Elaeophorosis in bighorn sheep in New Mexico.

Two bighorn sheep (Ovis canadensis) in New Mexico (USA) were found to be naturally infected with Elaeophora schneideri. An adult ram examined in 1997 in the Fra Cristobal Mountains had 26 nematodes in the carotid and iliac arteries, and microfilariae were present in the skin, nasal mucosa, brain, and lungs. This ram was markedly debilitated prior to euthanasia and extensive crusty, scabby lesions were observed on its head. In 1998, a yearling ewe found dead adjacent to Watson Mountain near the Gila Wilderness area was found to have 13 nematodes present in its heart. This is the first report of

A Case Report of Lyme Carditis Presenting as New-Onset Atrial Flutter in the Emergency Department.

Lyme disease (LD) is the most common vector-borne disease in the United States and has several well-known pathologic sequelae if not treated, including third-degree atrioventricular block. However, an association between Lyme carditis and atrial arrhythmias has been described in some cardiology literature but has not been reported in emergency medicine or wilderness medicine literature. This case report describes an older adult with new-onset atrial flutter with rapid ventricular rate in the setting of early disseminated LD, who ultimately was treated with antibiotics, had resolution of atrial

Antihypertensive drug effects according to the pretreatment self-measured home blood pressure: the HOMED-BP study.

OBJECTIVES: To clarify whether or not the antihypertensive drug effect is proportional to the baseline pretreatment self-measured home blood pressure (HBP) in accordance with the law of initial value (Wilder's law). DESIGN: A post-hoc analysis of a multicentre clinical trial. SETTING: Outpatients across Japan with mild-to-moderate essential hypertension. PARTICIPANTS: Among 3518 randomised participants, 2423 who self-measured HBP during the pretreatment drug-free period (10-28 days after starting fixed-dose antihypertensive monotherapy) with a mean 7.0 years follow-up were eligible. Circus Venomous: an interactive tool for toxinology education.

BACKGROUND: Clinical education about envenomations and their treatment may convey clinical and zoological details inadequately or flatly. In recent years, the widespread availability of models and videos of venomous species have created unique opportunities for toxinology education. We share our experiences using a new toolkit for educating a diverse array of clinicians, students, and wilderness medicine enthusiasts. METHODS: We examined the cost, number of participants, and satisfaction data since the initiation of a portable workshop featuring high-fidelity exhibits of venomous species. Ter

Amazonian former gold mined soils as a source of methylmercury: evidence from a small scale watershed in French Guiana.

Total mercury (HgT) and monomethylmercury (MMHg) were investigated in a tropical head watershed (1 km(2)) of French Guiana. The watershed includes a pristine area on the hill slopes and a former gold mined flat in the bottomland. Concentrations of dissolved and particulate HgT and MMHg were measured in rain, throughfall, soil water and at three points along the stream. Samples were taken in-between and during 14 storm events at the beginning and middle of the 2005 and 2006 rainy seasons. Dissolved and particulate HgT concentrations in the stream slightly increased downstream, while dissolved a

Antimicrobial Activity of Bark from Four North American Tree Species.

INTRODUCTION: Although many backcountry first aid kits contain antibiotic ointment, the supply can be quickly exhausted if a patient has extensive wounds or if there are multiple patients. METHODS: We assessed the antibacterial properties of bark extract from four North American woody plant species known to native Missourians as medicinal plants (Quercus macrocarpa, Salix humilis, Pinus echinata, and Hamamelis vernalis). We tested their antimicrobial properties, with the disc diffusion technique, against four common pathogenic bacterial species: Klebsiella pneumoniae, Pseudomonas aeruginosa,

Arthropod Envenomation in North America.

Arthropods (phylum Arthopoda) account for a higher percentage of morbidity and mortality to humans than do mammalian bites, snake bites, or marine envenomation. They are ubiquitous in domestic dwellings, caves, and campsites and in wilderness settings such as deserts, forests, and lakes. Although arthropods are most intrusive during warmer months, many are active throughout the winter, particularly indoors. Arthropods are also nocturnal and often bite unsuspecting victims while they are sleeping. Encounters with humans are generally defensive, accidental, or reactive. An individual stung by an

Cluster of sylvatic epidemic typhus cases associated with flying squirrels, 2004-2006.

In February 2006, a diagnosis of sylvatic epidemic typhus in a counselor at a wilderness camp in Pennsylvania prompted a retrospective investigation. From January 2004 through January 2006, 3 more cases were identified. All had been counselors at the camp and had experienced febrile illness with myalgia, chills, and sweats; 2 had been hospitalized. All patients had slept in the same cabin and reported having seen and heard flying squirrels inside the wall adjacent to their bed. Serum from each patient had evidence of infection with Rickettsia prowazekii. Analysis of blood and tissue from 14 so

Historical Features Regarding the Neuropathic Outbreaks in Brazilian Troops in the Paraguayan War.

During the Paraguayan War, Brazilian army and navy officers attributed outbreaks of combatant limb weakness with particular features such as tingling to palustrian causes. A dietary basis was not fully suspected at the time, and the popular name beriberi had not been coined. During wartime, there was a shortage in the food supply in addition to poor environmental conditions and diarrheal diseases, and many reports of "palustrian cachexia" were made. There are also reports of the use of native flora to feed troops, as well as alcoholism. There are also accounts of the death of horses with sympt

Gemtuzumab ozogamicin in acute myeloid leukemia: past, present and future.

After being in the therapeutic wilderness for several decades, acute myeloid leukemia has been recently thrust into the limelight with a series of drug approvals. Technical refinements in production, genetic manipulation and chemical modification of monoclonal antibodies led to growing interest in antibodies-based treatment strategies. Much of the focus of these efforts in acute myeloid leukemia has been on CD33 as a target. On September 2, 2017, the U.S. Food and Drug Administration approved gemtuzumab ozogamicin for treatment of relapsed or refractory CD33+ acute myeloid leukemia. This signa

Paroxysmal Sympathetic Hyperactivity: A Structured Approach for Clinical Practice.

Paroxysmal sympathetic hyperactivity is a neurological syndrome characterized by sudden episodes of sympathetic overactivity often triggered by non-noxious stimuli. First described by Wilder Penfield in 1929, it commonly follows severe brain injury and is associated with diffuse lesions involving diencephalic and brainstem structures. Its pathophysiology is not completely understood, but it is believed to result from an imbalance between excitatory and inhibitory pathways within the central nervous system. Diagnosis is clinical, based on exclusion of alternative causes and supported by the Par

The debate on apraxia and the supplementary motor area in the twentieth century.

Hand function and apraxia are equally relevant to neurosurgeons: as a symptom, as well as through the functional anatomy of "praxis" which underlies the dexterity needed for neurosurgical practice. The supplementary motor area is crucial for its understanding. Historically, Hugo Liepmann dominated the apraxia debate at the beginning of the twentieth century, a debate that has remained influential until today. Kurt Goldstein, a contemporary of Liepmann, is regularly mentioned as the first to have described the alien hand syndrome in 1909. Wilder Penfield was a key figure in exploring the role o

Feasibility of chest ultrasound up to 42 m underwater.

After recent advancements, ultrasound has extended its applications from bedside clinical practice to wilderness medicine. Performing ultrasound scans in extreme environments can allow direct visualization of unique pathophysiological adaptations but can be technically challenging. This paper summarizes how a portable ultrasound apparatus was marinized to let scientific divers and sonographers perform ultrasound scans of the lungs underwater up to - 42 m. A metallic case protected the ultrasound apparatus inside; a frontal transparent panel with a glove allowed visualization and operation of t

Travel Medicine: Special Issues.

Risks during travel depend on travelers' comorbidities and trip activities. Travel on cruise ships, for pilgrimages or large events, or into the wilderness carries unique risks. Asking travelers about what activities they have planned or may do during travel can tailor the subsequent pretravel counseling points and help guide decisions about vaccines and drugs. Some prescriptions (eg, doxycycline for prophylaxis against leptospirosis) depend on the planned activities while abroad. Travelers should be prepared for potential emergencies and should be encouraged to research unique customs and law

Ketamine for emergency anaesthesia at very high altitude (4243 m above sea-level).

A 22-year-old woman presenting with postpartum haemorrhagic shock at 4243 m altitude required anaesthesia to identify and treat the source of bleeding. Slow intravenous administration of ketamine (0.5 mg x kg(-1)) resulted in deep anaesthesia and apnoea requiring hand ventilation for 5 min. Haemodynamic stability was maintained throughout the procedure. Haemostasis was achieved following uterine packing and suture of a second-degree vaginal tear and small cervical tear. Confusion and visual hallucinations occurred upon awakening but recovery was otherwise uneventful. Ketamine can be used for e

Congenital primary upper eyelid entropion.

A case of primary congenital upper eyelid entropion ( PCUEE ) is reviewed. The author proposes a congenital shortness of the levator muscle with anomalous insertion onto the tarsal plate as the etiology, and a new simplified surgical correction is described. The classification of PCUEE as a syndrome with multiple congenital anomalies is supported, and the need for a complete systemic evaluation is stressed. Congenital primary upper eyelid entropion is an exceedingly rare condition. In 1969, Hiles and Wilder reviewed the 13 known cases in the ophthalmic literature, and presented a new case. The

The ecological niche of reported rabies cases in Canada is similar to Alaska.

The ecology of rabies in the circumpolar North is still not well understood. We use machine learning, a geographic information system and data explicit in time and space obtained for reported rabies cases and predictors in Canada to develop an ecological niche model for the distribution of reported rabies cases in the American north (Alaska and Canada). The ecological niche model based on reported rabies cases in Canada predicted reported rabies cases in Alaska, suggesting a rather robust inference and even similar drivers on a continental scale. As found in Alaska, proximity to human infrastr

Designing Penfield: Inside the Montreal Neurological Institute.

Neurosurgeon Wilder Penfield (1891-1976) envisioned hospital architecture as a powerful medical tool. Focusing on two key interiors in the 1934 Montreal Neurological Institute (MNI)-the operating room and the foyer-this article engages newly accessible textual and material evidence to show Penfield's intense involvement in the design of the building. A unique, tri-level surgical room, with a sophisticated setup for photography, made the MNI's surgery interactive. The OR is discussed with regard to the relationship of doctors and architects and Penfield's penchant for architectural travel. Subs

Rabies outbreak in African Wild Dogs (Lycaon pictus) in the Tuli region, Botswana: Interventions and management mitigation recommendations.

We describe an outbreak of rabies in a pack of African wild dogs (Lycaon pictus) in the Limpopo-Lipadi Private Game and Wilderness Reserve in the Tuli region of south-eastern Botswana. We define the pack's behavioural response to the disease, clinical signs, and management interventions undertaken and make recommendations to mitigate against future disease outbreaks of this nature. The outbreak, which occurred in late 2014 and early 2015, resulted in the death or disappearance of 29 individuals out of a pack of 35 wild dogs. The disruption to the social structure within the pack, the behaviour

[Current status of Rift Valley fever. What lessons to deduce from the epidemics of 1977 and 1987?].

The epidemiology of Rift Valley Fever has undergone extensive revision over the last twenty years since the epidemics that occurred in Egypt in 1977 and Mauritania in 1987. From 1931 when the disease was first described until the end of the 70s, Rift Valley Fever was considered to be a relatively benign zoonoses for man that developed in domestic animals (especially sheep) periodically following heavy rainy seasons. The presumptive reservoir for the virus in latent years was an unidentified animal living in the wilderness. However the outbreaks in Egypt and Mauritania and data obtained from en
