlusive complications has not been defined. ADMA concentrations were determined in HbSS (n = 43) and HbSC (n = 25) patients with healthy blood donors (HbAA) as controls. In the clinically asymptomatic state ADMA concentrations were elevated in sickle cell patients as compared to healthy controls (HbSS 0.70 micromol/L, HbSC 0.54 micromol/L, HbAA 0.39 micromol/L) (P < 0.0

Hydroxyurea therapy decreases the in vitro adhesion of sickle erythrocytes to thrombospondin and laminin.

The adhesion of sickle erythrocytes to the vascular endothelium and subendothelial matrix probably contributes to the pathogenesis of vaso-occlusive disease. The chemotherapeutic agent hydroxyurea (HU) decreases the frequency of vaso-occlusive crises in patients with sickle cell disease. However, the exact mechanism(s) of HU's effect on vaso-occlusive crises is not fully understood. The goal of this study was to determine the effect of HU therapy on the adhesion of sickle erythrocytes to the subendothelial matrix proteins thrombospondin (TSP) and laminin under conditions of flow in vitro. Prevention of red cell dehydration: a possible new treatment for sickle cell disease.

One of the pathogenetic mechanisms responsible for sickling of erythrocytes in patients with sickle cell disease is the decreased hydration status of the cells. In this brief review, we discuss the pathophysiologic background and explore some new treatment options to prevent vaso-occlusive crises or other problems in this patient population.

Hydroxyurea and Gut Microbiome Interactions in Sickle Cell Disease: Toward Adjunctive Microbiome-based Therapy.

Sickle cell disease (SCD) is a monogenic disorder marked by hemoglobin S polymerization, resulting in chronic hemolysis, vaso-occlusion, systemic inflammation, and progressive multiorgan damage. Despite major therapeutic advances, SCD remains a complex inflammatory condition with significant morbidity. Hydroxyurea is the cornerstone of treatment, primarily by inducing fetal hemoglobin and reducing vaso-occlusive crises and hemolysis. It also exerts anti-inflammatory effects by decreasing leukocyte activation and endothelial adhesion. However, hydroxyurea does not fully reverse microvascular in

Pain Management in Pediatric Patients with Sickle Cell Disease.

Sickle cell pain is a complex and multifaceted condition that varies widely across individuals, necessitating a personalized approach informed by a comprehensive history and careful consideration of the biopsychosocial context. Although opioids remain the mainstay of pain management for individuals experiencing acute pain during a vaso-occlusive episode (VOE), multiple disease-modifying therapies have emerged and are demonstrating promise in helping to reduce the frequency and duration of VOEs. In addition, nonopioid analgesics and nonpharmacologic interventions show potential as adjunct thera

Multimodal imaging features to diagnose abdominal complications of sickle cell disease.

Sickle cell disease (SCD) is a hereditary hemoglobinopathy, marked by chronic hemolysis and recurrent vaso-occlusive crises, which frequently leads to a wide range of abdominal complications. Sickled erythrocytes have reduced deformability and increased adhesion to vessel walls, leading to chronic vascular occlusion, inflammation, and hemolysis. Comprehensive imaging plays a pivotal role in the early detection and assessment of these complications, thereby mitigating morbidity and severe outcomes. Ultrasound (US), computed tomography (CT), and magnetic resonance imaging (MRI) are the three mod

Systematic review of machine learning approaches for predicting sickle cell crisis and mortality risk at the climate-health nexus.

BACKGROUND: Sickle cell anemia (SCA) is a severe genetic blood disorder characterized by recurrent vaso-occlusive crises and increased mortality, with the greatest burden occurring in low- and middle-income countries. Climatic and environmental conditions, including temperature variability, humidity, rainfall, air pollution, and seasonal changes, have been associated with disease exacerbation. However, the extent to which these factors have been incorporated into predictive models remains unclear. This study systematically reviews the application of machine learning (ML) models for predicting

Fetal Hemoglobin Research and Clinical Relevance in Sickle Cell Disease.

Sickle cell disease (SCD) is the most common monogenic disorder, globally affecting close to eight million individuals. SCD is a devastating disease where patients suffer from extreme pain due to vasoocclusive episodes (VOEs), end-organ damage, and premature death. The burden of SCD is projected to rise significantly in the near future, making basic and clinical research to develop curative treatments even more acute. SCD was the first genetic disorder where the underlying molecular alterations were defined, an amino acid change resulting in polymerization of the sickle hemoglobin molecule and

[Infectious, pathologic humeral fracture in a patient with sickle cell disease-A rare case?].

The vaso-occlusive crises of sickle cell disease are accompanied by bone necrosis, which favors endogenous bacterial colonization and thus osteomyelitis. This poses a major challenge for eradication and fracture management.A 22-year-old patient with sickle cell disease sustained a multifragmentary, humeral shaft fracture. During surgical management, pus drained from the fracture site and further diagnostic work-up revealed osteomyelitis with evidence of Klebsiella aerogenes. Septicemia due to Klebsiella aerogenes had been treated 5 months prior to the accident, which occured because of a vaso-

Myocardial ischaemia in sickle cell anaemia: evaluation using a new scoring system.

BACKGROUND: Sickle cell anaemia (SCA) is associated with recurrent multi-organ ischaemia and infarction. Myocardial ischaemia (MI) and infarction are increasingly recognised as features of SCA. The prevalence and severity of MI in children with SCA is not known. AIM: To evaluate the usefulness of a new scoring system based on the standard surface electrocardiogram (ECG) in determining the prevalence and severity of MI in children with SCA. METHOD: MI prevalence and scores derived from standard surface ECGs of 35 children with SCA aged 3-18 years who presented consecutively during 38 episodes

Treatment of Acute Pain Crisis in a Patient with Sickle Cell Disease Using Continuous Regional Anesthesia: A Case Report.

BACKGROUND: Sickle cell disease (SCD) is a predictor of both chronic pain and acute pain crises (APC). Vaso-occlusive APC typically require an escalation of home analgesic therapy, often requiring inpatient large doses of opioid pain medications. The analgesia and distal vasodilation provided by regional anesthesia could be of theoretical benefit when treating patients with SCD who present with vaso-occlusive APC. CASE REPORT: This case describes the treatment of a patient with SCD who presents with upper extremity APC. Her pain crisis was otherwise refractory to medical analgesic therapy and

Impact of Different Definitions of Vaso-Occlusion on Efficacy Assessments in Sickle Cell Disease Clinical Trials.

INTRODUCTION: Patients with sickle cell disease (SCD) experience recurrent, severe pain events due to vaso-occlusion. Eliminating these acute pain events is a key outcome in SCD clinical trials; however, the definition of a vaso-occlusive crisis (VOC) or a vaso-occlusive event (VOE) has not been consistently applied, hampering comparisons of treatment efficacy between different therapeutic approaches. We have examined the degree to which differing definitions of vaso-occlusion in clinical trial endpoints impact efficacy outcomes. METHODS: Descriptions of clinical endpoints related to vaso-occ

Fragment D-dimer levels: an objective marker of vaso-occlusive crisis and other complications of sickle cell disease.

Although abnormalities in coagulation tests have been reported during vaso-occlusive crises in patients with sickle cell disease, objective, readily performed laboratory tests that document the occurrence of this complication have not been available. We examined the relationship between fibrin D-dimer levels and the occurrence of complications in patients with sickle cell disease, using a commercially available latex bead agglutination assay. The patients were either asymptomatic, hospitalized for vaso-occlusive crisis, or had other complications of sickle cell disease including leg ulcers, ch

Acute management of sickle cell crisis in pregnancy.

Vaso-occlusive sickle cell crisis (VSCC) is the most common maternal complication in pregnancies associated with sickle hemoglobinopathies. Maternal and fetal morbidity and mortality secondary to this serious obstetric problem can be reduced significantly with a well-devised therapeutic plan, expert perinatal teams, and careful attention to obstetric and medical detail in a setting of comprehensive tertiary health care. A variety of better medical, obstetric, and neonatal care practices have helped considerably to improve the combined life span for affected mothers and fetuses. In the absence

Painful vaso-occlusive crisis as a prodromal phase of acute chest syndrome. Is only one chest X-ray enough? A case report.

The predominant pathophysiological feature of homozygous sickle cell anemia (SCA) is the vaso-occlusion. Vaso-occlusion can be associated with painful crises, which are the primary reason for those patients to seek medical care. Vaso-occlusion is responsible for the acute chest syndrome (ACS) with large morbidity and mortality or more rarely (and especially in adults) for priapism and acute neurological events (strokes). A 10-year-old boy with homozygous SCA was admitted to the Pediatric Emergencies with painful vaso-occlusive crisis and fever. Initially he had normal chest X-ray but, after 24
