Interobserver reliability of video recording in the diagnosis of nocturnal frontal lobe seizures.

BACKGROUND: Nocturnal frontal lobe seizures (NFLS) show one or all of the following semeiological patterns: (1) paroxysmal arousals (PA: brief and sudden recurrent motor paroxysmal behavior); (2) hyperkinetic seizures (HS: motor attacks with complex dyskinetic features); (3) asymmetric bilateral tonic seizures (ATS: motor attacks with dystonic features); (4) epileptic nocturnal wanderings (ENW: stereotyped, prolonged ambulatory behavior). OBJECTIVE: To estimate the interobserver reliability (IR) of video-recording diagnosis in patients with suspected NFLS among sleep medicine experts, epilept

Early-onset dementia with prolonged occipital seizures: an atypical case of Kufs disease.

OBJECTIVE: Kufs disease is the adult-onset form of neuronal ceroid lipofuscinosis (NCL). Its two clinical phenotypes are type A (progressive myoclonus epilepsy with dementia) and type B (behavioral abnormalities and dementia, associated with pyramidal and extrapyramidal signs). METHODS: We describe the clinical evolution of an atypical case characterized by progressive dementia and focal occipital seizures. RESULTS: A healthy 37-year-old woman began showing memory deficits and behavioral disturbances (apathy, lack of inhibitions, untidiness). After 4 years, she developed rare clusters of ton

Molecular basis for prey relocation in viperid snakes.

BACKGROUND: Vertebrate predators use a broad arsenal of behaviors and weaponry for overcoming fractious and potentially dangerous prey. A unique array of predatory strategies occur among snakes, ranging from mechanical modes of constriction and jaw-holding in non-venomous snakes, to a chemical means, venom, for quickly dispatching prey. However, even among venomous snakes, different prey handling strategies are utilized, varying from the strike-and-hold behaviors exhibited by highly toxic elapid snakes to the rapid strike-and-release envenomation seen in viperid snakes. For vipers, this mode o

Scheduled medications and falls in dementia patients utilizing a wander garden.

Little has been reported about the relationship of a dementia wander garden with scheduled psychiatric medications in addition to changes in fall number and severity. The 28 participating residents of a dementia unit were divided into high (HUG) and low (LUG) wander garden user groups and assessed for the number and severity of falls. The type and dose of scheduled psychiatric medications were monitored for 12 months before and 12 months after the wander garden was opened. Results indicated that the residents experienced about a 30% decrease for the raw number of falls and fall severity scores

Machine learning forecasts of laser intensity evolution through underwater turbulence.

This experimental study investigates whether the temporal evolution of received laser-beam intensity can be forecast from intensity images alone under controlled underwater turbulence. Conventional adaptive-optics systems commonly rely on present wavefront measurements and are therefore reactive to measured turbulence-induced distortions. We examine a different prerequisite problem: whether established deep-learning components can be assembled into short-horizon predictors of experimentally measured intensity evolution, without phase measurements or iterative wavefront reconstruction. Sex Differences in Behavioral and Psychological Signs and Symptoms of Dementia Presentation Regarding Nursing Home Residents with Cognitive Impairment Suffering from Pain - Results of the Services and Health for Elderly in Long-Term Care Study.

OBJECTIVE: Behavioral and psychological symptoms of dementia (BPSD) place a heavy burden on patients as well as caregivers. Recently, pain was identified as an important determinant of BPSD. However, it is not yet known what influence sex has on BPSD and pain. Thus, the present study aimed to identify possible associations between BPSD, pain, and sex. DESIGN: A retrospective evaluation of cross-sectional data derived from the Services and Health for Elderly in Long-Term Care (SHELTER) Study database, a cross-national European study on nursing home residents. SETTING AND PARTICIPANTS: The stu

What Happens to Disinhibited Social Engagement Disorder Over Time?

Disinhibited social engagement disorder (DSED) is one of the most intriguing syndromes in child and adolescent psychiatry but also one of the least understood. We know little about its developmental course in different groups of children, so I was delighted to see the incisive longitudinal work from the Bucharest Early Intervention Study by Guyon-Harris and colleagues.1 My own interest in DSED was sparked back in the early 1990s, when a travel bug had taken me to Guatemala and I had found a job in an orphanage. I was also the family doctor to the surrounding villages, in which children were co

Report on an open-label prospective study of divalproex sodium for the behavioral and psychological symptoms of dementia as monotherapy and in combination with second-generation antipsychotic medication.

BACKGROUND: Studies of pharmacotherapy for agitation in dementia have primarily been limited to single-drug trials and have not determined if some forms of agitated behaviors are more responsive to treatment than others. OBJECTIVE: The goal of this study was to determine if manifestations of agitation (ie, physical aggression, physically nonaggressive behavior, and verbally agitated behaviors) show different degrees of response to divalproex sodium (extended release or sprinkles) alone or in combination with second-generation antipsychotic agents. METHODS: This was a 6-week, open-label, natu

The cognitive anchor system: a structural model of self, task selection, and latent intention in human cognition.

INTRODUCTION: Human cognition requires both stability and flexibility: individuals maintain a continuous sense of self while shifting among tasks, intentions, and contexts. Existing theories explain information processing, prediction, attention, and control, but they leave under-specified the structural layer that determines what cognition is currently organized around. METHODS: This theoretical paper develops the Cognitive Anchor System, a structural framework that distinguishes among the self anchor, the task anchor, and the anchor pool. The model is compared with major theories of attentio

The evolutionary origin of psychosis.

Imagination, the driving force of creativity, and primary psychosis are human-specific, since we do not observe behaviors in other species that would convincingly suggest they possess the same traits. Both these traits have been linked to the function of the prefrontal cortex, which is the most evolutionarily novel region of the human brain. A number of evolutionarily novel genetic and epigenetic changes that determine the human brain-specific structure and function have been discovered in recent years. Among them are genomic loci subjected to increased rates of single nucleotide substitutions

Repetition of Computer Security Warnings Results in Differential Repetition Suppression Effects as Revealed With Functional MRI.

Computer users are often the last line of defense in computer security. However, with repeated exposures to system messages and computer security warnings, neural and behavioral responses show evidence of habituation. Habituation has been demonstrated at a neural level as repetition suppression where responses are attenuated with subsequent repetitions. In the brain, repetition suppression to visual stimuli has been demonstrated in multiple cortical areas, including the occipital lobe and medial temporal lobe. Prior research into the repetition suppression effect has generally focused on a sin

Aberrant early growth of individual trigeminal sensory and motor axons in a series of mouse genetic models of 22q11.2 deletion syndrome.

We identified divergent modes of initial axon growth that prefigure disrupted differentiation of the trigeminal nerve (CN V), a cranial nerve essential for suckling, feeding and swallowing (S/F/S), a key innate behavior compromised in multiple genetic developmental disorders including DiGeorge/22q11.2 Deletion Syndrome (22q11.2 DS). We combined rapid in vivo labeling of single CN V axons in LgDel+/- mouse embryos, a genomically accurate 22q11.2DS model, and 3D imaging to identify and quantify phenotypes that could not be resolved using existing methods. We assessed these phenotypes in three 22

Intramolecular energies of the cytotoxic protein CagA of Helicobacter pylori as a possible descriptor of strains' pathogenicity level.

The Helicobacter pylori cytotoxin-associated gene A (CagA) is known for causing gastroduodenal diseases, such as atrophic gastritis and peptic ulcerations. Furthermore Helicobacter pylori CagA positive strains has been reported as one of the main risk factors for gastric cancer (Parsonnet et al., 1997). Structural variations in the CagA structure can alter its affinity with the host proteins, inducing differences in the pathogenicity of H. pylori. CagA N-terminal region is characterized for be conserved among all H. pylori strains since the C-terminal region is characterized by an intrinsicall

Social mind at rest: resting-state connectivity associated with social thought relates to friendship quality.

Although mind wandering is often associated with adverse outcomes, certain thought content may support adaptive functions. This study investigated how latent dimensions of resting-state thought relate to adaptive functioning, mental, emotional, and behavioral problems, and resting-state connectivity patterns in 130 healthy adults. Exploratory factor analysis of the New York Cognition Questionnaire identified seven thought dimensions: five content-related factors (Social Cognition, Self-Oriented Reflection, Interpersonal Reflection, Near-Term Past/Future, and Positive & Meaningful) and two form

Developmental trajectories of vigilance in childhood and adolescence: Disentangling underlying mechanisms through behavioral modeling.

Sustained attention is crucial for daily functioning, yet research on vigilance development from childhood to early adulthood has yielded inconsistent findings. These discrepancies arise partly from the use of tasks with different vigilance demands and from limitations in the indices of sensitivity and response criterion used. This study examines vigilance development in a large sample of children, adolescents, and young adults (N = 809) using a novel, fine-grained analytical approach. Performance in executive vigilance was modeled using logistic curves (hits and false alarms), while arousal v

Intimate relationships and hypnosis: insecure adult attachment affects emotions and absorption during hypnosis.

INTRODUCTION: Hypnosis research indicates that subjects are not equally hypnotizable. Most studies on hypnotizability focused on the relationships with personality or cognitive variables. At the same time, only a few proposed the contribution of the attachment style, defined as the result of the childhood relationship with the caregivers and influencing the adult relations. METHODS: In the present investigation, two studies were carried out to test the possible association between adult attachment and hypnotic responsivity. The adult attachment was assessed using the Experiences in Close Rela

Frontolimbic alpha activity tracks intentional rest BCI control improvement through mindfulness meditation.

Brain-computer interfaces (BCIs) are capable of translating human intentions into signals controlling an external device to assist patients with severe neuromuscular disorders. Prior work has demonstrated that participants with mindfulness meditation experience evince improved BCI performance, but the underlying neural mechanisms remain unclear. Here, we conducted a large-scale longitudinal intervention study by training participants in mindfulness-based stress reduction (MBSR; a standardized mind-body awareness training intervention), and investigated whether and how short-term MBSR affected

Situating adult attention-deficit/hyperactivity disorder in the externalizing spectrum: Etiological, diagnostic, and treatment considerations.

Adult attention-deficit/hyperactivity disorder (ADHD) has a population prevalence of 5%. However, its prevalence is much higher in mental health and substance use treatment settings. It is associated with significant physical and psychiatric morbidity, as well as social, occupational, and legal consequences. Adult ADHD is considered to be a part of the externalizing spectrum with which it shares both homotypic comorbidity and heterotypic continuity across the lifespan. This is attributable to a shared genetic basis, which interacts with environmental risk factors such as nutritional deficienci

[An autopsy case of corticobasal degeneration mimicking frontal Pick's disease].

A pathologically proven case of corticobasal degeneration (CBD) with marked psychiatric symptoms was reported. A 59-year-old female was admitted to our hospital in October 1989 because of her "forgetfulness". Her abnormal behavior began in August 1989, and she had no psychiatric signs before that time. On this admission she was diagnosed to have panhypopituitarism due to Sheehan syndrome. Cortisone and levothyroxine were administered, but her mental symptoms deteriorated. Strange behaviors such as buying same materials every day and wandering around all day long fully dressed with all of her j

Foreign Body Appendicitis Coexisting With Ascariasis in a Pediatric Patient: A Case Report.

In clinical practice, the typical approach to ingested foreign bodies in stable patients involves expectant management, as most materials pass through the gastrointestinal (GI) tract without adverse effects. However, foreign bodies that travel through the appendix's lumen can cause acute appendicitis due to their inability to exit the colon. Rarer causes of appendicitis include parasitic infiltration by Ascaris lumbricoides. The wandering behavior of Ascaris lumbricoides within the GI tract can lead to various surgical complications in the abdomen. Occasionally, these parasites can migrate to
