Vitiligo management update.

Vitiligo is an acquired skin disorder caused by the disappearance of pigment cells from the epidermis, and results in well defined white patches that are often symmetrically distributed. The lack of melanin pigment makes the lesional skin more sensitive to sunburn. Vitiligo can be cosmetically disfiguring and is a stigmatizing condition, leading to serious psychological problems in daily life. It occurs worldwide in about 1% of the population, mostly between the ages of 10-30 years, and as often in males as in females. The cause is unknown, but might involve genetic factors, autoimmunity, toxi

Genital vitiligo in children: Factors associated with generalized, non-segmental vitiligo.

BACKGROUND/OBJECTIVES: It remains uncertain as to whether genital vitiligo of childhood is segmental or a forme fruste of non-segmental disease. This type of vitiligo is sometimes termed figure-of-eight disease due to female predominance and perineal and perianal hypopigmentation or depigmentation around two orifices, the anal and vaginal introitus. The objective of this series was to categorize the features of genital-limited vitiligo of childhood including comorbidities, family history, and long-term course. METHODS: IRB-approved retrospective chart review of vitiligo cases. RESULTS: Eight

Beyond vitiligo guidelines: combined stratified/personalized approaches for the vitiligo patient.

'Vitiligo' is a word that bears endless possibilities and no promises. Each vitiligo patient has a different story that demands a different therapeutic approach. Even though great efforts have been made to evaluate, study, compare and document the different therapeutic modalities available for vitiligo, clearly handling their modes of actions as well as their side effects and establishing clear stratified guidelines, numerous dilemmas are frequently met on practical grounds. 'Stabilize', 'repigment', 'depigment' or 'camouflage'? 'for whom and how do we achieve the best results' ? 'Separately o

Basic research confirms coexistence of acquired Blaschkolinear Vitiligo and acrofacial Vitiligo.

We report about a female patient with bilateral and unilateral blaschkolinear depigmentation on the extremities and coexistence of acrofacial vitiligo, who initially presented her first signs of depigmentation at the age of 32 years. The patient was otherwise healthy. The correct diagnosis was based on the latest up to date technology utilizing in vivo FT-Raman and Fluorescence spectroscopy, Wood's light examination of the depigmented skin and immunoreactivity of epidermal catalase expression in 3 mm punch biopsies from the linear depigmented area. The results yielded decreased catalase protei

A randomized interventional study that compares treatments for vitiligo and anti-vitiligo cream.

This study aimed to evaluate the efficacy of a novel combination therapy, AVC (Anti-Vitiligo Cream), compared to common treatments for vitiligo. A randomized interventional study was conducted on 1,000 patients with confirmed vitiligo, aged 7-70 years. Participants were divided into five groups (200 patients each): oral prednisolone, Tofacitinib, Ruxolitinib, AVC (Anti-Vitiligo Cream), and AVC combined with Tofacitinib. Outcomes were assessed over two years using the Vitiligo Area Scoring Index (VASI), patient satisfaction scores, and clinical observations. AVC-based therapies (Groups 4 and 5)

International Initiative for Outcomes (INFO) for vitiligo: workshops with patients with vitiligo on repigmentation.

BACKGROUND: There is no cure or firm clinical recommendations for the treatment of vitiligo. One of the main issues is the heterogeneity of outcome measures used in randomized controlled trials for vitiligo. OBJECTIVES: To define successful repigmentation from the patients' point of view and to propose how and when repigmentation should be evaluated in clinical trials in vitiligo. METHODS: We conducted three workshops with patients with vitiligo and their parents or caregivers. Workshop 1 was held at World Vitiligo Day (Detroit, MI), workshop 2 at the University of Texas Southwestern Medical

The Double Strike Hypothesis of the vitiligo pathomechanism: new approaches to vitiligo and melanoma.

Based on current research, we present a consolidated set of theses which explain the pathomechanisms of vitiligo- and melanoma-associated hypopigmentation. The main thesis is the Double Strike Hypothesis, i.e. vitiligo is caused by at least two different major pathomechanisms: an antibody-based pathomechanism and a T-cell-based pathomechanism. The antibody-based pathomechanism is dominant in diffuse vitiligo, while the T-cell based pathomechanism is dominant in localised vitiligo. Based on this main thesis we derive new therapeutical approaches to vitiligo and melanoma by using the different p

Development and Validation of Vitiligo-Specific-Belief About Therapy Questionnaire (BTQ-Vitiligo).

There is a need to capture therapy-related beliefs held by patients with conditions like vitiligo that have complex treatment regimens. Concerns about adverse effects and uncertainty about response to the prescribed treatment can impact adherence and the perceived treatment burden. This study aims to develop and validate a questionnaire on beliefs about therapy in patients with vitiligo. It is a cross-sectional study. A vitiligo-specific item pool was developed through literature review, in-depth interviews with patients with vitiligo (n = 10), and expert surveys (n = 8). The instrument was st

New insights into segmental vitiligo: A clinical and immunological comparison with nonsegmental vitiligo.

The overlaps between segmental vitiligo (SV) and nonsegmental vitiligo (NSV) suggest the underlying features of SV, which may be helpful for treating SV. In this study, 379 vitiligo patients were recruited and divided into SV (33.2%), mild-to-moderate NSV (M-NSV, affected body affected area [BSA] ≤10%, 34.0%), and severe NSV (S-NSV, affected BSA >10%, 32.7%) groups. Demographics and clinical data were collected through in-person interviews. The disease activity, progression, and prognosis were assessed through 6 months' follow-up. Serum cytokines profile and tissue-infiltrating immune cells we

Repigmentation in vitiligo: position paper of the Vitiligo Global Issues Consensus Conference.

The Vitiligo Global Issues Consensus Conference (VGICC), through an international e-Delphi consensus, concluded that 'repigmentation' and 'maintenance of gained repigmentation' are essential core outcome measures in future vitiligo trials. This VGICC position paper addresses these core topics in two sections and includes an atlas depicting vitiligo repigmentation patterns and color match. The first section delineates mechanisms and characteristics of vitiligo repigmentation, and the second section summarizes the outcomes of international meeting discussions and two e-surveys on vitiligo repigm

Vitiligo is more than skin deep: a survey of members of the Vitiligo Society.

BACKGROUND: Vitiligo is a common, largely acquired skin disease of unknown aetiology, which causes a variable amount of skin and hair depigmentation in affected people. It affects over half a million people in the UK alone, and a massive 50 million people worldwide. AIM: To quantify the psychosocial burden of vitiligo in the UK, by estimating its effect on daily life. METHODS: All members (n = 1790) of the Vitiligo Society, a UK national patient support group, were sent a questionnaire. Survey questions included demographics, disease-related characteristics, effect of vitiligo on daily life,

The definition and assessment of vitiligo: a consensus report of the Vitiligo European Task Force.

Vitiligo is the most common depigmenting disorder, which affects 0.5-1% of the worldwide population, causing disfigurement and serious disturbances in well being. There is a current lack of consensus in definition and methods of assessment of this disorder, which makes it generally impossible to compare the outcomes of different studies of the same treatment. This report summarizes the work carried out by the Vitiligo European Task Force to propose a consensus definition of the disease and to assess treatment outcomes using a system which combines analysis of extent, stage of disease (staging)

Knowledge, attitude, and practice toward vitiligo among vitiligo patients: a mediation analysis.

Knowing provoking factors of vitiligo progression and how to avoid them is essential to the successful self-management of vitiligo. Cultivating a positive attitude is also essential to avoid focusing on the negative aspects of the disease and to fuel proactive practice. This study aims to evaluate the knowledge, attitudes, and practices (KAP) of vitiligo patients and identify related factors. The study recruited 517 (54.55% male) adult vitiligo patients in Beijing and Nanjing. The mean knowledge, attitude, and practice scores were 5.45 ± 2.74 (possible range: 0-10), 23.15 ± 6.78 (possible rang

Vitiligo Treatment Impact score (VITs): development and validation of a vitiligo burden of treatment questionnaire using the ComPaRe Vitiligo e-cohort.

BACKGROUND: Vitiligo management is challenging and requires long-term adherence of patients who often complain of the burden associated with treatment. OBJECTIVE: To develop and validate a patient reported measurement of the burden of treatment in vitiligo. METHODS: The study was nested within the ComPaRe Vitiligo e-cohort, an online e-cohort of vitiligo patients in France. Items were derived from a literature review and from the qualitative analysis of a survey using open-ended questions of 204 patients with Vitiligo. Construct validity of the resulting instrument was assessed by comparing
