Successful treatment of postoperative nonobstructive recurrent cholangitis by tract conversion surgery after total pancreatectomy: a case report.

BACKGROUND: Postoperative cholangitis is a complication of biliary reconstruction during hepatobiliary pancreatic surgery. Most cases are associated with anastomotic stenosis, but there are also cases of cholangitis without stenosis, and treatment can be difficult, especially in patients with recurrent symptoms. In this report, we describe a case of repeated nonobstructive cholangitis in a patient after total pancreatectomy, in which a good outcome was obtained after performing tract conversion surgery. CASE PRESENTATION: The patient was a 75-year-old man. He underwent total pancreatectomy fo

Barrett's Metaplasia Progression towards Esophageal Adenocarcinoma: An Attempt to Select a Panel of Molecular Sensors and to Reflect Clinical Alterations by Experimental Models.

The molecular processes that predispose the development of Barrett's esophagus (BE) towards esophageal adenocarcinoma (EAC) induced by gastrointestinal reflux disease (GERD) are still under investigation. In this study, based on a scientific literature screening and an analysis of clinical datasets, we selected a panel of 20 genes covering BE- and EAC-specific molecular markers (FZD5, IFNGR1, IL1A, IL1B, IL1R1, IL1RN, KRT4, KRT8, KRT15, KRT18, NFKBIL1, PTGS1, PTGS2, SOCS3, SOX4, SOX9, SOX15, TIMP1, TMEM2, TNFRSF10B). Furthermore, we aimed to reflect these alterations within an experimental and

Hepatic hemangiomas mimicking gastrointestinal stromal tumors: A case report.

BACKGROUND: Hepatic hemangiomas can be challenging to diagnose, particularly when they present with atypical features that mimic other conditions, such as gastrointestinal stromal tumors (GISTs). This case highlights the diagnostic difficulties encountered when imaging subepithelial lesions, especially when conventional methods such as computed tomography (CT) and endoscopic ultrasound (EUS) are used. CASE SUMMARY: A 44-year-old woman presented with intermittent abdominal distension and heartburn for three months. Her medical history included iron deficiency anemia, menorrhagia, and previous

A case report of carcinoma of the papilla of Vater associated with a hyperplasia-dysplasia-carcinoma sequence by pancreaticobiliary maljunction.

BACKGROUND: Pancreaticobiliary maljunction (PBM) is a known risk factor for biliary tract cancer. However, its association with carcinoma of the papilla of Vater (PVca) remains unknown. We report a case with PVca that was thought to be caused by the hyperplasia-dysplasia-carcinoma sequence, which is considered a mechanism underlying PBM-induced biliary tract cancer. CASE PRESENTATION: A 70-year-old woman presented with white stool and had a history of cholecystectomy for the diagnosis of a non-dilated biliary tract with PBM. Esophagogastroduodenoscopy revealed a tumor in the papilla of Vater,

Helicobacter pylori associated gastric pathology.

Helicobacter pylori (HP), undoubtedly, the most common world-wide infection plays an important role in pathogenesis of peptic ulcer. Proof for a causal role for HP in peptic ulcer rests in two major points; 1) the majority of ulcer patients are HP infected and the prevalence of this infection for both gastric ulcer (GU) and duodenal ulcer (DU) is much higher than for gender- and age-adjusted controls and 2) the cure of HP infection dramatically reduces ulcer recurrence. Conclusions regarding the mechanisms by which HP induces peptic ulcer are restricted mainly to studies observing the conseque

pH, healing rate and symptom relief in acid-related diseases.

Suppression of gastric acid secretion is widely used and logical for the treatment of acid-related diseases. Healing of duodenal ulcer, gastric ulcer and gastroesophageal reflux disease is correlated significantly with the degree and the duration of suppression of intragastric acidity over 24 hours and with the length of the treatment. To date, proton pump inhibitors are the most effective agents among the currently available antisecretory drugs in offering the highest healing rate and fastest resolution of symptoms. Combinations of an antisecretory drug with one or more antimicrobial agents a

Subtotal cystectomy and antirefluxing hepaticoduodenostomy for choledochus cyst in children.

Total cystectomy with Roux-en-Y hepaticojejunostomy for choledochus cyst often causes malabsorption and other problems as a result of jejunal biliary diversion bypassing the duodenum. Restoration of near normal anatomy can only return the normal physiological harmony and assure normality. A simple antirefluxing hepaticoduodenostomy has proved successful.

Laparoscopic duodenojejunostomy for recurrent acute pancreatitis associated with superior mesenteric artery syndrome in a medically complex patient: A case report and literature review.

RATIONALE: Superior mesenteric artery syndrome (SMAS) is a rare cause of duodenal obstruction, and acute pancreatitis associated with SMAS is extremely uncommon. The causal relationship is usually inferred from persistent duodenal obstruction, exclusion of other etiologies, and resolution after decompression. PATIENT CONCERNS: A 19-year-old male with a history of esophageal atresia repair, previous upper abdominal surgery, refractory epilepsy, severe developmental delay, and a bedridden status presented with recurrent vomiting and acute pancreatitis. Pancreatitis recurred over a period of 4 y

Genetic Analyses Identify Human Serum Metabolites Associated With Risk of Gastroesophageal Reflux Disease: A Mendelian Randomization Study.

Aims/Background Gastroesophageal reflux disease (GERD) is a widespread upper esophagogastric disease with incompletely understood biological mechanisms. Emerging evidence supports a complex link between GERD and metabolic markers. Therefore, Mendelian randomization (MR), an innovative genomic approach, was used to evaluate the causal impacts of serum metabolites on GERD, aiming to identify novel biomarkers and elucidate underlying metabolic pathways. Methods A two-sample MR framework was employed to examine causal relationships between circulating metabolites and GERD. Genetic instruments for

Dual-Algorithm Integration Framework Reveals Qing-Wei-Zhi-Tong's Dual Mechanisms in Chronic Gastritis.

Background: Chronic gastritis (CG) involves gastric mucosal imbalance, with H. pylori (>90% cases), acid-pepsin imbalance, and bile reflux as druggable mechanisms. FDA-approved drugs show limited efficacy against antibiotic-resistant strains and fail to target undruggable pathways (e.g., inflammation, autoimmune atrophy). Traditional Chinese Medicine (TCM), particularly Qing-Wei-Zhi-Tong micro-pills (QWZT), offers multi-target advantages, though its mechanisms remain poorly understood. Methods: The dual-algorithm integration framework predicts QWZT's pharmacological effects to treat gastritis.

Molecular Profile of Barrett's Esophagus and Gastroesophageal Reflux Disease in the Development of Translational Physiological and Pharmacological Studies.

Barrett's esophagus (BE) is a premalignant condition caused by gastroesophageal reflux disease (GERD), where physiological squamous epithelium is replaced by columnar epithelium. Several in vivo and in vitro BE models were developed with questionable translational relevance when implemented separately. Therefore, we aimed to screen Gene Expression Omnibus 2R (GEO2R) databases to establish whether clinical BE molecular profile was comparable with animal and optimized human esophageal squamous cell lines-based in vitro models. The GEO2R tool and selected databases were used to establish human BE

Outcomes after prophylactic gastrectomy for hereditary diffuse gastric cancer.

BACKGROUND: Patients with hereditary diffuse gastric cancer and a CDH1 mutation have a 60-80 per cent lifetime risk of developing diffuse gastric cancer. Total prophylactic gastrectomy eliminates this risk, but is associated with considerable morbidity. The effectiveness (removal of all gastric mucosa) and outcomes of this procedure were evaluated retrospectively. METHODS: All consecutive individuals undergoing a prophylactic gastrectomy for a CDH1 mutation or gastric signet ring cell foci at the authors' institute between 2005 and 2017 were included. RESULTS: In 25 of 26 patients, intraoper

Delayed gastric emptying after esophagectomy for malignancy.

BACKGROUND: Controversy still exists about the need for pyloric drainage procedures after esophagectomy with gastric conduit reconstruction. Although pyloric drainage may prevent postoperative delayed gastric emptying (DGE), it may also promote dumping syndrome and bile reflux. The aims of this study were to audit the incidence and management of DGE in patients without routine pyloric drainage after esophagectomy in a university medical center. PATIENTS AND METHODS: From July 2006 to June 2012, data from 356 consecutive patients who underwent esophagectomy with a gastric conduit without pylor

Retinoic acid-induced glandular differentiation of the oesophagus.

BACKGROUND: Retinoic acid (RA) is a powerful differentiation agent. Barrett's oesophagus occurs when duodeno-gastro-oesophageal reflux causes squamous epithelium (SE) tissue to become columnar epithelium tissue by an unknown mechanism. The bile acid lithocholic acid (LCA) competes for the retinoid X receptor retinoid binding site. Hence, RA pathways may be implicated in Barrett's oesophagus. METHODS: RA activity in tissues and cell lines treated with all-trans retinoic acid (ATRA) with or without LCA was assessed using a reporter. Expression of p21 was determined by real-time PCR in Barrett's

Cancer in the Excluded Stomach After Gastric Bypass Surgery for Obesity.

OBJECTIVE: To assess whether gastric bypass surgery for obesity increases the risk of adenocarcinoma in the excluded stomach and delays adenocarcinoma diagnosis. BACKGROUND: Gastric bypass might increase adenocarcinoma risk through bile reflux and delay such diagnosis because of anatomical hurdles with endoscopic access. METHODS: This population-based cohort study included patients having undergone bariatric surgery in Denmark, Finland, or Sweden. Cancer risk after gastric bypass was compared with (1) other bariatric procedures using Cox regression, providing hazard ratios (HR) with 95% conf

Esophageal epithelial cell-derived kynurenine drives Th17 inflammation in reflux esophagitis and is targeted by Xuanfu Daizhe decoction.

BACKGROUND: The role of immunometabolic dysregulation in reflux esophagitis (RE), particularly the metabolic drivers of pathogenic T helper 17 (Th17) cell differentiation, is not fully understood. Although Xuanfu Daizhe Decoction (XDD) is a clinically effective Traditional Chinese Medicine (TCM) formula for RE, its potential to modulate these immunometabolic pathways remains unelucidated. OBJECTIVE: This study aimed to investigate the mechanism by which tryptophan metabolic dysregulation in RE influences Th17 cell differentiation and to evaluate whether XDD exerts therapeutic effects by targe
