Qualitative review of promising practices for testing vulnerable populations at off-site COVID-19 testing centers.

BACKGROUND: Many off-site COVID-19 testing centers (OSCTCs) are struggling with strategies to serve vulnerable populations who have some of the highest rates of COVID-19 cases, hospitalizations, and deaths. Inter-OSCTC sharing of successful protocols and systems has been hampered by evolving resource constraints, the changing science of testing, and ever increasing COVID-19 case counts. OBJECTIVE: The aim of the present study was to identify promising approaches to testing vulnerable populations. METHODS: We conducted a qualitative study using semi-structured interviews with 26 leaders of OS

COVID-19 Testing in the Era of Modern Neurosurgery: Mitigating Risk in Our Vulnerable Patient Populations.

BACKGROUND: The respiratory illness identified as coronavirus disease 2019 (COVID-19) has resulted in a pandemic illness that has changed the face of healthcare. As the COVID-19 pandemic continues, patients have continued to require neurosurgical interventions, and the endoscopic endonasal approach for surgery has continued to be a mainstay treatment of pituitary tumors and anterior skull base lesions. METHODS: We sought to highlight the current lack of recommendations regarding testing protocols for neurosurgical patients. RESULTS: We implemented a novel testing protocol for our patient pop

Genomic Drivers of Multidrug-Resistant Shigella Affecting Vulnerable Patient Populations in the United States and Abroad.

Multidrug-resistant (MDR) Shigella infections have been identified globally among men who have sex with men (MSM). The highly drug-resistant phenotype often confounds initial antimicrobial therapy, placing patients at risk for adverse outcomes, the development of more drug-resistant strains, and additional treatment failures. New macrolide-resistant Shigella strains complicate treatment further as azithromycin is a next-in-line antibiotic for MDR strains, and an antibiotic-strain combination confounded by gaps in validated clinical breakpoints for clinical laboratories to interpret macrolide r

Examining Age as a Potential Moderator of Response to Reduced Nicotine Content Cigarettes in Vulnerable Populations.

INTRODUCTION: Young adults (aged 18-24 years) have a higher smoking prevalence than younger and older age groups and young adulthood is an important developmental period during which long-term behavior patterns like cigarette smoking are established. The aim of the current study was to examine how young adult smokers with additional vulnerabilities to smoking respond to reduced nicotine content cigarettes. METHODS: This is a secondary analysis of a double-blind, within-subject experiment conducted with 169 cigarette smokers recruited from populations with comorbid psychiatric conditions or so

Codesigning health and other public services with vulnerable and disadvantaged populations: Insights from an international collaboration.

BACKGROUND: Codesign has the potential to transform health and other public services. To avoid unintentionally reinforcing existing inequities, better understanding is needed of how to facilitate involvement of vulnerable populations in acceptable, ethical and effective codesign. OBJECTIVE: To explore citizens' involvement in codesigning public services for vulnerable groups, identify challenges and suggest improvements. DESIGN: A modified case study approach. Pattern matching was used to compare reported challenges with a priori theoretical propositions. SETTING AND PARTICIPANTS: A two-day

Fatal side impact crash scenarios for rear seat and seat belt-restrained occupants from vulnerable populations.

Objective: Previous studies have revealed vulnerability of school-age children and older adults in rear seats in motor vehicle crashes. Detailed information about crashes in which these fatalities occur could help improve vehicle and restraint design. Methods: Police accident reports were obtained for crashes from the Fatality Analysis Reporting System data set. Inclusion criteria were crashes in which there was at least one fatally injured restrained rear seat occupant between the ages of 6 and 12 or 55 and older in a passenger vehicle no older than 10 years at the time of the crash. Serum vitamin B12 and folate concentrations and the effect of the Mediterranean diet on vulnerable populations.

Low vitamin B12 and folate levels in expectant mothers may lead to low stores in babies. The aim of this study was to determine the frequencies of vitamin B12 and folate deficiencies in pregnant women and neonates, and to assess the effect of maternal vitamin status on babies' vitamin levels in the Aegean region of Turkey, where the Mediterranean diet (mainly fresh fruits and vegetables) is adopted. We studied 72 pregnant women and their singleton-term babies. Venous blood samples of expectant mothers were collected 1 h before delivery and cord blood of babies were obtained at birth. The mean

Beyond the Clinic Walls: Nurse-Led Socialised Care and Health Equity for Vulnerable Populations in Australia.

AIM: This paper critically examines the Australian College of Nursing's proposed socialised healthcare model and the potential of nurse-led socialised care as a paradigm shift for addressing the intersection of medical need and social distress among vulnerable populations experiencing homelessness, domestic violence, poverty, and other disadvantage. APPROACH: This is a position paper. It offers a theoretical commentary and critical reading that situates the proposed socialised healthcare model within frameworks of social determinants of health, health equity, and socially responsive care deli

Contemporary Scurvy in Vulnerable Populations in High-Income Countries: A Systematic Review and Multilevel Meta-Analysis.

Background: Scurvy, a condition caused by vitamin C deficiency, has traditionally been considered eradicated in high-income countries (HICs; World Bank classification). However, emerging evidence suggests its re-emergence within identifiable vulnerability clusters, challenging the conventional assumptions about nutritional adequacy in affluent settings. This study aimed to systematically characterize the contemporary clinical and epidemiological profile of scurvy in HICs and quantify its multisystem burden. Methods: A systematic review and meta-analysis were conducted following the Preferred R

Impact on Vulnerable Populations After Discontinuation of Continuous Glucose Monitoring in Patients With Type 2 Diabetes.

BACKGROUND: Continuous glucose monitoring (CGM) reinforces positive nonpharmacologic behaviors and optimizes pharmacotherapy. Long-term CGM may be financially unsustainable in vulnerable populations, but data on durability of benefits after short-term use are limited. OBJECTIVE: Building upon a prior study of a 3-month pharmacist-led CGM program, this study compared 1-year changes in glycemic management (glycated hemoglobin [A1c]) between patients who continued vs discontinued CGM. METHODS: This single-center pilot study, approved by University of Houston Institutional Review Board, was cond

Navigating barriers to dementia specialty care among vulnerable populations: Insight from a multidiscipline care navigation team.

The emergency department evaluates many patients with undiagnosed cognitive impairment and presents an opportune setting to facilitate early detection and referral to memory care specialists. We evaluated a novel care navigation pathway that facilitated referrals of ethnoculturally diverse individuals with suspected cognitive impairment from geriatric professionals embedded in the emergency department to dementia specialist care. We compared rates of referrals and successful appointment attendance for patients in this pathway compared to patients in a traditional primary care provider referral

The Essential Role of Medicaid in Duchenne Muscular Dystrophy Care: A Call to Protect Vulnerable Populations.

BACKGROUND: Duchenne muscular dystrophy (DMD) is the most common fatal pediatric neuromuscular disorder, resulting in premature morbidity and mortality. Multidisciplinary care and new therapies have prolonged quantity and quality of life, for which many families remain dependent on Medicaid for access to standards of care. Proposed Medicaid funding cuts would disproportionately harm children with complex medical conditions, including DMD, jeopardizing their access to essential services and specialized care. Children with DMD rely on comprehensive multidisciplinary care, including neurology, ca

COVID-19 vaccination effectiveness and safety in vulnerable populations: a meta-analysis of 33 observational studies.

Background: Even 3 years into the COVID-19 pandemic, questions remain about how to safely and effectively vaccinate vulnerable populations. A systematic analysis of the safety and efficacy of the COVID-19 vaccine in at-risk groups has not been conducted to date. Methods: This study involved a comprehensive search of PubMed, EMBASE, and Cochrane Central Controlled Trial Registry data through 12 July 2022. Post-vaccination outcomes included the number of humoral and cellular immune responders in vulnerable and healthy populations, antibody levels in humoral immune responders, and adverse events.

[Trend, spatialization and circumstances associated with violence against vulnerable populations in Brazil, between 2009 and 2017].

The scope of this study was to analyze the trend, spatialization and circumstances associated with violence against vulnerable populations in Brazil between 2009 and 2017. Descriptive analysis, spatialization and polynomial regression were used. A total of 1,116,331 cases of violence were notified, with an increase of 667.5% over the period. In all populations under research there was a predominance of physical violence against women with low levels of schooling, perpetrated by men in residences. Sexual and psychological violence was highlighted in female and LGBT populations. Negligence, howe

Caring for vulnerable medical populations: Lessons learned from Hurricane Harvey (and every other hurricane before it).

Mass care on an epic scale - we thought we had seen it all during Hurricanes Katrina, Rita, Ike and then Sandy. However, catastrophic scales reached a new level during Hurricane Harvey. How did the response and recovery to Harvey differ from previous hurricane events and what lessons were (and were not) learned? This paper examines the impact on the delivery of medical care to the vulnerable populations in Harvey's path.

Community-based supplementary feeding for food insecure, vulnerable and malnourished populations - an overview of systematic reviews.

BACKGROUND: Supplementary feeding may help food insecure and vulnerable people by optimising the nutritional value and adequacy of the diet, improving quality of life and improving various health parameters of disadvantaged families. In low- and middle-income countries (LMIC), the problems supplementary feeding aims to address are entangled with poverty and deprivation, the programmes are expensive and delivery is complicated. OBJECTIVES: 1. To summarise the evidence from systematic reviews of supplementary feeding for food insecure, vulnerable and malnourished populations, including children

Fostering a "Feeling of Worth" Among Vulnerable HIV Populations: The Role of Linkage to Care Specialists.

To address barriers to adequate engagement in medical care among people living with HIV, Wisconsin's AIDS/HIV Program created a new position, the Linkage to Care (LTC) Specialist. Specialists provide intensive, short-term case management and patient navigation services for small caseloads of individuals at high risk of disengaging with medical care. Clients are eligible if they are newly diagnosed with HIV or new to medical care, recently released from incarceration, recently out of care, nonadherent to scheduled medical care visits, or have detectable viral load while in care. Barriers and facilitators to engagement of vulnerable populations in HIV primary care in New York City.

BACKGROUND: Engagement in HIV care helps to maximize viral suppression, which in turn, reduces morbidity and mortality and prevents further HIV transmission. With more HIV cases than any other US city, New York City reported in 2012 that only 41% of all persons estimated to be living with HIV (PLWH) had a suppressed viral load, whereas nearly three-quarters of those in clinical care achieved viral suppression. Thus, retaining PLWH in HIV care addresses this central goal of both the US National HIV/AIDS Strategy and Governor Cuomo's plan to end the AIDS epidemic in New York State. METHODS: We
