Amplitude of atrial electrical activity during sinus rhythm and during atrial flutter-fibrillation.

The onset of atrial flutter or fibrillation in a patient with a DDD pacemaker may result in sensing of the atrial arrhythmia and an inappropriate ventricular pacing response. In order to assess the potential of this problem, we evaluated the amplitude of atrial electrograms recorded from the right atrial appendage during sinus rhythm and during atrial flutter or fibrillation during 19 episodes in 18 patients. In 11 episodes of fibrillation and eight episodes of flutter, there was no difference in amplitude of either unipolar or bipolar atrial electrograms compared to that recorded during sinus

Incidence of atrioventricular block and chronic atrial flutter/fibrillation after implantation of atrial pacemakers; follow-up of more than ten years.

The incidence of atrioventricular (AV) block and chronic atrial flutter/fibrillation was determined in 41 patients, mean age 62.2 years (22 to 88 years) who received atrial pacemakers before October 1980. The mean follow-up period was 12.3 years, range 11-14.4 years. Twenty-five patients suffered from bradycardia-tachycardia syndrome, 13 symptomatic bradycardia, and 3 bradycardia related ventricular tachycardia. At follow-up: 18 patients (44%) had permanent I-AV block; 11 patients had II-AV block, 9 patients (22%) transient Wenckebach block and 2 patients (5%) transient Mobitz block, all asymp

[Oxprenolol in the treatment and prevention of cardiac arrhythmias].

Authors have analyzed the antiarrhythmic effect of oxprenolol, a beta blocking agent, in the treatment of various types of arrhythmias and in the prophylaxis of recurrences of auricular flutter and fibrillation. The results obtained in a group of 68 cases of different arrhythmias may be summarized in the following way: a) the drug efficiently reduces the ventricular rate in sinus tachycardia and in atrial flutter and fibrillation with high ventricular rate, even if resistant to treatment with digitalis; b) in patients with asynchronous pacemaker oxprenolol leads to disappearance or an importan

[Proarrhythmic effect of propafenone in patients with atrial fibrillation and atrial flutter].

We present two cases of proarrhythmia after propafenone treatment. In the first case slowing of the flutter cycle length from 205 ms to 290 ms resulted in a change from well tolerated two to one atrioventricular conduction to one to one conduction with hypotension and presyncope. In the second case organization of atrial fibrillation to atypical atrial flutter and use-dependent left bundle branch block resulted in fast broad QRS tachycardia that mimicked ventricular tachycardia.

Conduction abnormalities detected by electrophysiologic testing following repair of ostium primum atrioventricular septal defect.

Since 1983 we have performed electrophysiologic studies in 6 patients who had previously undergone repair of an ostium primum atrioventricular septal defect. Information obtained during electrophysiologic studies was crucial in guiding appropriate pacemaker therapy in these patients. As judged from the resting electrocardiogram, sinus or junctional bradycardia was present in 3/6, atrial flutter / fibrillation in 2/6, and paced rhythm in 2 patients who had had ventricular pacemakers implanted for complete atrioventricular block. During maximal exercise testing 4 patients had reduced heart rates

Case report: An irregular wide QRS rhythm with flutter waves and a positive precordial concordance.

We report a 69-year-old woman with persistent palpitation, a history of myocardial infarction. ECG showed irregular wide QRS tachycardia with positive precordial concordance and flutter waves, initially misdiagnosed as atrial flutter with accessory pathway. Direct current cardioversion restored sinus rhythm but failed to terminate tachycardia, revealing ventricular tachycardia (VT). Amiodarone resolved VT. This case highlights the challenge of differentiating wide QRS tachycardias, emphasizing QRS morphology and atrioventricular (AV) relationship analysis for accurate diagnosis.

Incidence of atrial fibrillation post-cavotricuspid isthmus ablation in patients with typical atrial flutter: left-atrial size as an independent predictor of atrial fibrillation recurrence.

INTRODUCTION: Atrial fibrillation and atrial flutter often coexist. The long-term occurrence of atrial fibrillation in patients presenting with atrial flutter alone is unknown. We report the long-term follow-up in patients who underwent cavotricuspid isthmus ablation for treatment of lone atrial flutter. METHODS AND RESULTS: Between January 1997 and June 2002, 632 patients underwent cavotricuspid isthmus ablation for the treatment of typical atrial flutter at the Cleveland Clinic Foundation. Three hundred sixty-three patients were included in this study and followed for a mean duration of 39

Right Heart and Wrong Rhythm: Atrial Flutter in Dextrocardia.

Atrial flutter is characterized by rapid atrial activity, causing an abnormal heart rhythm. Recognition and prompt management are of utmost importance since this cardiac arrhythmia could increase the risk of thromboembolic stroke and atrial fibrillation, which may lead to disability and death. Risk factors include myocardial infarction, surgery, medication, and structural heart abnormalities. One distinctive structural abnormality is dextrocardia. Herein, we present a case of a 47-year-old male who initially complains of difficulty in ambulation. Further workup showed atrial flutter with rapid

What Is the Optimal Digoxin Level? Challenging Case of Fetal Atrial Flutter Treatment in a Monochorionic Diamniotic Twin.

Background: Atrial flutter is an infrequent yet potentially fatal arrhythmia. Digoxin is the preferred first-line treatment for fetal atrial flutter due to its efficacy and favorable safety profile. The optimal digoxin serum target level for neonatal atrial flutter management remains uncertain, with the standard target level ranging from 1.0 to 2.0 ng/mL due to potential toxicity concerns above this threshold. Case Presentation: We present a case of atrial flutter in a fetus within a monochorionic diamniotic (MCDA) twin pregnancy that was successfully managed using a higher-than-standard targe

[Specific percutaneous radiofrequency ablation of atrial flutter].

Radiofrequency catheter ablation has recently been successfully applied to patients suffering from atrial flutter. We report our experience in the ablation of typical atrial flutter in 17 consecutive patients refractory to drug treatment. The procedure was initially successful restoring sinus rhythm in 16 patients (94%). During a mean follow-up of 9 months atrial flutter recurred in 4 patients (23%). A second procedure was successfully performed in 2 patients; the other two patients are presently in sinus rhythm under amiodarone which was formerly ineffective. Finally, 59% of the patients were

[The use of Cordanum in combination with Corinfar-Retard for the treatment of ectopic arrhythmias in the sick sinus syndrome in patients with ischemic heart disease].

38 patients with ischemic heart disease (IHD) and sick sinus syndrome (SSS) received combined therapy with nifedipine (Corinfar-Retard) and talinolol (Cordanum). The former drug had a positive chronotropic effect on the heart, the latter's chronotropic effect was slightly negative. All the patients had sinus bradycardia and ectopic arrhythmia which needed therapeutic correction: supraventricular and ventricular extrasystoles, fibrillation paroxysms or/and atrial flutter, paroxysmal supraventricular tachycardia, ventricular tachycardia. Cordanum was given in a dose 50 mg twice a day, Corinfar-R

Radiofrequency ablation of atrial flutter with 1:1 accessory pathway conduction improved cardiac function in a patent with PRKAG2 Cardiomyopathy. Insights with 68Ga-FAPI PET/CT imaging.

PRKAG2 syndrome is an autosomal dominant cardiac glycogenosis caused by mutations in the PRKAG2 gene, clinically characterized by the triad of ventricular hypertrophy, conduction disease, and ventricular pre-excitation, along with a high propensity for supraventricular arrhythmias. This report describes a young male carrier of the R302Q mutation who developed wide QRS tachycardia due to atrial flutter with 1:1 conduction via an accessory pathway. Electrophysiological study revealed atrial flutter activation in a counterclockwise direction around the tricuspid annulus, coexisting with a left-si

Constrictive pericarditis developed after childhood repair of ventricular septal defect.

The patient was an 18-year-old man who had undergone surgical closure of ventricular septal defect at the age of 2 years. The patient was urgently hospitalized due to syncope after longstanding uneventful postoperative course, and diagnosed with atrial flutter. Echocardiography showed poor ventricular motion and markedly dilated both atriums. After termination of atrial flutter, ventricular motion improved remarkably; however, there was little improvement in heart failure. Although we suspected constrictive pericarditis, calcification observed on computed tomography was too localized to fully

[2 or 3 level blocks in the Tawara node during atrial tachycardia].

In atrial flutter (or paroxysmal atrial tachycardia), the ventricular response is dependant on the passage through 3 superposed zones of conduction in the Tawara node, the zone of decremential conduction being the central zone N. When the ventricular response is between half and a quarter of the atrial rate there are two possible explanations: type B alternate Wenckebach period (mobitz I block in the central zone N, 2/1 block at the nodo-ventricular junction) or type A alternate Wenckebach period (Mobitz I block in the central zone N and 2/1 block at the atrio-nodal junction). These two respon

Echocardiographic diagnosis of atrial flutter in a neonate.

A pregnant female presented at 37 weeks of gestation in labor. The fetus was noted to be tachycardic on fetal monitor. Postnatally, the male neonate was still noted to be tachycardic with heart rates in the low 200 bpm range. EKG was consistent with tachycardia, but rhythm diagnosis was not definitively made. Echocardiogram with M-mode analysis clearly demonstrated 2:1 atrial to ventricular contraction pattern consistent with atrial flutter. The neonate was subsequently transferred to a tertiary pediatric hospital where the diagnosis of atrial flutter was confirmed.
