Solitary plasmacytoma of the index finger.

INTRODUCTION: Solitary osseous plasmacytoma rarely involves the distal extremities. We report a case and provide a brief review of the relevant literature. CASE PRESENTATION: We report a 64-year-old man who presented with swelling, mild pain and a deformed right index finger. The workup led to the diagnosis of solitary osseous plasmacytoma and the patient eventually required amputation of his finger. With clinical follow-up, the disease spread to regional lymph nodes and subsequently the patient developed systemic involvement and received chemotherapy. CONCLUSIONS: Solitary osseous plasmacyt

Phalangeal quantitative ultrasound measurements in chronic hemodialysis patients: a 4-year follow-up.

In the course of chronic kidney disease, bone metabolism disturbances occur and become aggravated simultaneously with the progression of renal disorder, worsening patients' quality of life. We conducted a 4-year follow-up to assess phalangeal quantitative ultrasound (QUS) measurements in 32 patients undergoing chronic hemodialysis (17 males and 15 females) whose mean ages were 56.3 ± 15.2 years. The QUSs of hand phalanges were performed using DBM 1200 (IGEA, Carpi, Italy) and are expressed as amplitude-dependent speed of sound (Ad-SoS), Z-scores, and T-scores. In comparison with the age-, sex-

Long-term results following osteotomy of the thumb delta phalanx in Rubinstein-Taybi Syndrome.

Rubinstein-Taybi Syndrome is a rare condition affecting 1:125,000 children. It is associated with short broad radially deviated thumbs, secondary to a delta proximal phalanx of the thumb. We undertook a retrospective review of seven children (13 thumbs) with Rubinstein-Taybi syndrome whose thumbs were treated using a corrective osteotomy to the delta phalanx over a 13 year period. The types of osteotomy used in the series were reverse wedge osteotomy, opening wedge osteotomy and dome shaped osteotomy. The mean preoperative radial deviation of thumbs was 68 degrees (range 45-85 degrees ). At fo

Longitudinal changes in ultrasound measurements: a parallel study in subjects with genetic disorders and healthy controls.

Disturbances in skeletal status in subjects with genetic disorder may increase their fracture risk. The aim of the study was longitudinal observation of phalangeal speed of sound changes across the bone over a period of 2 y in 24 patients (14 boys and 10 girls, mean age 9.63 +/- 1.8 y.) and 24 age-matched healthy controls (14 boys and 10 girls, mean age 9.65 +/- 1.71 y.). Weight and height did not differ between patients and controls at baseline and follow-up. Patients with the following disorders were evaluated: 7 with Down syndrome, 6 nonspecific mental retardations of unknown etiology, 5 Ma

The heterozygous c.2241G>C variation in FGFR2 may cause autosomal dominant Kirner's deformity in a Chinese Han pedigree.

OBJECTIVE: Kirner’s deformity is a progressive deformity of the distal phalange of the little finger in a volar and radial direction. The FGFR2 gene plays a critical role in skeletal development and growth, with abnormal expression and activation of FGFR2 being associated with skeletal abnormalities. This study presents a family with Kirner’s deformity involving multiple patients. METHODS: Whole-exome sequencing was performed. SIFT, MutationTaster and Gerp+, while Clustal Omega was used for conservative analysis. In addition, I-TASSER was used to predict the 3D protein model. PyMOL was used fo

Characterization of digital annular pulleys and their entheses: an ultrasonographic study with anatomical and histological correlations.

OBJECTIVES: Digital annular pulleys (DAPs) are important anatomical structures for finger function. The anatomy, histology and imaging assessment of DAPs, particularly at the level of their entheses, are still not clearly defined. The advent of high-frequency US transducers opened new perspectives in evaluating submillimetre-scale structures, such as pulleys, paving the way for their global assessment. The study aimed to characterize DAPs from an anatomical, histological and US perspective, focusing on the detection and complete description of pulley entheses. METHODS: US assessment and gross

Critical role of thrombospondin-1 in promoting intestinal mucosal wound repair.

Acrodermatitis continua of Hallopeau (ACH) is a rare, sterile pustular eruption of one or more digits. The condition presents with tender pustules and underlying erythema on the tip of a digit, more frequently arising on a finger than a toe. As far as classification, ACH is considered a localized form of pustular psoriasis. The eruption typically occurs after local trauma or infection, but such a history is not always present and various other etiologies have been described including infectious, neural, inflammatory, and genetic causes. The natural progression of ACH is chronic and progressive

Idiopathic Acroosteolysis: A Novel Cutaneous Sign Can Help Identify the Condition Early.

Acroosteolysis (AO) is a rare condition characterized by resorption of the distal phalanges of the fingers and/or toes. It can be familial, idiopathic (IAO), occupational, or secondary. Other authors suggest a classification into primary (genetic disorders, lysosomal storage disorders) or secondary AO. Various skin and nail changes have been reported in this condition. However, the cutaneous change on the affected digit(s)/toe(s) during the natural course of AO has been poorly documented. A 5-year-old girl presented with a 3-month history of a distinct transverse boundary between normal skin p

Maffucci syndrome: Case report and review of diagnostic signs of the rare disease.

Maffucci syndrome is a non-hereditary congenital condition that affects the skin and skeleton. Enchondromas (benign cartilage enlargements), bone abnormalities, and venous anomalies (hemangiomas) are all symptoms. Enchondromas occur as a result of mesodermal dysplasia and have the potential to become cancerous. They are most commonly found on the phalanges and long bones. Venous abnormalities commonly manifest themselves as soft lumps or tumors on the distal extremities. A 19-year-old boy presented with swellings on his fingers and left foot since the age of 5, along with a few bluish soft tis

Outcomes of Surgery for Enchondromas within the Hand.

BACKGROUND: Enchondromas are the most common benign bone tumours found in the hand. They are usually accidentally diagnosed on an X-ray, because they grow asymptomatically. In some cases, a pathological fracture of the involved phalanx may be the first sign. The objective of this study was to assess the results of operative treatment of enchondromas involving hand phalanges and metacarpals. MATERIAL AND METHODS: The study group consisted of 24 patients, 16 women (67%) and 8 men, (33%), aged a mean of 31 years, who were operated on at our centre. The surgery consisted in curettage, and - in mo

Acrocallosal syndrome in fetus: focus on additional brain abnormalities.

Acrocallosal syndrome (ACS) is an autosomal recessive disorder characterized by craniofacial dysmorphism, agenesis or hypoplasia of the corpus callosum, duplication of the phalanges of the hallux, more rarely the thumbs, post-axial polydactyly, syndactyly and severe mental retardation. Here we report the two first descriptions of acrocallosal syndrome in fetus with extensive neuropathological study and provide new data regarding additional brain abnormalities in ACS. The first case was a 25-gestational week male fetus displaying craniofacial and limb abnormalities, with bilateral syndactyly of

Surgical reconstruction of irreversible ulnar nerve paralysis in leprosy.

Twenty-five patients with irreversible leprotic ulnar nerve palsy having undergone lumbrical replacement with two different tendon transfer techniques were assessed 6-120 months after surgery. Nineteen patients were reconstructed with the flexor digitorum four-tail procedure (FDS-4T), and six with Zancolli's lasso procedure (ZLP). Mean paralysis times were 103 months for FDS-4T, and 68 months for ZLP. Mean age of the patients was 36 years (21-57). Grip strength measurements, improvement in active range of motion at the PIP joints, patients' ability to open and close their hands fully, as well

[Nail recession emergency treatment: a new technique for the repair of fingertip amputations].

INTRODUCTION: In spite of the recent advances made in microsurgery, the reconstruction of oblique fingertip amputations remains problematical. In cases where reimplantation is technically impossible, the surgeon can utilize a number of different flaps to preserve digital length. METHODS: In certain cases, instead of local flap repair using an advancement flap, the nail complex can be recessed so that after bone shortening of the distal phalanx the free edge of the nail can be stitched to the skin without resulting tension. Two longitudinal incisions are made, and a flap including the nail com

Aesthetic refinements in toe-to-hand transfer surgery.

Techniques for aesthetic refinement are as important as those for functional improvement in toe-to-hand transfer. The appearance of the thumb reconstructed using various types of great toe transfer can be improved by reduction of the soft tissue, bone, interphalangeal joint, and nail and by secondary pulp reduction and contouring procedures. Finger and thumb reconstructions using lesser toes can be improved aesthetically by minimal inclusion of adipofibrous tissue under the plantar skin flap especially at the metatarsophalangeal joint region, thus decreasing the anterior-posterior bulkiness. [Burn injury of the child's hand].

The treatment of the burnt hand of a child requires careful consideration of both the physical and psychological aspects involved in this programme; measures such as physical therapy, which play an important part in the treatment of adults, are of less significance. Nevertheless, due to a very low incidence of complications eg. joint stiffness, the final results are very good. Generally the recommended approach to the treatment programme is to use the closed method (i. e. dressings), whereas in such cases care on a special unit employing the open method and a topical bactericidal agent is cons

Bone loss determined by quantitative ultrasonometry correlates inversely with disease activity in patients with endogenous glucocorticoid excess due to adrenal mass.

OBJECTIVE: Glucocorticoid excess is widely recognized as one of the most important causes of bone loss. The mechanism of glucocorticoid-induced osteoporosis is presumably multifactorial, and consists of the loss of organic and non-organic compounds. Efforts have been made to develop simple physical methods for the assessment of bone tissue for the screening of subjects at high risk of osteoporosis, without the use of radioactive sources or ionizing radiation. Quantitative ultrasonometry (QUS) has been suggested as a useful method for monitoring patients undergoing glucocorticoid therapy, which

Toward human-compatible compliant manipulation with an anthropomorphic dexterous hand.

Achieving human-like compliant manipulation remains a fundamental challenge in robot hands due to the difficulty of realizing biomechanical compatibility, compliant interaction, and dexterous operation. To address this, we propose a human-compatible anthropomorphic dexterous hand inspired by the anatomical structure and coordinated motion of the human hand. The design integrates anatomical morphology, joint coordination, and compliant manipulation through a rigid-soft synergistic architecture with a cable-driven mechanism. Flexible thermoplastic polyurethane elastomers and cables emulate human
