The relationship between the First World War and neurology: 100 years of "Shell Shock".

The First World War was a global war, beginning on 28 July 1914, until 11 November 1918. Soon after the beginning of the war, there was an "epidemic" of neurological conversion symptoms. Soldiers on both sides started to present in large numbers with neurological symptoms, such as dizziness, tremor, paraplegia, tinnitus, amnesia, weakness, headache and mutism of psychosomatic origin. This condition was known as shell shock, or "war neurosis". Because medically unexplained symptoms remain a major challenge, and considering the close relationship of symptoms described in shell shock with clinica

Epidemiological Isolation as an Infection Mortality Risk Factor in U.S. Soldiers from Late Nineteenth to Early Twentieth Centuries.

It remains uncertain why most infectious disease mortalities disappeared before modern medical interventions. Historical epidemiology using prospectively collected U.S. Army data from the Civil War (1860-1861), Spanish-American War (1898-1899), and First World War (1917-1918) suggests that epidemiological isolation was a major mortality risk factor for soldiers. Morbidity and mortality due to common infections decreased progressively from 1860 to 1918, except for influenza during the 1918 pandemic. Adult measles or mumps infections are indicative of isolated rural populations and correlated wi

Venereal disease prevention.

Outlining his interest and work in the prevention of VD, especially gonorrhea and syphilis, the author addresses the questions of prophylaxis and contraceptives, together with their technological advances, and ties these two concepts into the complex fabric of human behavior, history of available therapy in both world wars as well as the economic, social, moral and national concepts of what constitutes proper emphasis: prevention or cure. Thus, the absence of therapeutic modalities spurred the United States Armed Services to utilize Credé's solution for urethral irrigation and locally applied

Shell shock and mild traumatic brain injury: a historical review.

Mild traumatic brain injury is now claimed to be the signature injury of the Iraq and Afghanistan conflicts. During World War I, shell shock came to occupy a similar position of prominence, and postconcussional syndrome assumed some importance in World War II. In this article, the nature of shell shock, its clinical presentation, the military context, hypotheses of causation, and issues of management are explored to discover whether there are contemporary relevancies to the current issue of mild traumatic brain injury. When shell shock was first postulated, it was assumed to be the product of

The Maturation of the International Health Crisis Response: The Polish Typhus Epidemic of 1916-1923 Compared to the African Ebola Virus Disease Epidemic of 2013-2016: Part I, the Polish Epidemic.

Poland suffered an epidemic of louse-borne typhus from 1916-1923, with 400,000 cases and more than 130,000 deaths. The causative factors were depressed economic conditions and a refugee crisis that engulfed Poland after World War I. The recognition of the epidemic in 1919 stimulated the creation of the League of Red Cross Societies (LRCS). However, the LCRS had limited resources, and the Poles requested help from other governments and the League of Nations (LoN). The United States sent the American-Polish Relief Expedition to conduct delousing. However, the Polish-Soviet War of 1920 disrupted

The long postwar and the politics of penicillin: early circulation and smuggling in Spain, 1944-1954.

In this paper I explore the early circulation of penicillin. I review the early distribution in Spain of a scarce product, reflect on the available sources about the illegal penicillin trade and discuss some cases of smuggling. I argue the early distribution of penicillin involved time and geography, a particular chronology of post Second World War geopolitics. Penicillin practices and experiences belong to this period, in a dictatorship that tolerated smuggling and illegal trade of other products, some, like penicillin, produced in neighbouring countries. As a commodity that crossed borders,

Back to the Future: Whole Blood Resuscitation of the Severely Injured Trauma Patient.

Following advances in blood typing and storage, whole blood transfusion became available for the treatment of casualties during World War I. While substantially utilized during World War II and the Korean War, whole blood transfusion declined during the Vietnam War as civilian centers transitioned to blood component therapies. Little evidence supported this shift, and recent conflicts in Iraq and Afghanistan have renewed interest in military and civilian applications of whole blood transfusion. Within the past two decades, civilian trauma centers have begun to study transfusion protocols based

The remarkable Dr Robertson.

Muriel Robertson (1883-1973) was a pioneering protozoologist who made a staggering number of important contributions to the fields of parasitology, bacteriology and immunology during her career, which spanned nearly 60 years. These contributions were all the more remarkable given the scientific and social times in which she worked. While Muriel is perhaps best known for her work on the life cycle and transmission of the African trypanosome, Trypanosoma brucei, which she carried out in Uganda at the height of a major Sleeping Sickness epidemic, her work on the Clostridia during the First and Se

The Greater Good: Agency and Inoculation in the British Army, 1914-18.

As the First World War progressed, rates of typhoid diminished. This was heralded as a triumph of sanitary improvement and disease protection; yet as to how the British military achieved this remains a contentious issue. Objections arose around the danger of inoculation and the unpleasant and potentially deadly side effects. Between the unaffected and the sufferers of the vaccine's side effects are the unexplored stories of the refusers. Often bizarre, their accounts include stories of unsanctioned cajoling, arrests, suspension of privileges, and even physically forced inoculation. Soldiers co

The dysentery epidemic in Poland in 1920-1921.

This article describes in general the issues related to the dysentery epidemic in 1920-1921. The current literature on the subject lacks publications presenting these issues fully. Based on historical sources from that period, including articles published in medical magazines, the incidence rate, the methods and results of the battle against that epidemic were depicted. The article represents an important contribution to a better insight in the struggle of Polish medical services with infectious diseases afflicting people in the first years following the end of World War I. It also sheds light

Canadian Military Nurse Deaths in the First World War.

This paper examines the lives of sixty-one Canadian Nursing Sisters who served during the First World War, and whose deaths were attributed, more or less equally, to three categories: general illness, Spanish Influenza, and killed in action. The response by Canadian Army Medical Corps (CAMC) physicians to the loss of these early female officers who were, in fact, Canada's first female war casualties, suggests a gendered construction of illness at work in the CAMC. While nurses tried to prove themselves good soldiers, military physicians were quick to attribute their illnesses and deaths to hor

A historical vignette (17). The illness of soldier Rik Wouters.

The Flemish painter and sculptor Rik Wouters was born in Mechelen. He died from a fibrosarcoma of the right maxillary sinus at the age of 33. At the time, he was a soldier with the 9th Regiment of the Line of the Belgian Army during the Great War and he was imprisoned in Holland. His symptoms are reviewed; some comments are made relating to the errors in diagnosis, the quality of the treatment, its poor results, the optimism of the patient and his lucidity, and finally the moving confidence demonstrated by him in his successive physicians (Figures 1-4).

[Prostitution and Venereal Diseases in Dubrovnik During the First World War (1914–1918)].

The article examines the issue of people suffering from sexually transmitted diseases during the First World War, based on the archival material from Dubrovnik hospital found in the State Archives’ “Hospital – unsettled material” fund. In the 19th century, Dubrovnik society blamed the existence of non-institutionalized prostitution for the high increase in individuals suffering from venereal diseases. The emergence of brothels was intended to lead to disease control. At the beginning of the First World War, with the number of soldiers increasing and the proximity to the Montenegrin battlefield

Pierre Marie, 1916-1917: Functional radiographic imaging of vision and aphasia.

This article describes how Pierre Marie developed a radiographic method to localize functional areas in the brain of French World War I soldiers having sustained a penetrating craniocerebral injury. The brains cadavers were removed from their skulls and lead wires were placed in the Rolandic, Sylvian, and calcarine fissures, and major sulci. The brains were returned into their skulls and x-rays were taken using the same size and magnification used clinically in visually impaired or aphasic soldiers. The position of the wires outlining the fissures and sulci were averaged and traced on a sheet

Spanish Flu in Tehran from 1918 to 1920.

The Spanish flu spread from September 23, 1918 to 1920. This disease was one of the historical catastrophes in Iran, and a large number of people in Tehran were infected. Evidence also shows that 5000-10000 out of the 250000 infected people died in Tehran over three years. Besides, an increase was detected in the prevalence of other diseases such as pericarditis, orchitis, mastoiditis, meningitis, optic neuritis, paralysis of the palate, mania, cholera, and dysentery. Overall, five percent of the city were destroyed, and the population and economic development were severely damaged. This study

Shell Shock: The Response of UK Neurology.

The neurological and psychological manifestations of trauma, confinement, and terror became apparent throughout Europe as soldiers were evacuated from the trenches of the Western Front. The response in the UK evolved as a result of the experience of medical staff embedded with the troops in base hospitals and the philosophy of those treating returned soldiers in specialist establishment. There were widely disparate approaches to the management encompassing simple supportive care, a psychanalytic approach and radical electric shock therapy. The latter was partially driven by the Queen Square ex
