Azole Resistance in Clinical and Environmental Aspergillus Isolates from the French West Indies (Martinique).

The emergence of azole resistant Aspergillus spp., especially Aspergillus fumigatus, has been described in several countries around the world with varying prevalence depending on the country. To our knowledge, azole resistance in Aspergillus spp. has not been reported in the West Indies yet. In this study, we investigated the antifungal susceptibility of clinical and environmental isolates of Aspergillus spp. from Martinique, and the potential resistance mechanisms associated with mutations in cyp51A gene. Overall, 208 Aspergillus isolates were recovered from clinical samples (n = 45) and envi

Lipid profiles in French West Indies sickle cell disease cohorts, and their general population.

BACKGROUND: The pathophysiology of sickle cell disease (SCD) and the variability of its clinical expression remain not fully understood, whether within or between different SCD genotypes. Recent studies have reported associations between lipid levels and several SCD complications. If lipid levels have been previously described as low in sickle cell anemia (SCA), few data have been provided for sickle cell SC disease (SCC). We designed our epidemiological study to isolate lipid levels and profiles by genotype in Guadeloupian cohorts of SCA and SCC adult patients, at steady state. We compared SC

Heterogeneous distribution of Dirofilaria immitis in dogs in St. Kitts, West Indies, 2014-2015.

In the Caribbean region, Dirofilaria immitis (heartworm) has been detected on almost all of the islands. While it is assumed to be endemic throughout the region, there is a lack of baseline data from the Leeward Islands (north-east of the Caribbean region). The objective of this study was to estimate the prevalence and the spatial distribution of heartworm disease in dogs on St. Kitts, a small 174km2 island which is part of the Leeward Islands. A cross-sectional study was conducted in 2014 with 100 dogs tested using a commercial antigen test. Dogs were from all regions of the island except the

Epidemiology of interstitial lung diseases in a native Afro-Caribbean population of French West Indies.

INTRODUCTION: Data about interstitial lung diseases (ILDs) epidemiology in the Afro-Caribbean population is lacking. Differences in incidence and prevalence compared to European populations have already been reported. The main objective of this study was to estimate ILD incidence in Guadeloupe overall and by etiology. The secondary objective was to determine the clinical, demographic, and environmental characteristics of patients with ILD. MATERIALS AND METHODS: We conducted a descriptive epidemiological study to estimate the incidence and prevalence of ILD in Guadeloupe between 2013 and 2019

Reptiles in Guadeloupe (French West Indies) are a reservoir of major human Salmonella enterica serovars.

The epidemiology of human Salmonella enterica infections in Guadeloupe (French West Indies) appears to be specific, with a higher prevalence of the subspecies enterica serovars Panama and Arechavaleta (Panama and Arechavaleta) than in other regions. A study was performed in Guadeloupe to identify the reservoir of Salmonella serovars by comparing their distribution in warm- and cold-blooded animals and in humans living in Guadeloupe and mainland France. Furthermore, a case-control study was conducted in 2012-2013 to identify the main epidemiologic risk factors for S. enterica infection among ch

A review of molar pregnancy at the university hospital of the West Indies over a 16-year period.

There is a wide variation in reported incidence, risk factors and presentation of molar pregnancy. This necessitates population-based studies to determine these parameters at the University Hospital of the West Indies, which is a referral centre for these conditions. The incidence of molar pregnancy at the University Hospital of the West Indies was found to be 2.81 per 1,000, which fell in the range of worldwide values. Partial moles made up 61.1% and complete moles 31.0%. The mean age of the patients was 28.49 years old with 85% of patients aged between 20 and 40 years old. The median gestati

A clinico-pathological study of Cushing's syndrome at the University Hospital of the West Indies and a review of the literature.

Cushing's syndrome is an uncommon but important disease. Twenty-one confirmed cases of spontaneous Cushing's syndrome were documented at the University Hospital of the West Indies over a 21-year period. They were predominantly young females (F:M ratio of 17:4; mean age 25 years and 3 months). The commonest presenting symptoms were amenorrhoea (41%) and obesity (19%). Common clinical features were cushingnoid features (95%), hypertension (76%) and hirsutism (82%). Twenty-nine per cent had frank hyperglycaemia. Cushing's syndrome was due to Cushing's disease in 10 cases, adrenal adenoma in 3 and

[Risk factors of ciguatera in the French West Indies in Saint-Barthélémy, Saint-Martin and Anguilla].

An epidemiological study on ciguatera fish poisoning in the French West Indies (St-Barthelemy, St-Martin and Anguilla) was conducted during the years 1985-1986. The investigation on intoxications shows a non seasonal significant prevalence. Though it was difficult to list the cases, the morbidity seems to vary between 7 and 30 per thousand. A study of fish toxicity was realised; 46 different species were tested (individually or by pools) by the mosquito bio-test. Observations of intoxications give the following results: High risk species: Caranx bartholomaei, C. lugubris, Seriola dumerili, Lut

The Question of the Origin of Overrepresentation of Violence in the French West Indies: A Psychosocial Approach to Attachment Issues.

The French West Indies, particularly Martinique and Guadeloupe, are grappling with high rates of violence, particularly among youth, which might be deeply rooted in historical trauma from slavery. A significant portion of the population has experienced abuse and violence in early life, prompting researchers to explore the connection with attachment theory. The transatlantic slave trade tore families apart and disrupted the secure attachments that are essential for healthy emotional development. Children raised without stable caregivers often develop insecure attachment styles, which may have b

Cardiovascular Anesthesia and Critical Care in the French West Indies: Historical Evolution and Current Prospects.

Anesthesiology, the medical specialty that deals with the management of vital functions in patients undergoing surgery, has played an important role in the successful development of cardiac interventions worldwide. Tracing the historical roots of cardiac anesthesia and critical care from its inception in the late 1950s, a paradigm shift in perioperative care has been driven by a better understanding of the mechanisms of organ dysfunction in stressful conditions and technological advances regarding surgical approach, patient monitoring, and organ protection. Although progress in cardiac anesthe

Changing patterns in the distribution of the Mayaro virus vector Haemagogus species in Trinidad, West Indies.

The Mayaro virus disease (MAYVD) is an emerging mosquito borne zoonosis that was first reported on the island of Trinidad in 1954. The viral agent for this disease is known to presently be endemic to Central and South America. The enzootic cycle of the Mayaro virus (MAYV) is not fully characterized, though primates are thought to be the main reservoir with Haemagogus species of mosquitoes as the primary vector. This virus has been responsible for several sporadic cases of infections and limited outbreaks, but it is postulated that the MAYVD will become a major epidemic in the future, following

Role of Rats ( Rattus norvegicus) in the Epidemiology of Toxoplasma gondii Infection in Grenada, West Indies.

Rodents are known to be reservoir hosts of Toxoplasma gondii infection for other animals, such as cats and pigs. From February to July 2017, 167 rats ( Rattus norvegicus) were trapped in Grenada, and serum, heart, skeletal muscle, and brain were examined for T. gondii infection by serological examination (modified agglutination test, 1:25) for T. gondii antibodies and for viable parasites by bioassay in mice. Samples of heart, skeletal muscle, and brain of all rats were bioassayed in Swiss Webster (SW) outbred albino mice and interferon gamma gene knockout (KO) mice. Toxoplasma gondii was isol

Evaluation of different serological assays for early diagnosis of leptospirosis in Martinique (French West Indies).

BACKGROUND: Leptospirosis is a potentially life-threatening but curable zoonosis whose prognosis depends on accurate and timely diagnosis. Because of its non-specific clinical presentation, laboratory testing is essential to confirm the diagnosis. Here, we aimed to assess the performance of two enzyme-linked immunosorbent assays (ELISAs) (ELISA Serion and ELISA-Hb Pasteur) and one immunodot (GenBio) using quantitative PCR (qPCR) as gold standard, instead of the traditional microscopic agglutination test, for the diagnosis of acute leptospirosis in an endemic area. Infections in neurosurgical patients admitted to the intensive care unit at the University Hospital of the West Indies.

Patients admitted to the Intensive Care Unit (ICU) are at risk of developing life-threatening nosocomial infections, especially with organisms resistant to commonly used antibiotics. Neurosurgical patients are particularly vulnerable because of the serious nature of their illness, the frequency of associated trauma and the presence of invasive devices. Of 120 neurosurgical patients admitted to the ICU of the University Hospital of the West Indies (UHWI) between September 1995 and December 1999, the records of 73 patients were available for analysis. All had prophylactic antibiotics. Twenty-one
