[Gunshot wound with a bullet lodged in the prostate].

OBJECTIVE: We describe a most unusual urological emergency: a firearm wound in which the blunt-nosed hand-gun bullet was lodged in the prostate. A bibliographic search of the literature revealed no similar case. METHODS: The patient was admitted presenting a gunshot wound with the entrance hole on the right buttock. There was no exit wound. An X-ray revealed the bullet behind the pubic symphysis. We introduced a suprapubic catheter and then carried out a laparotomy making a discharge colostomy Urethrogram revealed a pathway of contrast to the rectum. Several days later, we reached the bladder

Meningitis following gunshot wound of the neck.

It is generally assumed that a missile fired from a gun is subjected to sufficient heat to render it sterilized. For this reason, retained bullets are not usually considered a source of infection. The infectious complications associated with gunshot wounds are typically attributed to perforation of a hollow viscus with leakage of gastrointestinal contents causing peritonitis or intra-abdominal abscess. There are several reports of bacterial meningitis involving the spinal cord in gunshot wounds that perforate the intestine prior to involving the thoracic or lumbar vertebral column; however, th

Hypopituitarism secondary to transfacial gunshot wound.

Hypopituitarism secondary to penetrating head trauma is extremely rare, and its diagnosis may be delayed for several years. We present a patient who developed hypopituitarism secondary to a transfacial gunshot wound and who experienced damage to the hypophysis secondary to transmission of energy and bullet fragments. The importance of a computerized tomographic scan of the head in facial gunshot wound is discussed.

[An unusual case of gunshot wound to the head].

We are going to present a special case of head injury caused by a gunshot. In this case it resulted in a fracture of the skull, but the bullet did not penetrate the skull. It was deflected by the bone, leaving the body at an angle. But bone fragments, acting as secondary bullets, penetrated the brain. In spite of a massive cerebral trauma and brain injury, no retrograde amnesia could be diagnosed. The patient recovered to such an extent, that he could return to his former job. Special characteristics of head injuries caused by bullets will be referred to.

Pathological changes of gunshot wounds at various intervals after wounding.

Pathological changes of soft tissue wounds inflicted by high-velocity bullets (M 193, 5.56 mm in calibre) and middle-velocity bullets (T 56, 7.62 mm in calibre) were observed 6, 24 and 72 hrs after wounding in 40 dogs. It was noted that the extent of tissue damage would not increase further with the passage of time. Pathological changes of gunshot wounds caused by either high-velocity or middle-velocity bullet were essentially the same. However, in the high-velocity gunshot wounds, there were larger primary wound tracks, more damages to the contusion zone and more marked destructions of ultras

Assessment of Early and Late Management of Submental Gunshot Wounds.

Background: Management of submental gunshot wounds is becoming more common and requires complex surgical decisions. Objective: Compare outcomes of early and definitive reconstructive techniques following submental gunshot wounds. Methods: Retrospective chart review evaluated subjects who sustained a self-inflicted submental gunshot. The incidence of complications requiring unplanned operations was compared for early management techniques of skin/soft tissue, bone, and mucosal lining and definitive reconstructive techniques using Fisher's or Pearson Chi-square exact test with p ≤ 0.05 considere

Cutaneous skin defects overlying tangential gunshot wounds: A case series.

In examining deaths resulting from gunshot wounds, it is important to describe the wound pathway created by the projectile. Forensic pathologists must identify all entrance and exit wounds and account for all projectiles. Occasionally, confusion arises as to whether a skin defect represents an entrance wound or not, an exit wound, or some other type of wound. Herein, we propose the descriptor "bullet track skin defect" as a formal description for a superficial skin defect created along the pathway of a projectile within the body. Specifically, we define the "bullet track skin defect" as a part

Outcomes of civilian pediatric craniocerebral gunshot wounds: A systematic review.

BACKGROUND: Pediatric craniocerebral gunshot injuries (CGIs) occur both in the context of accidental and intentional trauma. The incidence and physiology of pediatric CGIs merit reexamination of prognostic factors and treatment priorities. This study characterizes the current understanding of mortality and prognostic factors in this patient population. METHODS: A systematic search was conducted. Selection criteria included all studies published since 2000, which described civilian isolated CGIs in pediatric patients. Data were analyzed qualitatively and quantitatively to identify factors prog

[Epidemiology of gunshot wounds at Ciudad Juárez, Chihuahua General Hospital].

BACKGROUND: Since 2007, Ciudad Juárez, Chihuahua has been considered as one of the most violent cities in the world. The General Hospital in this city is the main facility where patients with gunshot wounds are taken. The increased number of admissions of patients with these injuries to many hospitals in the country deserves special attention, as it has an impact on hospital resources and management protocols. OBJECTIVES: To disseminate the epidemiology of fractures caused by gunshot wounds and the hospital care of these patients. METHODS: A retrospective, observational cohort study was cond

Locked intramedullary nailing of metacarpal fractures secondary to gunshot wounds.

PURPOSE: To investigate the results of fixation of 10 metacarpal shaft fractures secondary to low-velocity gunshot wounds with locked intramedullary nails. METHODS: We reviewed the results of 10 patients with locked intramedullary nailing of the metacarpal for low-velocity gunshot wounds. Autogenous bone grafting was used in 9 of the 10 fractures. The follow-up period averaged 26 months. The parameters evaluated included angulation, rotational alignment, shortening of the digit, postoperative metacarpophalangeal (MCP) range of motion, and time to union. RESULTS: Nine of 10 fractures showed c

Current treatment of gunshot wounds to the hip and pelvis.

Gunshot wounds to the pelvis can result in injuries to various organ systems. These injuries require the care of a multispecialty trauma team at a trauma center. Orthopaedic care includes wound treatment, fracture stabilization, and infection prophylaxis. Wound care is determined by the personality of the injury, and not solely based on the velocity of the missile. Antibiotic treatment usually is indicated for patients with gunshot wounds to the pelvis, but is controversial in patients with low-energy wounds. Fractures are treated based on their stability. Regional consideration of pelvic guns

Operative management of civilian rectal gunshot wounds: simpler is better.

Extraperitoneal rectal gunshot wounds have been managed with a variety of methods from simple diverting colostomy to combinations of rectal repair, proximal diversion, transperitoneal or presacral drainage, and distal bowel irrigation techniques. Treatment methodology is chosen based on anecdotal experience, and there is no clear evidence that any technique is superior to the others. The objective of this study was to compare 3 methods of managing civilian extraperitoneal gunshot wounds. Retrospective analysis of 30 consecutive patients with extraperitoneal rectal gunshot wounds was undertaken

Neurotologic evaluation of facial nerve paralysis caused by gunshot wounds.

Facial nerve injury is one of the most common neurotologic sequelae of a gunshot wound (GSW) to the head or neck. However, few neurotologic studies have been performed on the nature and time course of such facial nerve impairments. This study was designed to characterize the neurotologic manifestations and time course of facial nerve paralysis caused by GSWs to the head and neck. We conducted a battery of electrodiagnostic tests on 10 patients who had experienced traumatic facial paralysis due to a GSW to the head or neck. The etiologies of facial nerve paralysis--including direct injury, comp

Gunshot wounds to the buttocks: predicting the need for operation.

BACKGROUND: Gunshot wounds to the buttocks may cause significant intra-abdominal injuries. Policies of aggressive abdominal exploration or extensive diagnostic testing have been suggested to avoid delays in treatment and consequent morbidity. Our group has recently suggested that clinical examination is a safe and reliable tool for triaging patients with anterior and posterior abdominal gunshot wounds. OBJECTIVE: This study was undertaken to test the hypothesis that patients with gunshot wounds to the buttocks can be managed selectively on the basis of clinical findings. SETTING: A large aca

Thoracic gunshot wounds in children under 17 years of age.

Over the past decade there has been a significant increase in the incidence of gunshot wounds (GSW) among Americans under 19 years of age. Despite the increase, pediatric GSW have received little attention in the literature, and no study has focused on pediatric victims of thoracic GSW. In the present study, the authors performed a retrospective review of the records of 51 patients under 17 years of age who were treated for thoracic GSW at adjoining level I pediatric and adult trauma centers between July 1987 and June 1995. The primary catchment area for these institutions is a lower socioecon

Anterograde interlocked nailing of distal femoral fractures after gunshot wounds.

Standard treatment for distal femoral fractures has been with blade plate or condylar screw plate devices. Excellent fixation is obtained, but bone grafting is required and the procedure exposes the fracture site. More recently, indirect methods of reduction have been initiated to limit the devascularization of the fracture site. Anterograde interlocked nailing has the advantage of using an incision far from the fracture site, and the technique is familiar to most surgeons. It can be performed regardless of the degree or length of comminution. Thirty-eight patients 16-50 years of age who under

Gunshot wounds to the spine associated with a perforated viscus.

The cases of 42 patients with low-velocity gunshot wounds to the spine with an associated perforated viscus were reviewed. All viscus perforations occurred prior to the spinal injury. There were a total of 51 perforations, including 14 of the colon, 15 of the small bowel, 15 of the stomach, five of the esophagus, and two of the pharynx. All patients had significant neurologic deficits, with 23 patients suffering a complete neurologic injury. Average clinical follow-up was 18 months (range: 4-64 months). Only three patients developed documented spinal or paraspinal infections. One case of acute
