We report on a patient operated on for degenerative myxomatous mitral and tricuspid valve disease (Barlow's disease) and aortic root dilatation. A valve repair operation and the postoperative course were uneventful. Multigenerational genetic analyses revealed two different mutations in the transforming growth factor beta-2 gene in the same patient. The two mutations in different exons were inherited from both parents each. None of the parents presented with either valve dysfunction or aortic root dilatation. This rare case illustrates potentially common genetic and signaling pathways of concom

BACKGROUND: The ASE (American Society of Echocardiography) guidelines recommend the presence of at least 4 specific criteria to confirm a diagnosis of definite severe mitral regurgitation (MR). However, the performance of these criteria, particularly when <4 are present, remains unclear.
OBJECTIVES: This study aims to evaluate and optimize the ASE native MR guidelines by comparing them with quantitative cardiac magnetic resonance (CMR), considered the reference standard for MR assessment.
METHODS: In this prospective study, 177 patients underwent near-simultaneous CMR and transthoracic echoc

Double-chambered right ventricle is an uncommon congenital anomaly consisting of obstruction of the trabecular zone of the ventricle by an elevated, hypertrophied moderator band, with or without additional muscle bundles(s). During an 18 month period (August 1984 to February 1986) 12 patients underwent repair of double-chambered right ventricle. Their ages ranged from 13 months to 12 years (mean 4.4 years). Associated lesions were ventricular septal defect in 11 patients (three closed spontaneously), atrial septal defect in two, left superior vena cava in three, aortic valve prolapse in one, a

Marfan syndrome (MFS) is a systemic disorder of connective tissue with autosomal dominant inheritance. The diagnosis of MFS is based on the identification of a combination of clinical manifestations in the ocular, musculoskeletal, and cardiovascular organ systems defined in the Ghent Nosology (De Paepe et al, 1996). Confirmation of the diagnosis in an individual requires the presence of major clinical manifestations in at least two organ systems associated with involvement of a third organ system. In relatives of an affected proband, major involvement of one organ system and involvement of a s

BACKGROUND: Barlow's mitral valve (MV) disease remains a surgical challenge. We compared short- and medium-term outcomes of neochordal ("loop") versus edge-to-edge ("Alfieri") minimally invasive MV repair in patients with Barlow's disease.
METHODS: From January 2009 to April 2014, 123 consecutive patients with Barlow's disease (defined as bileaflet billowing or prolapse [or both], excessive leaflet tissue, and annular dilatation with or without calcification) underwent minimally invasive MV operations for severe mitral regurgitation (MR) at our institution. Three patients (2.4%) underwent MV

4DE (i.e., dynamic three-dimensional) echocardiography is a new developing technique in recent years. In our study, a three-dimensional echo scan computer system was used to acquire and store the two-dimensional information, then to reconstruct the stereoscopic image of the heart according to its space-time continuum. It can yield a better approach, which can help identify the various structures of the heart and great arteries and facilitate understanding of spatial relations and motion. In addition, it can display physiologic information such as the direction, course, size, and shape of the b

Sudden athlete death (SAD) is a widely publicized and increasingly reported phenomenon. For many, the athlete population epitomize human physical endeavour and achievement and their unexpected death comes with a significant emotional impact on the public. Sudden deaths within this group are often without prior warning. Preceding symptoms of exertional syncope and chest pain do, however, occur and warrant investigation. Similarly, a positive family history of sudden death in a young person or a known family history of a condition associated with SAD necessitates further tests. Screening program

Atherosclerotic CAD is the most common cause of cardiac chest pain in Western countries. Other cardiac syndromes may also cause anginalike pain and may be difficult to differentiate from atherosclerotic CAD. It is essential to make this distinction, because management and prognosis of these conditions are entirely different. A detailed history and, in some cases, special diagnostic methods can help make the diagnosis. When evaluating patients with anginalike chest pain and normal coronary arteries, physicians need to consider this group of diseases and tailor workup and diagnosis on an individ

Heart murmurs, especially the mitral regurgitant murmurs of 40 patients were analyzed using the fast Fourier transformation technique. 1. Three types of frequency spectral pattern of mitral regurgitation (MR) were demonstrated: A) broad, spanning 100 to 500 Hz, B) narrow, characterized by one giant peak, and C) two peaks. The reason for these patterns was not clear, but they may be related to various hemodynamic events. 2. The mean frequency (f) in MR was 295 +/- 38 Hz and it increased in proportion to the regurgitant grade: e.g., Sellers II, 258 +/- 27 Hz; Sellers III, 294 +/- 23 Hz; and Sell

Sudden death in infants is rarely due to cardiac disease. Sudden death in children and adolescents is usually associated with cardiac disease. The major congenital lesions associated with sudden death are aortic stenosis, cardiomyopathies, idiopathic hypertrophic subaortic stenosis, Eisenmenger's syndrome, and cyanotic congenital defects with pulmonary stenosis or atresia. There is a high incidence of postoperative sudden death, especially associated with repair of tetralogy of Fallot and transposition of the great arteries. Cardiac arrhythmias, including the long Q-T syndrome, sick sinus synd

The echocardiographically visualized mitral valve motion pattern has been shown to be related to left ventricular diastolic parameters and systolic performance. In order to analyze the influence of a negative inotropic agent (penbutolol, Betapressin) and a positive inotropic drug (AR-L 115 BS, Vardax) on the movement of the anterior mitral valve leaflet (aMV), the following motion phases of the aMV were determined in 23 patients following completion of diagnostic cardiac catheterization: DE slope, EF slope, the final closing velocity of the aMV (AC/BC slope), CE amplitude, AC interval, and PQ-

INTRODUCTION: Marfan syndrome (MFS) and related connective tissue disorders (CTDs) are increasingly recognised. Genetic testing has greatly improved the diagnostic outcome/power over the last two decades. In this study we describe a multicentre cohort of adults with MFS and related CTDs, with a particular focus on results from genetic testing.
METHODS: All patients with MFS and related CTDs were identified from the databases of five centres in the canton of Zurich. Echocardiographic and clinical findings including systemic Marfan score, use of medication and genetic results were retrospective

The present study sought to determine whether myocardial fatty acid metabolism as assessed with iodine-123-labeled 15-(p-iodophenyl)-3-R,S-methylpentadecanoic acid (BMIPP) scintigraphy is impaired in patients with aortic valve disease (AVD) and whether the degree of the metabolic abnormality reflects the severity of AVD. BMIPP scintigraphy was performed in 12 patients with aortic stenosis (AS), 14 patients with aortic regurgitation (AR), and 9 healthy volunteers, and from that the heart-mediastinum uptake ratio (H/M ratio) corrected by the left ventricular (LV) mass (U/Mass ratio) and the myoc

Valvular heart diseases (VHD) are one of the major causes of cardiovascular diseases that are having high mortality rates worldwide. The early diagnosis of VHD prevents the development of cardiac diseases and allows for optimum medication. Despite of the ability of current gold standards in identifying VHD, they still lack the required accuracy and thus, several cases go misdiagnosed. In this vein, a study is conducted herein to investigate the efficiency of deep learning models in identifying VHD through phonocardiography (PCG) recordings. PCG heart sounds were obtained from an open-access da

BACKGROUND: Transcatheter closure is a well-established therapy for patients with perimembranous ventricular septal defects (VSDs), but with limited experience in intracristal VSDs (IVSDs) with aortic cusp prolapse (ACP).
METHODS AND RESULTS: From 2012 to 2014, we reviewed 38 patients with IVSDs complicated with mild ACP who underwent device closure, and, in light of the findings, assessed the effect of transcatheter intervention on preoperative mild ACP. The zero eccentric VSD occluder was chosen for closure (Shanghai Shape Memory Alloy Ltd, Shanghai, China). The mean defect was 4.8±1.6 mm (

BACKGROUND: MicroRNAs (miRNAs) are small RNAs regulating gene expression post-transcriptionally. While acquired changes of miRNA and mRNA profiles in cancer have been extensively studied, little is known about expression changes of circulating miRNAs and messenger RNAs (mRNA) in monogenic constitutional anomalies affecting several organ systems, like Marfan syndrome (MFS). We performed integrated miRNA and mRNA expression profiling in blood samples of Marfan patients in order to investigate deregulated miRNA and mRNA networks in these patients which could serve as potential diagnostic and prog
