Temporal approach for reduction of zygomatic fractures. Clinical results and advantages of the technique.

We have used closed reduction based on Gillies' method using an original Y-shaped zygomatic elevator in 50 cases after radiographic evaluation of the displacement by three directional radiography. No patients had residual restriction of mandibular motion, double vision or facial asymmetry. Radiography showed poor reduction in two cases, and displacement of the initial reduction in three. In all other cases, satisfactory results were achieved.

Three-dimensional intraoperative computed tomography imaging for zygomatic fracture repair.

OBJECTIVES: Zygomatic complex (ZMC) fractures comprise up to 40% of all facial fractures. Misaligned bone fragments and misplaced fixation hardware traditionally detected postoperatively on plain radiographs of the skull might require re-operation. The intraoperative O-Arm (Medtronic, USA) is a three-dimensional (3D) computed tomographic imaging system. MATERIALS AND METHODS: This retrospective single-center study evaluated the utility of O-Arm scanning during corrective surgeries for ZMC and zygomatic arch (ZA) fractures from 2018 to 2020. Three females and 16 males (mean age, 31.52 years; r

Prevalence of Ocular Complications in Patients with Zygomatic Bone Fractures in an Iranian Population.

OBJECTIVES: Damages to the middle third of the facial bone generally involve the orbital skeleton and can lead to eye impairment. In this study, it is attempted to determine the incidence of ophthalmic injuries in maxillofacial trauma with zygomatic bone fractures. MATERIALS AND METHODS: One hundred and fifteen cases with ophthalmic (ocular) involvement after maxillofacial trauma were referred to the Shariati Hospital, Tehran, Iran, and were visited at the Ophthalmology Department between 2016 and 2018. Zygomatic fractures and resulting ocular complications were evaluated in 87 males and 28 f

Treatment of Isolated Zygomatic Arch Fracture: Improved Outcomes with External Splinting.

BACKGROUND: The various approaches to reduction and fixation of isolated displaced zygomatic arch fractures have not been well studied. The authors compared established treatment methods for zygomatic arch fractures for both aesthetic and functional outcomes. METHODS: Consecutive patients with isolated zygomatic arch fractures with a minimum of 6 months' follow-up were studied in five groups: group 1, intraoral approach (no fixation); group 2, temporal approach (no fixation); group 3, coronal flap plate fixation; group 4, external splint fixation; and group 5, no surgery. Perioperative compli

Maxillary Sinus Mucocele as a Late Complication in Zygomatic-Orbital Complex Fracture.

This article presents an unusual case of maxillary sinus mucocele as a late complication of zygomatic-orbital complex fracture, 23 years after the initial treatment. The patient was referring diplopia and decreased visual acuity with signs of dystopia, proptosis, and epiphora. Computed tomographic scan revealed an expansive lesion in the maxillary sinus with surrounding bone erosion and displacement of the eyeball. Treatment option was excisional biopsy and orbital floor reconstruction with titanium mesh restoring the appropriate orbital position. We propose that in the case of postoperative o

A multi-outcome analysis of the effects of treatment timing in the management of zygomatic fractures.

The aim of this study was to analyze the effects of surgical treatment delay in the management of zygomatic fractures. A retrospective case series of 99 patients was undertaken. Four outcome measures were analyzed in relation to delay: facial symmetry, facial scarring, trismus, and radiographic outcome. Five additional variables were subsequently analyzed: operation, diagnosis, primary operator, regular alcohol use, and regular cigarette use. Statistically significant associations were found between delay and facial scarring, and delay and radiographic outcome. For each additional delay of a d

[Zygomatic arch fracture extended to the glenoid fossa: a cause of TMJ pain].

INTRODUCTION: Fractures of the zygomatic arch extended to the glenoid fossa are rare and seldom reported. OBSERVATION: A 26-year-old man with a slightly dislocated fracture of the zygoma associated with trismus, and localized pain in the right Temporo-Mandibular Joint (TMJ) on palpation, but without any condylar fracture. The craniofacial CT scan revealed a fracture of the zygoma extended to the glenoid fossa. The treatment was an intraoral reduction of the zygoma and screw fixation of the glenoid fossa lateral region. DISCUSSION: The risk of delayed healing by mobilization of the bone fragm

A 10-year analysis of the "Amsterdam" protocol in the treatment of zygomatic complex fractures.

Despite many publications on the epidemiology, incidence and aetiology of zygomatic complex (ZC) fractures there is still a lack of information about a consensus in its treatment. The aim of the present study is to investigate retrospectively the Amsterdam protocol for surgical treatment of ZC fractures. The 10 years results and complications are presented. The study population consisted of 236 patients (170 males, 66 females, 210 ZC fractures, 26 solitary zygomatic arch fractures) with a mean age of 39.3 (SD: ±15.6) years (range 4-87 years). The mean cause of injury was traffic accident follo

Ocular reconstruction after zygomatic complex fracture with retention of a foreign body.

Facial injuries with the retention of foreign bodies inside the tissues, both in soft and hard ones, can cause major functional and aesthetic damage. Among the different etiological agents, cutting tools, fragments of a firearm, the splinter of wood, steel, or iron, launched by misuse, or even caused by defects in equipment, are the main cause of these injuries. The aim of this study was to discuss the peculiarity of the multidisciplinary approach in caring of a 33-year-old man, victim of an accident at work, by the rupture of an emery disc and consequent penetration of the fragments in violat

Odontogenic infection leading to orbital cellulitis as a complication of fracture of the zygomatic bone.

We describe a 51-year-old man in whom chronic maxillary sinusitis developing from a deep periodontal pocket, at 26, gave rise to cellulitis of the left orbit. The immediate cause was a fracture of the left zygomatic bone with some displacement of the infraorbital margin and the orbital floor. Treatment consisted of drainage and antibiotic medication. The zygomatic bone fracture was not reduced. Eye movements returned to normal and visual acuity was not permanently affected.

Loss of vision associated with surgical treatment of zygomatic-orbital floor fracture.

A case of visual loss associated with surgical repair of a zygomatic-orbital floor fracture is presented. A review of the literature indicates relatively few cases of blindness reported in association with surgical intervention for these fractures; however, it must be recognized that this serious complication does occur. The patient's ocular status should be evaluated before, during, and after orbital surgery.

Intraoperative cone beam assessment after reduction of zygomatic fractures helps in taking a decision on associated orbital floor fractures.

INTRODUCTION: Orbital floor fracture (OFF) are frequently associated with zygomatic fractures (ZF). The reduction of the ZF may modify the features of the associated OFF. Intraoperative cone beam CT (CBCT) has shown diagnostic performance of OFF. The aim of our study was to evaluate how intraoperative CBCT control made after the reduction of ZF may help to take a decision on the associated OFF. MATERIAL AND METHOD: Patients with a unilateral displaced ZF associated with an OFF were consecutively included during a 3-year period. Intraoperative CBCT, systematically performed after reduction of
