His-LBB Twig: The Origin for Premature Ventricular Complexes With Morphology Almost Identical to Sinus Rhythm.

BACKGROUND: Premature ventricular complexes with a QRS morphology almost identical to the sinus rhythm (PVC-iSR) are scarce and have been insufficiently investigated. OBJECTIVES: The purpose of this study was to explore the electrophysiology characteristics, true origin, and ablation strategy for PVC-iSR. METHODS: Among 3,804 patients referred for PVC ablation, 20 patients with PVC-iSR were identified. Detailed mapping, ablation, and analysis were performed. RESULTS: The earliest activation site (EAS) of PVC-iSR consistently recorded a sharp Purkinje potential with a Purkinje-ventricular in

Premature Ventricular Complexes After Ablation for Paroxysmal Atrial Fibrillation and Recurrent Atrial Arrhythmias: The admIRE Subanalysis.

BACKGROUND: Premature ventricular complexes (PVCs) have been reported to independently predict incident atrial fibrillation in the general population. The prognostic importance of PVCs after catheter ablation for paroxysmal atrial fibrillation (PAF) is unknown. OBJECTIVES: In patients undergoing catheter ablation for PAF, we assessed whether postablation PVCs influence the risk for recurrence. METHODS: We analyzed data from admIRE (Assessment of Safety and Effectiveness in Treatment Management of Atrial Fibrillation With the Biosense-Webster Irreversible Electroporation Ablation System; NCT0

Diving Deep into Arrhythmias: Unravelling the Impact of Underwater Environments on Premature Ventricular Complexes in Divers.

This review examines the relationship between the physiological demands of diving and premature ventricular complexes (PVCs) in divers. In the general population, some individuals have a greater tendency to experience PVCs, often without awareness or a clear understanding of the triggering factors. With the increasing availability and popularity of both scuba and apnoea diving, more people, including those with a predisposition to PVCs, are engaging in these activities. The underwater environment, with its unique stressors, may increase the risk of arrhythmogenic events, particularly PVCs. Her

Predictors of adverse outcome in patients with frequent premature ventricular complexes: The ABC-VT risk score.

BACKGROUND: No independently validated score currently exists for risk stratification of patients with frequent premature ventricular complexes (PVCs). OBJECTIVES: The purpose of this study was to develop a risk score to predict adverse events in patients with frequent PVCs. METHODS: We analyzed consecutive patients between 2012 and 2017 undergoing 14-day continuous monitoring with frequent PVCs (>5%) and concurrent echocardiography. We performed binary logistic regression to determine multivariate predictors of adverse left ventricular remodeling (left ventricular ejection fraction [LVEF] <

Risk stratification in patients with frequent premature ventricular complexes in the absence of known heart disease.

BACKGROUND: Frequent premature ventricular complexes (PVCs) can be an indicator of structural heart disease. OBJECTIVE: The purpose of this study was to determine the prevalence of scarring detected by delayed enhancement cardiac magnetic resonance (DE-CMR) imaging in patients with frequent PVCs without apparent structural heart disease and to determine the value of programmed ventricular stimulation (PVS) for risk stratification in patients with frequent PVCs and myocardial scarring. METHODS: DE-CMR imaging was performed in patients without apparent heart disease who had frequent PVCs and w

Electrocardiographic comparison of ventricular premature complexes during exercise test in patients with CPVT and healthy subjects.

BACKGROUND: Catecholaminergic polymorphic ventricular tachycardia (CPVT) is a rare but highly malignant inherited arrhythmic disorder. Although a standardized exercise stress test (ST) is the most reliable way to diagnose CPVT, in 30% only single ventricular premature beats (VPCs) were recorded. OBJECTIVE: To evaluate whether electrocardiographic characteristics of VPCs during ST distinguish patients with CPVT from healthy subjects. METHODS: Electrocardiographic characteristics of VPCs during ST in 16 calsequestrin-2 (CASQ2) mutation carriers CPVT patients were compared with that in 36 healt

Effect of deep breathing at six breaths per minute on the frequency of premature ventricular complexes.

Although the effect of reflex increase in vagal tone on the frequency of premature ventricular complexes (PVC) is known, the effect of timed deep breathing on the frequency of PVC has not been reported. We serendipitously discovered that deep breathing at six breaths per minute abolished PVC in an 18-year-old female with frequent PVC, anxiety, and palpitations. In five of a series of 10 consecutive patients with frequent (> or = 10/min) unifocal PVC, deep breathing at 6 breaths/min reduced the frequency of PVC by at least 50%. This is possibly due to increased vagal modulation of sinoatrial an

The QRS-right ventricular apex interval as a cut-off value to differentiate the origin of outflow tract premature ventricular complexes.

BACKGROUND: Many electrocardiography (ECG) criteria have been proposed for the localization of outflow tract premature ventricular contractions (PVCs); however, in some cases, it is difficult to accurately localize the origin of PVCs using the surface ECG. The authors aimed to study the QRS-right ventricular apex (RVA) interval measured during electrophysiological study and its role in the differentiation between different sites of origin of outflow tract PVCs. METHODS: The study included 90 patients (81 females, mean age 37.20 ± 7.87) referred for outflow tract PVC ablation. The authors meas

Block of branching portion of bundle of His from catheter shaft during ablation of left ventricular outflow tract ventricular premature complexes.

Ventricular premature contractions are often located in the left ventricular outflow tract (LVOT). Components of the normal atrioventricular conduction apparatus are located just below the aortic valve in proximity to the membranous septum, between the noncoronary cusp and right coronary cusp. We present a case of injury to the bundle of His during an ablation of a ventricular premature contraction in the LVOT below the left coronary cusp, remote from the bundle of His, due to pressure from the proximal shaft of the catheter between the noncoronary cusp and the right coronary cusp.

Intramural left ventricular posterior superior process premature ventricular complex ablation with intracardiac echocardiography.

Intramural premature ventricular complexes (PVCs) are notoriously challenging during catheter ablation due to the depth of the arrhythmogenic substrate. We present a case utilizing intracardiac echocardiography (ICE) integrated with 3D electroanatomical mapping to ablate an intramural PVC in a patient with methamphetamine-associated cardiomyopathy. A 22-year-old female with a history of stimulant use disorder presented with debilitating palpitations and a 46% PVC burden. PVCs originating from the posterior superior process of the left ventricle (PSP-LV) were successfully targeted under ICE gui

Early detection of right ventricular functional abnormalities in patients with complex right premature ventricular contractions.

BACKGROUND: Right ventricular (RV) premature contractions, although generally benign, may represent the first manifestation of arrhythmogenic RV cardiomyopathy. The diagnostic and prognostic value of RV functional abnormalities evidenced by equilibrium radionuclide angiocardiography (ERNA) with multiharmonic Fourier analysis has been validated in patients with severe RV arrhythmias suspected of being affected by arrhythmogenic RV cardiomyopathy. The aim of this study was to assess the prevalence of the same RV functional abnormalities in patients with frequent left bundle branch block pattern

Outflow Tract Premature Ventricular Complex Induced Cardiomyopathy: A Narrative Review.

Frequent premature ventricular complexes (PVCs) are well known to impair left ventricular systolic function. However, most patients with frequent PVCs do not develop cardiomyopathy. We review the various predictors and mechanisms of PVCs that lead to cardiomyopathy, with an emphasis on right ventricular outflow tract PVC-induced left ventricular dyssynchrony and diastolic dysfunction. The variable myocardial susceptibility to PVCs may be due to an undiagnosed subclinical cardiomyopathy. Advanced imaging modalities and genetic screening may help identify primary cardiomyopathic processes. Radio

Coupling Interval Ratio Is Associated with Ventricular Premature Complex-Related Symptoms.

BACKGROUND AND OBJECTIVES: Frequent ventricular premature complex (VPC) is one of the most common arrhythmia syndromes. Symptoms observed frequently with this arrhythmia syndrome remain limited. We sought to identify predictors of VPC-related symptoms by analyzing demographic information, VPC burden, and VPC surface electrocardiogram characteristics. SUBJECTS AND METHODS: We prospectively enrolled 109 patients with idiopathic outflow tract VPCs (63 males, 49±16 years old). They were divided into Group A (n=30, without VPC-related symptoms of palpitations or "dropped beats") and Group B (n=79,

Relation of symptoms and symptom duration to premature ventricular complex-induced cardiomyopathy.

BACKGROUND: Frequent idiopathic premature ventricular complexes (PVCs) can result in a reversible form of cardiomyopathy. In this study, the determinants of PVC-induced left ventricular (LV) dysfunction were assessed. METHODS: The subjects of this study were 241 consecutive patients (115 men [48%], mean age 48 ± 14 years) referred for ablation of frequent PVCs. One hundred eighty patients (75%) experienced palpitations and 61 (25%) did not. The PVC burden was determined by 24-hour Holter monitoring, and echocardiograms were performed to assess LV function. An LV ejection fraction of <50% was

Are antiarrhythmic agents indicated in premature ventricular complex-induced cardiomyopathy and when?

INTRODUCTION: Premature ventricular complexes (PVCs) are the most common ventricular arrhythmia that are encountered in the clinical practice. Recent data suggests that high PVC burden may lead to the development of PVC-induced cardiomyopathy (PVC-CM) even in patients without structural heart disease. Treatment for effective suppression of PVCs, can reverse PVC-CM. Both antiarrhythmic drugs (AADs) and catheter ablation (CA) are recognized treatment modalities for any cardiac arrhythmias. However, with increasing preference of CA, the role of AADs needs further defining regarding their efficacy

Beneficial Effect of Renal Denervation on Ventricular Premature Complex Induced Cardiomyopathy.

BACKGROUND: Frequent ventricular premature complexes (VPCs) can lead to the development of dilated cardiomyopathy and sudden cardiac death. Renal artery sympathetic denervation (RDN) may protect the heart from remodeling. This study aimed to investigate the effect of frequent VPCs on structural and electrical properties and whether RDN can protect the heart from remodeling. METHODS AND RESULTS: Eighteen rabbits were randomized to control (n=6), VPC (n=6), and VPC-RDN (n=6) groups. Surgical and chemical RDNs were approached through bilateral retroperitoneal flank incisions in the VPC-RDN group
