Guideline for the management of chronic obstructive pulmonary disease--2011 update.

OBJECTIVE: To revise the South African Guideline for the Management of Chronic Obstructive Pulmonary Disease (COPD) based on emerging research that has informed updated recommendations. KEY POINTS: (1) Smoking is the major cause of COPD, but exposure to biomass fuels and tuberculosis are important additional factors. (2) Spirometry is essential for the diagnosis and staging of COPD. (3) COPD is either undiagnosed or diagnosed too late, so limiting the benefit of therapeutic interventions; performing spirometry in at-risk individuals will help to establish an early diagnosis. (4) Oral corticos

Assessing current diagnostic, staging, and treatment practices in community and academic centers for individuals with stage IB-IIIA non-small cell lung cancer.

BACKGROUND: The 5-year survival rate for individuals with surgically resected, early-stage non-small cell lung cancer (NSCLC) is around 50%. Recent availability of biomarker-driven therapy after surgery offers the promise of improved outcomes. However, current gaps in the quality of diagnostic evaluation, delivery of evidence-based treatment, coordination of care, and availability of biomarker testing remain barriers to optimal outcomes. METHODS: The Association of Cancer Care Centers (ACCC) conducted a survey of providers who treat NSCLC to assess current patterns in the diagnosis and treatm

Beta-blocker Use in Moderate and Severe Chronic Obstructive Pulmonary Disease.

INTRODUCTION: The most appropriate choice of pharmacological treatment of heart rhythm disorders occurring in patients with chronic obstructive pulmonary disease (COPD) and cardiovascular comorbidity is often a topic of debate between pulmonologists and cardiologists in clinical practice, although numerous studies and clinical trials have demonstrated evidence to support the use of selective beta-blockers (BBs) in these patients. AIM: To examine the difference in the number of exacerbations in patients treated with a combination of verapamil and digoxin or BB alone in patients with different

Prediction and diagnosis of suspected foreign body aspiration in children using flexible bronchoscopy: a retrospective cohort study.

UNLABELLED: A thorough clinical history of witnessed or suspected choking and a high index of suspicion are critical for reducing and managing early and late complications of foreign body aspiration (FBA). Flexible fiberoptic bronchoscopy (FFB) is increasingly used for diagnostic and therapeutic purposes. This study aimed to identify risk factors and predictors of FBA in children undergoing FFB. A total of 141 children (median age, 24 months; 60% male) who underwent FFB were included and categorized into FBA-positive (35.4%) and FBA-negative (64.6%) groups. The Foreign Body Aspiration Score (F

Reevaluation of Diagnosis in Adults With Physician-Diagnosed Asthma.

IMPORTANCE: Although asthma is a chronic disease, the expected rate of spontaneous remissions of adult asthma and the stability of diagnosis are unknown. OBJECTIVE: To determine whether a diagnosis of current asthma could be ruled out and asthma medications safely stopped in randomly selected adults with physician-diagnosed asthma. DESIGN, SETTING, AND PARTICIPANTS: A prospective, multicenter cohort study was conducted in 10 Canadian cities from January 2012 to February 2016. Random digit dialing was used to recruit adult participants who reported a history of physician-diagnosed asthma esta

[The patient's opinion matters: experience in the nutritional care in an ALS multidisciplinary team].

UNLABELLED: Amyotrophic Lateral Sclerosis (ALS) is a neurodegenerative disease which has no cure, so the treatment will be symptomatic in a Multidisciplinary Unit. It is composed of professionals, experts in patient care, with an interdisciplinary vision in order to act in a coordinated manner depending on the different situations which may arise over the course of the disease. There are several studies showing improved survival in patients treated within the framework of a multidisciplinary team compared to treatment by isolated specialties. An ALS Multidisciplinary Unit was created in 2004 i

Education and experience improve the performance of transbronchial needle aspiration: a learning curve at a cancer center.

STUDY OBJECTIVES: Transbronchial needle aspiration (TBNA) is an indispensable part of the pulmonologist's armamentarium, although it continues to be woefully underutilized despite its demonstrated safety and usefulness. We herein review our experience with the procedure. MATERIALS AND METHODS: All TBNAs were conducted according to standard techniques using 21-gauge cytology needles or 19-gauge histology needles connected to a flexible bronchovideoscope. All procedures were conducted at a 180-bed cancer center, and results were analyzed retrospectively. The mediastinum and hilar lymph node map

Development and validation of artificial intelligence-assisted volumetric response criteria in pleural mesothelioma (ARTIMES): a retrospective, multicohort, multicentre study.

BACKGROUND: Response evaluation in pleural mesothelioma is challenging because its crescent growth pattern is poorly captured by diameter-based criteria. We aimed to develop and validate artificial intelligence (AI)-assisted volumetric response criteria (ARTIMES) based on automated tumour segmentation and biologically derived thresholds. METHODS: In this retrospective, multicentre study, we included 10 926 CT scans from 2080 patients from 14 cohorts. A subset totalling 1176 CT scans from routine care (Netherlands Cancer Institute-Antoni van Leeuwenhoek Hospital) and trial cohorts (INITIATE, N

The Meaning and Value of Palliative Rehabilitation Through a Case Report Analysis.

PURPOSE: Palliative rehabilitation is defined as the process of helping individuals with a progressive, often advanced or incurable disease reach their physical, psychological, and social potential consistent with physiologic and environmental limitations and life preferences. However, the evidence on this topic is very heterogeneous in the medical scientific literature and studies diverge in their results depending on both the pathologies treated and the outcome measures studied as well as the types of interventions brought to the patients. Additionally, little is known about the role of the

How I approach optimization of patients at risk of cardiac and pulmonary complications after CAR T-cell therapy.

Chimeric antigen receptor (CAR) T cells have transformed the care for patients with hematologic malignancies. Patients treated with CAR T cells may experience cardiovascular and pulmonary complications, which primarily occur in the setting of cytokine release syndrome. In addition, many patients considered for CAR T-cell therapy have preexisting cardiac and pulmonary comorbidities. Among patients with good functional status, these conditions should not prevent patients from being offered these lifesaving therapies. In this article, we use a case-based approach to discuss how we evaluate and op

Lymphangioleiomyomatosis and Pregnancy-Do We Have All the Answers for a Woman Who Desires to Conceive?-Literature Review.

Lymphangioleiomyomatosis (LAM) is a rare, progressive, and poor-prognosis systemic disorder that primarily affects women of reproductive age, with a higher prevalence among individuals of Caucasian origin. However, there are limited reliable data on the prevalence of LAM during pregnancy. The fulminant respiratory clinical presentation that often includes progressive dyspnea on exertion, cough, or hemoptysis, frequently complicated by pneumothorax, and the increased risk of spontaneous abortion due to increased estrogen and progesterone production during gestation, are arguments that most ofte
