Prognostic Analysis of Emergency Decompressive Craniectomy for Patients with Severe Traumatic Brain Injury with Bilateral Fixed Dilated Pupils.

OBJECTIVE: For patients with severe traumatic brain injury (sTBI) with bilateral fixed dilated pupils (BFDP), the value of aggressively decompressive craniectomy (DC) treatment is still controversial. The objective of this study was to analyze and validate the outcome of DC in patients with sTBI with BFDP. METHODS: We retrospectively collected data from 44 patients with sTBI with BFDP who underwent DC treatment from July 2011 to June 2018. Outcomes used as indicators were mortality and favorable outcome. The analysis was based on the Glasgow Outcome Scale score recorded at discharge, 6, and 1

Next-generation sequencing to dissect hereditary nephrotic syndrome in mice identifies a hypomorphic mutation in Lamb2 and models Pierson's syndrome.

The study of mutations causing the steroid-resistant nephrotic syndrome in children has greatly advanced our understanding of the kidney filtration barrier. In particular, these genetic variants have illuminated the roles of the podocyte, glomerular basement membrane and endothelial cell in glomerular filtration. However, in a significant number of familial and early onset cases, an underlying mutation cannot be identified, indicating that there are likely to be multiple unknown genes with roles in glomerular permeability. We now show how the combination of N-ethyl-N-nitrosourea mutagenesis an

Locus-Coeruleus Norepinephrine Functioning as a Predictor of Childhood Mental Health (LOCUS-MENTAL): Protocol for a Longitudinal Study.

BACKGROUND: Mental health disorders (MHDs) remain a leading cause of the global burden of diseases. Early identification of neurobiological mechanisms mediating a risk for MHDs is key to reducing a lifetime burden. Recent findings emphasize the locus coeruleus-norepinephrine (LC-NE) system as a neuromodulator of arousal translating acute stress responses into neuronal excitability. We propose that individual differences in LC-NE functioning can explain a differential susceptibility to psychological adversity, which mediates the development of transdiagnostic psychopathology in early childhood.

"Best Pick and Hold" with "Wrap and Roll" technique for extreme subluxated crystalline lens.

BACKGROUND: A severely subluxated crystalline lens refers to significant displacement of the eye's natural lens from its normal position, often due to zonular weakness or rupture.[1] This condition can result from trauma, connective tissue disorders like Marfan syndrome, or advanced pseudoexfoliation.[2,3] Visual symptoms include blurred vision, monocular diplopia, and glare. On examination, lens edge may be visible within the pupil, and phacodonesis (lens wobbling) may be noted. Severe subluxation increases the risk of complications such as angle-closure glaucoma, lens-induced uveitis, or ret

Chronic Disease and Disability in the United States: Trends, Inequities, and Imperatives for Policy Reform.

UNLABELLED: Policy Points Position Universal, High-Quality Education as a Foundational Public Health Strategy. Long-term health and life expectancy in the United States are shaped more by educational attainment than by any other single factor. Disparities in access to early childhood, K-12, and higher education perpetuate cycles of chronic disease, economic hardship, and shortened lifespan, particularly in marginalized communities. Policymakers must recognize education as a critical public health intervention and commit to providing universal access to high-quality education at every stage of

Living donor kidney transplantation in patients with hereditary nephropathies.

Patients with some hereditary nephropathies-including autosomal dominant polycystic kidney disease (ADPKD), Fabry disease and Alport syndrome-can progress to end-stage renal disease (ESRD) and are candidates for kidney transplantation. When considering whether a potential living donor is appropriate for a particular patient, clinicians should be aware of the increased risk of adverse outcomes for the donor and the recipient. Renal transplantation from a living related donor is not contraindicated in most nephropathies that have an autosomal recessive mode of inheritance (for example, autosomal

Investigating the Effects of Paralinguistic Configuration in Background Human Speech on Attention: Protocol for a Within-Subject Study.

BACKGROUND: Background noise is common in everyday environments; yet, its effects on attention vary according to task demands, individual characteristics, and properties of the noise itself. Previous research has largely focused on music, nonspeech noise, or intelligible speech, leaving the effects of paralinguistic components of human speech (eg, tone, pitch, and rhythm) difficult to disentangle from linguistic content. OBJECTIVE: This study aims to investigate whether paralinguistic characteristics of semantically inaccessible background speech influence attentional performance. Specificall

A Rare Thrombophilic Occurrence: Dural Venous Sinus Thrombosis in a Patient with Significant Family History of Protein S Deficiency.

Protein S is a potent anticoagulant that downregulates thrombin formation and is a vitamin K-dependent glycoprotein which is primarily synthesized in the liver. A deficiency in this protein or decreased activity, as seen in hereditary protein S deficiency, can lead to life-threatening thrombosis. Hereditary protein S deficiency is a rare disease as listed by the National Organization for Rare Disorders (NORD). It is known to cause venous as well as arterial thromboembolic events commonly occurring in the deep leg and pelvic veins. Dural venous sinus thrombosis is a rare consequence of protein

Graph Neural Networks and sequential architectures for autism detection from eye-tracking biomarkers: a multi-site study.

BACKGROUND: Autism Spectrum Disorder (ASD) affects 1 in 100 children globally, yet early detection remains challenging due to reliance on subjective behavioral assessments and limited specialist availability, particularly in resource-constrained settings. Eye-tracking biomarkers offer objective, quantitative measurements of visual attention patterns, but existing Neural Network approaches lack systematic evaluation across diverse neural network architectures and multi-site data. This study provides a comprehensive comparison of Graph Neural Networks, Long Short-Term Memory networks, and Multil

Japanese encephalitis (JE) part II: 14 years' follow-up of survivors.

Japanese encephalitis, the commonest Arbovirus encephalitis, has been endemic in many parts of Asia, the Pacific Islands, and India; also, there have been many epidemics. Most of the post JE cases have been associated with neurological and neuropsychiatric deficits but have not been properly classified and followed. Practically all the previous studies were in children or young adults. The aim of this study, involving only adult cases, the largest ever being reported, has been to follow the 688/1,199 survivors of JE patients out of 1,282 of acute cases admitted during four epidemics for a peri

The role of coagulopathy and subdural hematoma thickness at admission in predicting the prognoses of patients with severe traumatic brain injury: a multicenter retrospective cohort study from China.

BACKGROUND: Traumatic brain injury (TBI) is one of the diseases with high disability and mortality worldwide. Recent studies have shown that TBI-related factors may change the complex balance between bleeding and thrombosis, leading to coagulation disorders. The aim of this retrospective study was to investigate the prediction of coagulopathy and subdural hematoma thickness at admission using the Glasgow Outcome Scale (GOS) in patients with severe TBI at 6 months after discharge. METHODS: In this retrospective cohort study, a total of 1006 patients with severe TBI in large medical centers in

Unraveling congenital ptosis with the aid of the pediatric perimeter device.

BACKGROUND: Ocular morbidity with an early onset can have a significant impact on the long-term development of an individual. Hence, careful assessment of visual functions early on is very important. However, testing infants always poses a challenge. Standard tools to assess infants' visual acuity, ocular motility, and so on rely on the clinician's quick subjective judgments of an infant's looking behavior. Eye movements are usually observed from head rotations or spontaneous eye movements in infants. Judging eye movements in the presence of strabismus is even more challenging. PURPOSE: This

[Consensus, hedonism: the characteristics of new family and their consequences for the development of children].

Over the last three decades, the marital family model described by Durkheim at the end of the nineteenth century has undergone numerous changes, e.g. questioning about the principle of authority, women emancipation, occurrence of the "new fathers", the growing influence of the media on the daily life of families, the less frequent and most precious child (due to the reduced number of children per family),... Through clinical, psychoanalytical and developmental models we, here, analyze these changes together with their impact on child. Historical and sociological approaches also allowed us to e

Polysubstance use in early adulthood and associated factors in the Republic of Ireland: An analysis of a nationally representative cohort.

BACKGROUND AND AIMS: Alcohol and other drug use is common in early adulthood; however, research on contemporary polysubstance use patterns-defined as use of multiple psychoactive substances-and their associated factors is limited. This study aimed to identify groups with differing polysubstance use patterns and to examine associations with individual, family and socio-environmental factors. DESIGN: This is a cohort study based on data from the Growing Up in Ireland (GUI) study. GUI employed a two-stage clustered sampling design, using the national primary school system as the initial sampling

Educational and health outcomes of schoolchildren in local authority care in Scotland: A retrospective record linkage study.

BACKGROUND: Looked after children are defined as children who are in the care of their local authority. Previous studies have reported that looked after children have poorer mental and physical health, increased behavioural problems, and increased self-harm and mortality compared to peers. They also experience poorer educational outcomes, yet population-wide research into the latter is lacking, particularly in the United Kingdom. Education and health share a bidirectional relationship; therefore, it is important to dually investigate both outcomes. Our study aimed to compare educational and he

Multimodal Psychophysiological Assessment of Craving in Patients With Alcohol Dependence During Virtual Reality Cue Exposure: Exploratory Single-Arm Clinical Study.

BACKGROUND: Craving is a diagnostic criterion and predictor of relapse in patients with alcohol dependence (AD) and is induced in cue exposure therapy (CET) to prepare patients for real-life risk situations. The benefits of virtual reality (VR) as an innovative tool within treatment for this highly prevalent disorder include increased practicability, standardization, and personalization of CET. Accurate measurement of craving is essential to develop effective virtual reality cue exposure (VR-CE) scenarios. Despite being relevant for diagnostics and therapy, the psychophysiological reaction to
