Pelvic floor muscle activation in response to pressure stimuli applied to the vulvar vestibule: an observational study comparing women with and without provoked vestibulodynia.

BACKGROUND: The nature of pelvic floor muscle (PFM) involvement in provoked vestibulodynia (PVD) is poorly understood. AIM: We aimed to determine if PFM electromyographic (EMG) activity in anticipation of or response to pressure applied to the posterior vaginal fourchette differs between those with and without PVD, and if the magnitude of PFM response is associated with pressure pain sensitivity, psychological or psychosexual function. METHODS: This was an observational case-control study. Forty-two volunteers with PVD and 43 controls with no vulvar pain participated. Five on-line questionna

Localization of chromogranin, synaptophysin, serotonin, and CXCR2 in neuroendocrine cells of the minor vestibular glands: an immunohistochemical study.

Sections of vulvar tissue containing minor vestibular glands (15 sections from 14 women with vulvar vestibulitis syndrome and six control sections from five asymptomatic women) were immunohistochemically stained with antibodies against chromogranin, synaptophysin, serotonin, and CXCR2. The degree of inflammation in vestibular tissue from women with and without vulvar vestibulitis syndrome was not significantly different. All vulvar minor vestibular glands contained neuroendocrine cells that expressed chromogranin to some degree. Only one case failed to express synaptophysin. The number of cell

Interferon therapy for condylomatous vulvitis.

During a 3-year period, 30 patients were referred for symptoms of vulvar pain characteristic of the vulvar vestibulitis syndrome. Colposcopic examination with a blue filter and pathologic biopsy revealed evidence of papillomavirus in 17 patients. Interferon alpha-2b recombinant was injected intradermally into the vestibule in a specific pattern three times weekly for 4 weeks. Fifteen women responded favorably with total absence of vulvar pain. Five women reported flu-like symptoms as a result of the injections. Patients without evidence of papillomavirus failed to respond to interferon therapy

[Vulvodynia].

Vulvodynia (vulvar dysesthesia) refers to vulvar pain (burning, irritation and rawness) of the external female genitalia for more than 3 months without other dermatological or gynecological causes. The term primary vulvodynia should be reserved for vulvar vestibulitis and essential (dysesthetic) vulvodynia. Vulvar vestibulitis is characterized by dyspareunia, allodynia and vulvar erythema. Most patients are Caucasian, premenopausal and sexually active. The prevalence is estimated as high as 15%. Damage to the sympathetic nerves with an increased pain sensitivity is the likely explanation for t

[Vulvodynia].

Vulvodynia is a complex syndrome of chronic vulvar pain. It is divided into several subtypes: 1. cyclic vulvovaginitis (pain occurs after coitus), 2. vulvar vestibulitis syndrome (pain mainly with intercourse), 3. dysaesthetic vulvodynia (psychosomatic; diagnosis of exclusion), 4. vulvar dermatoses (e.g. pemphigus vulgaris, contact dermatitis). Additional causes have been described in single cases. Though vulvodynia is often accompanied by psychological distress, somatic causes have to be considered in each case.

Vulvodynia. Diagnostic patterns.

The symptom of vulvar burning demands special evaluation and management. Possible factors include complications of therapy, inflammatory dermatoses, lichen sclerosus, and infection with organisms such as Candida, human papillomavirus, and herpes simplex virus. Vulvar vestibulitis and dysesthetic vulvodynia or pudendal neuralgia have characteristic patterns of discomfort. Psychological aspects of vulvodynia include the need for supportive therapy.

[Vulvar pain syndrome (vulvodynia)--dilemmas in terminology].

BACKGROUND: In recent years there has been increasing awareness of chronic vulvar pain, both in the medical and lay communities. The etiology of vulvar pain is largely unknown. Furthermore, there is no worldwide consensus regarding the terminology and classification of this condition, which makes it difficult to compare the results of different treatments. In 2003, following more than three decades of uncertainty, the terminology and classification of vulvar pain was re-established. AIM: To review the development of nomenclature for vulvar pain and the increasing understanding of its pathophy

[Vestibulitis and dyspareunia].

Vulvar vestibulitis syndrome is defined as chronic vulvar erythema involving dyspareunia. Some infectious causes have been suggested, as human papillomavirus. Other known or suspected causes include use of chemicals or other irritants. Sexual active women of the third decade are classically exposed to this syndrome. Dyspareunia is complex with a neurophysiological cause and an emotional outcome. Patients require a medical, surgical or psychological follow-up according to each case.

Vulvodynia--a complex syndrome of vulvar pain.

Vulvodynia is a syndrome of unexplained vulvar pain, sexual dysfunction, and psychological disability. The incidence of prevalence of this condition is not known. Several subtypes of vulvodynia have been recognized. Recognition of the distinct subsets of vulvodynia is a pre-requisite for successful management. Vulvar vestibulitis syndrome, cyclic vulvovaginitis, and dysesthetic vulvodynia are the most common subtypes. Other frequently misdiagnosed vulvar or vaginal conditions which can also cause culvodynia are vulvar papillomatosis, cytolytic vaginosis, lactobacillosis, and desquamative infla

Reliability of vulvar blood perfusion in women with provoked vestibulodynia using laser Doppler perfusion imaging and laser speckle imaging.

BACKGROUND: Microvascular assessment has become increasingly used in gynecology to better understand the pathophysiology of various vulvar conditions and to study sexual function. Alteration in blood perfusion of the vulvar area has been observed using laser technologies in women with provoked vestibulodynia (PVD), the leading cause of vulvar pain. However, no studies have thus far investigated the reliability of and agreement between lasers for evaluating vulvar blood perfusion. OBJECTIVES: The main objective was to investigate the between-session reliability of the laser Doppler perfusion i

Vulvar vestibular effects of ospemifene: a pilot study.

The study aimed to assess the effects of ospemifene on vulvar vestibule in postmenopausal women with vulvar pain and dyspareunia. Fifty-five postmenopausal women used oral ospemifene 60 mg/d for 60 d. Symptoms of dryness, burning, and dyspareunia were evaluated on a 10 cm visual analog scale. Visual examination of the vulvar vestibule was also conducted. Patients also underwent current perception threshold (CPT) testing obtained from the vulvar vestibule. Fifty-five patients (94.6%) completed the treatment. Hot flashes were the most frequent adverse effects, but this led to a discontinuation o

Evaluation and differential diagnosis of dyspareunia.

Dyspareunia is genital pain associated with sexual intercourse. Although this condition has historically been defined by psychologic theories, the current treatment approach favors an integrated pain model. Identification of the initiating and promulgating factors is essential to reaching a successful diagnosis. The differential diagnoses include vaginismus, inadequate lubrication, atrophy and vulvodynia (vulvar vestibulitis). Less common etiologies are endometriosis, pelvic congestion, adhesions or infections, and adnexal pathology. Urethral disorders, cystitis and interstitial cystitis may a

Vulvitis and vulvovaginitis: cutaneous considerations.

Vulvar dermatoses (previously dystrophies) include psoriasis, allergic or irritant reactions, lichen sclerosus, lichen simplex chronicus, lichen planus, and tinea. Some of these have bullous or erosive forms, but they differ from the immune-mediated vesiculobullous disease group, which includes vulvar pemphigus, benign familial pemphigus, pemphigoid, linear IgA disease, and dermatitis herpetiformis. Vulvar ulcers can occur in dermatoses resulting from systemic disease (Behçet's syndrome, lupus, pellagra, and Reiter's disease) or malignancies resembling dermatoses (extramammary Paget's disease,
