Improvements in Out-of-Hospital Cardiac Arrest Survival from 1998 to 2013.

OBJECTIVES: Out-of-hospital cardiac arrest (OHCA) remains a major public health burden. Aggregate OHCA survival to hospital discharge has reportedly remained unchanged at 7.6% for almost 30 years from 1970 to 2008. We examined the trends in adult OHCA survival over a 16-year period from 1998 to 2013 within a single EMS agency. METHODS: Observational cohort study of adult OHCA patients treated by Tualatin Valley Fire & Rescue (TVF&R) from 1998 to 2013. This is an ALS first response fire agency that maintains an active Utstein style cardiac arrest registry and serves a population of approximate

Ambient noise levels in the chemotherapy clinic.

Many of the drugs used for chemotherapy treatments are known to be ototoxic, and can result in permanent hearing threshold shifts. The degree of ototoxic damage can be influenced by many factors including dosage, duration of exposure, genetics, and coadministration with other ototoxic agents. Cisplatin is known for its ototoxic effects on hearing thresholds, particularly in the high frequencies. Recent studies have indicated a synergistic relationship between Cisplatin administration and moderate to high noise level exposure starting between 70-85 dB SPL. This study measured the noise levels i

Varieties of centralized intake: the Portland Target Cities Project experience.

To assess the possible influence of centralized intake on client outcomes, initial, six- and twelve-month Addiction Severity Index composite scores (in the alcohol, drug, legal and psychiatric areas) for clients who experienced provider intake were compared with scores for those going through two different models of centralized intake. Centralized intake clients were more likely than provider intake clients to have legal problems, and those legal problems became fewer over time. Clients from in-jail intake, including pretreatment services and accompanied placement, showed a greater initial and

Validation of measures of condom and other contraceptive use among women at high risk for HIV infection and unintended pregnancy.

This study assessed the applicability of the transtheoretical model of behavior change (J.O. Prochaska & C.C. DiClemente, 1983, 1984) to the measurement of contraceptive use among 296 women at high risk for HIV infection and transmission. Structural equation modeling suggested that a measure of general contraceptive use could be used to assess use of oral contraceptives and hormonal implants but that measurement of condom use required separate assessments for main and other partners. Self-efficacy (SE) and decisional balance scales were internally consistent for general contraceptive use, for

Are Head Start effects sustained? A longitudinal follow-up comparison of disadvantaged children attending Head Start, no preschool, and other preschool programs.

This study investigates the sustained effects into kindergarten and grade 1 of Project Head Start for disadvantaged black children. Participation in generic Head Start programs was compared to both no preschool and other preschool experience for disadvantaged children in two American cities in 1969-1970. Incorporating both pretest/posttest and comparison group information, the study has advantages over other Head Start impact studies. Both preprogram background and cognitive differences were controlled in a covariance analysis design, using dependent measures in the cognitive, verbal, and soci

Single-cell analysis of UNC13D-mediated immune and dedifferentiation heterogeneity in acute myeloid leukemia and development of a prognostic model.

BACKGROUND: Acute myeloid leukemia (AML) is a highly heterogeneous hematologic malignancy, with its pathogenesis closely associated with cellular states at various stages of differentiation. Existing clinical prognostic models often fail to account for this heterogeneity and lack integration of key molecular pathways. This study aimed to characterize AML differentiation-associated heterogeneity at the single-cell level, investigate the role of UNC13D in immune and dedifferentiation states, and develop a prognostic model integrating these features. METHODS: This study combined single-cell RNA

Social Interaction With Younger Females Extends Lifespan in Aged Male Drosophila melanogaster Independent of Vitamin C.

BACKGROUND: Aging is a multifactorial process shaped by environmental, social, and nutritional influences that collectively drive molecular and physiological decline. The fruit fly Drosophila melanogaster is a well-established model for aging research due to its short lifespan and the conservation of key regulatory pathways across species. While many interventions target single mechanisms, the combined effects of nutritional antioxidants, such as vitamin C, and social interactions on stress responses, lifespan, and functional aging remain insufficiently understood. Therefore, this study aimed

Maternal levels of care and association with severe maternal morbidity during birth hospitalizations.

BACKGROUND: Maternal level of care guidelines were published to facilitate risk-appropriate care and improve outcomes, yet evidence of benefit is lacking. All birth hospitals should be equipped to provide routine childbirth care and be prepared to manage common, treatable childbirth complications. Thus, our objective was to examine the association of maternal level of care with severe maternal morbidity (SMM) without transfusion among 1) all obstetric patients and 2) obstetric patients with common, treatable childbirth complications (e.g., hemorrhage and infection). METHODS: Using a retrospec

Long-distance object motion evokes direction selectivity in retinal neurons through nicotinic receptors in bipolar cells.

In the retina, the direction of a moving object is encoded by starburst amacrine cells (SACs) and ON-OFF direction-selective ganglion cells (ooDSGCs). Multiple neural circuits have accounted for the underlying mechanisms of direction selectivity. We previously found that the cholinergic feedback circuit through α7-nicotinic acetylcholine receptors (α7-nAChRs) in bipolar cells mediates direction selectivity in SACs. We investigated whether distinct visual stimuli differently affected cholinergic feedback through α7-nAChRs. We conducted two-photon Ca2+ imaging using Ca2+ indicators of Oregon Gre

Cavernous Sinus Medial Wall Resection: A Retrospective Single-Institution Study on Outcomes in Functional Pituitary Adenomas.

BACKGROUND AND OBJECTIVES: Surgery remains first-line therapy for most functioning pituitary adenomas (FPA). Remission rates are lower in some patients due to cavernous sinus invasion. Medial wall resection (MWR) may improve biochemical remission but remains largely underutilized. Our objective is to assess outcomes of MWR in patients with FPA at Oregon Health & Science University performed by a single neurosurgeon. METHODS: Retrospective, Institutional Review Board-approved study: patients ≥18 years, FPA after MWR with ≥ 3-months follow-up. Remission: normal prolactin, insulin-like growth fa

Socioecological factors linked to co-occurring early childhood sleep health disparities and developmental outcomes: protocol for the sleep in preschoolers cross-sectional study.

INTRODUCTION: Sleep deficiencies, such as sleep disordered breathing (SDB) and insufficient sleep, are linked to adverse health outcomes. These sleep deficiencies are more common in racial and ethnic minoritised children and have significant negative impacts on neurobehavioural and social-emotional development. Non-Latine Black/African American children are 4-6 times more likely than non-Latine White children to experience both SDB and short sleep duration. Although SDB and insufficient sleep often co-occur in young children, there is a paucity of research considering the potential unique and

Impact of intravenous iron or exogenous erythropoietin on hemoglobin mass, exercise performance, and acute mountain sickness during altitude acclimatization.

Chronic exposure to high altitude leads to increases in hemoglobin mass (Hbmass), which may improve exercise performance and decrease acute mountain sickness (AMS) symptoms. We evaluated the influence of intravenous iron or erythropoietin (EPO) treatment on Hbmass, exercise performance, and AMS during a 14-day exposure to 3,094 m. Thirty-nine participants (12 F) completed the study conducted in Eugene, Oregon [sea level (SL), 130 m] and Leadville, Colorado (3,094 m). Participants were dosed with either a placebo (saline; n = 13), iron [Fe(III)-hydroxide sucrose 200 mg, 2 times; n = 14], or EPO

Methods for scaling up an outreach intervention to increase colorectal cancer screening rates in rural areas.

BACKGROUND: Mailed fecal immunochemical test (FIT) outreach and patient navigation are evidence-based practices shown to improve rates of colorectal cancer (CRC) and follow-up in various settings, yet these programs have not been broadly adopted by health systems and organizations that serve diverse populations. Reasons for low adoption rates are multifactorial, and little research explores approaches for scaling up a complex, multi-level CRC screening outreach intervention to advance equity in rural settings. METHODS: SMARTER CRC, a National Cancer Institute Cancer Moonshot project, is a clu

Treatment retention, return to use, and recovery support following COVID-19 relaxation of methadone take-home dosing in two rural opioid treatment programs: A mixed methods analysis.

OBJECTIVES: In March 2020, the Substance Abuse and Mental Health Services Administration permitted Opioid Treatment Programs (OTPs) to relax restrictions on take-home methadone and promoted telehealth to minimize potential exposures to COVID-19. We assessed the effects of COVID-19-related changes on take-home methadone dosing in two OTPs serving five rural Oregon counties. METHODS: We used a mixed-methods convergent design. The OTPs extracted urine drug test (UDT) results, take-home methadone regimens, and treatment retention from the electronic health record (EHR) for patients (n = 377). A m

Endoglin Is an Endothelial Housekeeper against Inflammation: Insight in ECFC-Related Permeability through LIMK/Cofilin Pathway.

Endoglin (Eng) is an endothelial cell (EC) transmembrane glycoprotein involved in adhesion and angiogenesis. Eng mutations result in vessel abnormalities as observed in hereditary hemorrhagic telangiectasia of type 1. The role of Eng was investigated in endothelial functions and permeability under inflammatory conditions, focusing on the actin dynamic signaling pathway. Endothelial Colony-Forming Cells (ECFC) from human cord blood and mouse lung/aortic EC (MLEC, MAEC) from Eng+/+ and Eng+/- mice were used. ECFC silenced for Eng with Eng-siRNA and ctr-siRNA were used to test tubulogenesis and p

Ixodes spp. from Dogs and Cats in the United States: Diversity, Seasonality, and Prevalence of Borrelia burgdorferi and Anaplasma phagocytophilum.

Ixodes spp. are commonly found on dogs and cats throughout the world. In the eastern United States, 16S rDNA sequence of Ixodes scapularis, the predominant species, reveals two clades-American and Southern. To confirm the species and clades of Ixodes spp. ticks submitted from pets, we examined ticks morphologically and evaluated 16S rDNA sequence from 500 ticks submitted from 253 dogs, 99 cats, 1 rabbit, and 1 ferret from 41 states. To estimate pathogen prevalence, flaB of Borrelia burgdorferi (Bb) sensu stricto and 16S rDNA of Anaplasma phagocytophilum (Ap) were amplified and sequenced. Most
