Mild Traumatic Brain Injury in U.S. Military Veterans: Results from the National Health and Resilience in Veterans Study.

OBJECTIVE: This study provides nationally representative data on the prevalence, risk factors, and associated mental health and functional outcomes of mild traumatic brain injury (mTBI) in U.S. military veterans. METHODS: Data (N = 4,069) were analyzed from the National Health and Resilience in Veterans Study (NHRVS). Analyses estimated mTBI prevalence, exposure to different mTBI injuries, and past-week mTBI symptoms (i.e. persistent post-concussive symptoms [PCS]). Comparisons were made between veterans with and without mTBI+PCS on sociodemographic, military, trauma, and psychiatric characte

Use of Veterans Health Administration Homeless Programs Among Transgender and Non-Transgender Veterans Experiencing Self-Reported Housing Instability.

INTRODUCTION: Transgender individuals have higher rates of housing instability than non-transgender individuals. This study assessed the extent to which unstably housed transgender veterans participated in Veterans Health Administration (VHA) Homeless Programs. METHODS: Using VHA medical record data for transgender (n=491) and nontransgender (n=473) veterans who self-reported housing instability during fiscal years 2013-2016, chi-square and t-tests identified differences in socio-demographic characteristics and services use; multiple logistic and negative binomial regression assessed the type

Medical utilization and cost outcomes for poststroke veterans who receive assistive technology devices from the Veterans Health Administration.

The study objectives were to (1) advance understanding of the relationship between provision of assistive technology devices (ATDs) and healthcare consumption and outcomes in a system that does not limit provision of ATDs to in-home use and (2) determine how the provision of ATDs relates to inpatient/outpatient utilization and costs of services for veterans 12 months poststroke when controlling for case-mix. This was a retrospective study using Department of Veterans Affairs administrative/workload databases to identify 12,046 veterans with stroke during fiscal years 2001 and 2002. Measures we

Social Characteristics, Health, and Mortality Among Male Centenarians Using Veterans Affairs (VA) Health Care.

We studied male centenarian Veterans using VA health care to understand the impact of social characteristics on their annual mortality rate, adjusting for prevalent health conditions. This longitudinal study used VA Electronic Health Record data from 1997 to 2012 (n = 1,858). Covariates included age, race, marital status, and periods of military service. The mean age was 100.4 ± 1.4 years, 76% were white, and 49% were married. The average annual mortality rate was 32 per 100 person-years. The annual mortality rate was stable and not affected by race but did vary by marital status. Divorced or

Monitoring mental health treatment acceptance and initial treatment adherence in veterans: veterans of Operations Enduring Freedom and Iraqi Freedom versus other veterans of other eras.

Identifying factors that influence mental health outcomes in veterans can aid in the redesign of programs to maximize the likelihood of early resolution of problems. To that end, we examined demographic and clinical process data from 2,684 veterans who scored positive on a mental health screen. We investigated this data set for patterns and possible predictors of mental health referral acceptance and attendance. The majority of patients had not received mental health treatment within the last two years (76%). Veterans of Operations Enduring Freedom and Iraqi Freedom (OEF/OIF) were more likely

Trade-Offs Between Patient Experience and Health Care Access: Comparing Veterans Health Administration Versus Community Care by Veteran Race and Ethnicity.

BACKGROUND: Racial and ethnic disparities in patient-reported experiences exist in the Veterans Health Administration (VA) and in VA-paid health care from non-VA providers ("community care"), but how these disparities compare is unknown. OBJECTIVES: We compared racial and ethnic disparities in patient-reported experiences between VA primary care and community care, and calculated differences in VA versus community care by patient race and ethnicity. METHODS: Using cross-sectional data from the 2020 to 2023 VA Survey of Healthcare Experiences of Patients-VA Primary Care and Community Care sur

Classifying Veterans' pre-pregnancy health risks using latent class analysis.

BACKGROUND: Veterans using Department of Veterans Affairs (VA) healthcare have a high burden of pre-pregnancy chronic disease that likely contributes to the observed high rate of pregnancy-related morbidity. Many common diseases frequently co-occur; understanding patterns of multimorbidity may inform the design and delivery of pre-pregnancy interventions to lower pregnancy morbidity risk. OBJECTIVE: The current study sought to identify patterns of co-occurrence of pre-pregnancy chronic disease among Veterans. DESIGN: We conducted a retrospective cohort study using VA administrative data. ME

Population-Level Health Intervention and Primary Care Quality for Veterans.

IMPORTANCE: The COVID-19 pandemic caused substantial disruptions to primary care, potentially impacting quality of care for chronic conditions. The Veterans Health Administration (VHA) implemented the Preventive Health Inventory (PHI) initiative, a multicomponent care management intervention, focused on chronic disease care and preventive screening to support the delivery of delayed care. OBJECTIVE: To examine the association of PHI implementation with measures of primary care quality and potentially avoidable health care use. DESIGN, SETTING, AND PARTICIPANTS: This cohort study used VHA adm

Respiratory Health Among U.S. Veterans Across Age and Over Time.

Many veterans and their advocates are concerned that military service may cause impaired respiratory function resulting from occupational exposures to environmental hazards (e.g., Agent Orange in the Vietnam War, burn pits in the Global War on Terror) or infectious diseases (e.g., pneumonia). Such exposures occurring in service members' young adulthoods may not have immediately discernible effects on lung health; some symptoms may emerge in the immediate months or years after separation from the military, and diagnosable conditions may not manifest until decades later. Health care providers se

Engagement in mental health treatment among veterans returning from Iraq.

OBJECTIVES: Many veterans return from combat experiencing a variety of mental health concerns. Previous research has documented a stigma associated with seeking treatment that interferes with the decision to seek treatment. This study, conceptualized using the theory of planned behavior, assessed beliefs about mental health treatment in order to understand mental health treatment seeking behavior among a group of returning National Guard soldiers who served in the war in Iraq. METHODS: Participants were one hundred and fifty Operation Iraqi Freedom National Guard soldiers who screened positiv

Comparing Suicide Rates Across Subpopulations of Veterans Receiving Veterans Health Administration Care, 2017-2021: The Suicide Risk Encyclopedia.

This study presents suicide surveillance findings from the Veterans Health Administration (VHA) Suicide Risk Encyclopedia, a novel population-based suicide rate comparison tool, demonstrating which Veteran VHA patient subpopulations experience the most elevated suicide rates. For cohorts of Veteran VHA patients alive as of the last day of 2016, 2017, 2018, 2019, and 2020 and with VHA encounters in the prior 2 years, we assessed subpopulation size, number of suicide deaths in the subsequent calendar year, and the suicide rate for 482 measures of Veteran characteristics, diagnoses, and services

Psychological Resilience in U.S. Military Veterans: Results from the 2019-2020 National Health and Resilience in Veterans Study.

Following exposure to traumatic life events, most individuals are psychologically resilient, and experience minimal-to-no symptoms of posttraumatic stress, major depressive, or generalized anxiety disorders. To date, however, most research has focused on factors associated with adverse post-trauma mental health outcomes rather than understanding those associated with psychological resilience. In particular, little is known about factors associated with psychological resilience in veterans, despite their high rates of trauma exposure, such as combat and military sexual trauma. To address this g

Hostility and telomere shortening among U.S. military veterans: Results from the National Health and Resilience in Veterans Study.

Chronic disorders of aging are critical concerns for the U.S. veteran population, which is, on average, two decades older than the non-veteran population. Characterization of risk factors that may accelerate biological aging is important in identifying targets for prevention and intervention. In the current study, we analyzed data from a contemporary, and nationally representative sample of U.S. veterans to evaluate the relationship between a broad range of sociodemographic, military, and clinical variables, and peripheral telomere length, which is an indicator of biological age and linked to
