Risk of the Watch-and-Wait Concept in Surgical Treatment of Intraductal Papillary Mucinous Neoplasm.

IMPORTANCE: The natural history of intraductal papillary mucinous neoplasms (IPMNs) remains uncertain. The inconsistencies among published guidelines preclude accurate decision-making. The outcomes and potential risks of a conservative watch-and-wait approach vs a surgical approach must be compared. OBJECTIVE: To provide an overview of the surgical management of IPMNs, focusing on the time of resection. DESIGN, SETTING, AND PARTICIPANTS: This cohort study was conducted in a single referral center; all patients with pathologically proven IPMN who received a pancreatic resection at the institu

Is watch and wait a safe and effective way to treat rectal cancer in older patients?

INTRODUCTION: The aim was assess the oncological and functional outcome of the watch-and-wait (W&W) approach in older patients with a clinical (near)complete response after neoadjuvant treatment for rectal cancer. MATERIAL AND METHODS: Patients were included in a W&W-approach (2004-2019) when digital rectal examination, endoscopy and MRI showed a (near)clinical complete response. Patients underwent endoscopy and MRI every 3 months during the first year, and 6-monthly thereafter. Patients aged ≥75 and ≥ 2 years of follow-up (FU) were selected. Oncological outcomes were assessed with Kaplan-Mei

'Watch and wait' as initial management for patients with follicular lymphomas: still a viable strategy?

Observation until the development of symptoms, or 'watch and wait', has long been a proposed management technique for patients with indolent lymphomas. However, investigators have found that there may be differences in outcomes for various patient groups according to histopathology, clinical features of the disease, biologic factors as yet to be fully recognized, and type of initial therapy offered. Recently, investigators from the UK National Cancer Research Institute reported that progression-free results were significantly improved when asymptomatic patients with indolent follicular lymphom

Immediate postoperative radiotherapy or "watch and wait" in the management of adult low-grade glioma?

BACKGROUND: The EORTC Trial 22845 on the role of immediate postoperative radiotherapy in patients with supratentorial lowgrade glioma revealed an advantage of immediate postoperative radiotherapy for progression-free survival, but not for overall survival. It is still an open question in which clinical setting immediate radiotherapy should be considered and whether chemotherapy may become a useful alternative. MATERIAL AND METHODS: Reports in the literature spanning 60 years of radiation therapy were reviewed with respect to timing of radiotherapy, prognostic factors, dose prescriptions, mode

Watch and Wait in practice: Adherence to surveillance after nonoperative management for rectal cancer.

BACKGROUND: Surveillance after neoadjuvant therapy and complete clinical response, known as "Watch and Wait," is an accepted treatment approach for locally advanced rectal cancer. Watch and Wait replaces surgical resection with rigorous surveillance for early identification of regrowth. However, there are scant data on adherence to surveillance. This study aimed to assess the adherence to Watch and Wait and the receipt of optimal surveillance. METHODS: A multicenter cohort of patients diagnosed with rectal adenocarcinoma from 2021to 2022 was retrospectively reviewed. Patients who underwent Wa

MRI-based assessment and surveillance in watch-and-wait for rectal cancer: a multidisciplinary review.

PURPOSE: The management of rectal cancer has evolved toward a paradigm of personalized, multidisciplinary care. The Watch-and-Wait (W&W) strategy has emerged as an organ-preservation alternative for patients achieving a clinical complete response (cCR) after neoadjuvant therapy, aiming to avoid the morbidity and functional impairment associated with total mesorectal excision (TME) while maintaining acceptable oncologic outcomes when applied under structured surveillance. METHODS: This narrative, multidisciplinary review summarizes current evidence on W&W in rectal cancer, with a particular em

An approach to anxiety during watch-and-wait for Chronic Lymphocytic Leukemia: Monitor and move on.

Chronic Lymphocytic Leukemia (CLL) is the most frequently diagnosed hematologic malignancy with the majority of patients at diagnosis in the "watch and wait" stage of treatment - language that gives the perception of an axe waiting to fall, belying the fact that up to 30% of patients will never need treatment in their lifetime. While receiving active surveillance, patients report anxiety, distress, and depression, yet there is little research capturing the experience of this patient population, nor describing interventions to improve their experience (Damen, 2022). In an effort to "do somethin

The 'inbetweeners': living on a watch and wait approach for chronic lymphocytic leukaemia - a qualitative study.

BACKGROUND: Chronic lymphocytic leukaemia (CLL) is an incurable disease; many people with the condition do not require active treatment and are monitored using a watch and wait approach. AIM: The aim of this study was to explore the experiences of people living with a diagnosis of CLL and on watch and wait. METHODS: Using a descriptive qualitative approach, seven participants on the watch and wait approach were interviewed. Data analysis was guided by systematic text condensation. FINDINGS: Participants reported anxiety, referring to 'wait and worry'. Their information needs were not met, a

Role of Local Excision for Suspected Regrowth in a Watch and Wait Strategy for Rectal Cancer.

Rectal cancer patients with a clinical complete response to neoadjuvant (chemo)radiation are eligible for Watch and Wait (W&W). For local regrowth, total mesorectal excision (TME) is considered the standard of care. This study evaluated local excision (LE) for suspected local regrowth. From 591 patients prospectively entered into a national W&W registry, 77 patients with LE for regrowth were included. Outcomes analyzed included histopathologic findings, locoregional recurrence, long-term organ preservation, and colostomy-free and overall survival. In total, 27/77 patients underwent early LE (<

A Case of Postoperative Lymphatic Ascites in Endometrial Cancer Spontaneously Resolving After Approximately One Year of Watchful Waiting.

Postoperative lymphatic ascites following lymph node dissection for gynecologic malignancies is not uncommon, although in most cases it resolves spontaneously within two to three weeks, or at most within four months. We present the case of a 73-year-old woman who underwent total hysterectomy with bilateral salpingo-oophorectomy and pelvic and para-aortic lymph node dissection for endometrial cancer. She subsequently developed a large volume of ascites, which was diagnosed as lymphatic ascites based on biochemical analysis of the ascitic fluid, cytological examination, and CT findings. An initi

Comparing Active Surveillance and Watchful Waiting With Radical Treatment Using Machine Learning Models Among Patients With Prostate Cancer.

PURPOSE: In 2021, 59.6% of low-risk patients with prostate cancer were under active surveillance (AS) as their first course of treatment. However, few studies have investigated AS and watchful waiting (WW) separately. The objectives of this study were to develop and validate a population-level machine learning model for distinguishing AS and WW in the conservative treatment group, and to investigate initial cancer management trends from 2004 to 2017 and the risk of chronic diseases among patients with prostate cancer with different treatment modalities. METHODS: In a cohort of 18,134 patients

The Natural Course of Tympanic Membrane Retractions in the Posterosuperior Quadrant of Pars Tensa: A Watchful Waiting Policy.

INTRODUCTION: Tympanic membrane retraction (TMR) is a relatively common otologic finding. Currently, there is no consensus on the optimal treatment of TMR. Some ENT-surgeons advocate surgical correction while others opt for a watchful-waiting policy. Our aim was to investigate the natural course of retraction pockets in the posterosuperior quadrant of the pars tensa in a large cohort of patients. METHODS: An observational retrospective cohort study was conducted including patients of all ages with a posterosuperior pars tensa retraction. Primary outcome measure was difference between audiomet

Watchful waiting of the neck in early stage oral cancer is unfavourable for patients with occult nodal disease.

For cT1/2N0 oral squamous cell carcinoma (OSCC), treatment of the neck is a matter of debate. Two treatment strategies were evaluated in this study: selective neck dissection (SND) and watchful waiting (WW). One hundred and twenty-three SND patients and 70 WW patients with cT1/T2N0M0 OSCC of the tongue, floor of mouth, or buccal mucosa were analysed retrospectively. Extracapsular spread (ECS), 3-year overall survival (OS), and disease-specific survival (DSS) were determined. Twenty-nine percent of SND patients and 13% of WW patients had occult nodal disease. WW-N+ patients showed thicker tumou

Evaluation of patients with bladder outlet obstruction and mild international prostate symptom score followed up by watchful waiting.

OBJECTIVES: To examine the variability of bladder outlet obstruction and mild lower urinary tract symptoms in patients with benign prostatic hyperplasia (BPH) followed up by watchful waiting. METHODS: The International Prostate Symptom Score (IPSS) has four questions related to voiding symptoms and three related to filling symptoms. Scores of 0 to 7, 8 to 19, and 20 to 35 represent mild, moderate, and severe symptoms, respectively. Over a period of 36 months the IPSS questionnaire was administered to 479 patients 50 to 81 years old (mean age 63) with BPH. A pressure-flow study was used to det

Rituximab Versus Watchful Waiting for Asymptomatic Advanced-Stage Low-Tumor-Burden Follicular Lymphoma: A Systematic Review and Meta-Analysis.

The optimal initial management of asymptomatic patients with advanced-stage low-tumor-burden follicular lymphoma (LTB-FL) remains uncertain. Although watchful waiting (WW) has historically been the standard approach, rituximab monotherapy has emerged as an alternative strategy. A systematic review and meta-analysis comparing rituximab monotherapy with WW was performed in this population. PubMed, Embase, and Cochrane Library were systematically searched for studies comparing rituximab monotherapy with WW in adult patients with advanced-stage LTB-FL. The primary outcome was overall survival (OS)
