Betahistine metabolites, aminoethylpyridine, and hydroxyethylpyridine increase cochlear blood flow in guinea pigs in vivo.

OBJECTIVE: Betahistine is a histamine-like drug that is used in the treatment of Ménière's disease. It is commonly believed that betahistine increases cochlear blood flow and thus decreases the endolymphatic hydrops that is the cause of Ménière's. Despite common clinical use, there is little understanding of the kinetics or effects of its metabolites. This study investigated the effect of the betahistine metabolites aminoethylpyridine, hydroxyethylpyridine, and pyridylacetic acid on cochlear microcirculation. DESIGN: Guinea pigs were randomly assigned to one of the groups: placebo, betahistin

Temporal bone histopathology 14 years after cytomegalic inclusion disease: a case study.

Temporal bones were examined from a 14-year-old male who died of sequelae of congenital cytomegalic inclusion disease (CID). Cytomegalovirus (CMV) was not isolated from inner ear fluid or multiple systemic tissues at the time of death. Examination of temporal bones revealed chronic pathology of both cochlear and vestibular sensory and nonsensory tissues. Endolymphatic hydrops was observed in the basal turn of the cochlear duct, while Reissner's membrane was collapsed in the more apical turns. Strial atrophy and a loss of cochlear hair cells were observed along the entire length of the basilar

Histopathology of chloroform-induced inner ear damage.

Inner ear function loss was caused in guinea pigs and rats by injecting chloroform into the middle ear. After symptoms for cochlear and vestibular deficit had been registered, the animals were permitted to survive for one day to five months. Ear histopathology was then studied in celloidin sections. In both species, hair cells and afferent nerve fibers were intact at all survival times. The acute stage of functional loss in guinea pigs was associated with inner ears of normal histological appearance. Within days after chloroform injection a severe otitis media developed which led to fibrous oc

New evaluation method using sound stimulation for asymptomatic vestibular dysfunction.

The currently available evaluation methods for determining appropriate treatment strategies in patients with asymptomatic inner ear dysfunction are insufficient. Recently, we demonstrated the efficacy of 100-Hz sound stimulation for treating vestibular dysfunction in Meniere's disease. This case report presents the utility of 100-Hz sound stimulation as a new method for evaluating cases with asymptomatic vestibular dysfunction and their potential for recovery with early medical intervention. We evaluated the change in cervical vestibular-evoked myogenic potential (cVEMP) amplitude before and a

Asymmetry and tuning shift of the cervical vestibular evoked myogenic potential indicate saccular dysfunction in idiopathic normal pressure hydrocephalus.

OBJECTIVE: The purpose of this study was to investigate the effects of excessive cerebrospinal fluid (CSF) retention on the peripheral vestibular function and the inner ear fluid in patients with idiopathic normal pressure hydrocephalus (iNPH). METHODS: In 25 patients with iNPH (14 females, age 65-88 years), cervical vestibular evoked myogenic potential (cVEMP) was measured before the spinal tap test. The asymmetry ratios (ARs) and tuning properties in 500 Hz and 1,000 Hz short-tone burst stimuli of cVEMP were evaluated. Furthermore, cVEMP was measured in an age-matched control group of 12 no

Comparison of the video head impulse test results with caloric test in patients with Meniere's disease and other vestibular disorders.

Background: The caloric test has been used to evaluate the semi-circular canal function for decades. In 2009, the video head impulse test (vHIT) was introduced, which can be used to evaluate the semi-circular canal function within a short time. Although both tests examine the semi-circular canal, the stimulation methods differ and it is unclear whether the vHIT is equivocal to the caloric test.Aims/objectives: This study aimed to discern the differences between the vHIT and caloric test.Material and methods: This study comprised 112 patients with vertigo who visited the vertigo/dizziness centr

Audiovestibular Function Deficits in Vestibular Schwannoma.

Introduction. Vestibular schwannomas (VS) are benign tumours of the vestibular nerve and can lead to hearing loss, tinnitus, vertigo, facial palsy, and brainstem compression. Audiovestibular diagnostic tests are essential for detection and treatment planning. Methods. Medline was used to perform a systematic literature review with regard to how audiovestibular test parameters correlate with symptoms, tumour size, and tumour location. Results. The auditory brainstem response can be used to diagnose retrocochlear lesions caused by VS. Since hearing loss correlates poorly with tumour size, a retr

Molecular analysis of aquaporin genes 1 to 4 in patients with Menière's disease.

BACKGROUND: Menière's Disease (MD) is an episodic cochleovestibular dysfunction of unknown etiology, still lacking a specific test and therapy. The proposed theories on the pathophysiology include genetic factors and factors relating to inner ear homeostasis. Various aquaporins (AQP), water channels, expressed in the inner ear and the vestibular organ, are involved in homeostasis. Mutations in AQP genes could result in disturbed inner ear homeostasis and endolymphatic hydrops, and therefore be involved in the pathogenesis of MD. AIM: To search for mutations in AQP1 to 4 in patients suffering

Contralateral hearing loss after vestibular schwannoma surgery: case report.

OBJECTIVE: To describe a case of contralateral hearing loss (CHL) in vestibular schwannoma (VS) surgery and to discuss the factors potentially related with this complication. CLINICAL PRESENTATION: A 48-year-old man awakened with complete bilateral hearing loss after an uneventful retrosigmoid excision of a 20 mm left-sided VS. The patient had no complaints of vertigo or facial palsy on the contralateral side. The hearing loss proved to be endocochlear in origin and no improvement was observed after a 24-month follow-up period. DISCUSSION: CHL in VS surgery is not commonly reported but can o

Electrocochleographic effects of ear canal pressure change in subjects with Menière's disease.

This study examined the relationship between objective changes in electrocochleographic (ECochG) parameters and subjective changes in symptoms in a group of patients with Menière's disease/endolymphatic hydrops (MD/ELH) after application of positive and negative atmospheric pressure changes to the sealed ear canal. Our goals were to further examine the therapeutic effects of atmospheric pressure change on the treatment of MD/ELH, as reported by a series of Swedish studies, and to examine the utility of ECochG for monitoring these effects. By combining a clinical immittance unit and an evoked p

Masking and pitch shift of tone bursts and clicks by low-frequency tones.

From experiments in animals and investigations in humans it is known that the normally phase-dependent masking of a short stimulus by a low-frequency continuous tone does not occur in the case of endolymphatic hydrops. The recording of the masked threshold of short tone stimuli in a loud tone of 30 Hz is to be evaluated for the clinical diagnostics of Ménière's disease. To this purpose, the main parameters of the measurements (type, frequency, duration of the stimulus, and intensity of the masker) and their effect of phase-dependent masking and pitch-shift are investigated. Stimuli above 2 kHz

The prognostic value of the glycerol test: a review of 60 ears with unidirectional inner ear valve implants.

The following are our conclusions in regard to glycerol test interpretation based on 60 valve implants: Negative Tests 1. A negative glycerol test result and no audiometric evidence of significant hearing fluctuations indicate that a patient is unlikely to have postoperative hearing that is better than his best preoperative hearing. 2. The patient with a negative test can still have an excellent chance (greater than 80 per cent) to be relieved of vertigo. 3. A negative test and no audiometric evidence of significant hearing fluctuations in a patient with cochlear Meniere's disease would contra

A modern conceptual framework for study and treatment of Meniere's disease.

Prosper Meniere made his immortal contribution to the field of otology in 1861. At that time, all manner of "fits" were lumped together under the diagnosis of "apoplectiform cerebral congestion"-too much blood in the brain. His genius was to identify a specific subset of this heterogeneous pool whose cardinal symptoms, tinnitus, fluctuating progressive deafness, and episodic vertigo, were due to dysfunction of the inner ear. Seventy-seven years later, in 1938, Hallpike and Cairns in England and Yamakawa in Japan identified cochleosaccular endolymphatic hydrops (EH) as the histopathologic corre

Visualization of the saccule and utricle with non-contrast-enhanced FLAIR sequences.

OBJECTIVES: 3D-fluid attenuation inversion recovery (FLAIR) collected 4 h after intravenous gadolinium injection can delineate the perilymphatic space (PLS) from the endolymphatic space (ELS) to capture endolymphatic hydrops, the pathological counterpart of Ménière's disease. We aimed to optimize visualization of such inner ear internal anatomy using 3D-FLAIR without injection. METHODS: 3D-FLAIR signal from different fluid compartments such as PLS and ELS was first simulated. Then, twenty-two healthy subjects were scanned at 3.0-T MRI with non-injected 3D-FLAIR using variable T2 preparations

Recurrent vestibulopathy: are cVEMP, oVEMP and inner ear MRI useful to distinguish patients with Menière's disease and vestibular migraine?

PURPOSE: The aim of this study was to assess the rate of endolymphatic hydrops (EH) on MRI and the rate of otolithic dysfunction with cVEMP and oVEMP, in patients with recurrent vertigo such as Menière's disease (MD), vestibular migraine (VM) and vestibular Menière's disease (vMD). METHODS: In this retrospective study, we performed 3D-FLAIR sequences with delayed acquisition in 20 MD, 20 VM and 20 vMD patients. Each subject was then assessed for the presence of EH on MRI. All patients underwent pure-tone audiometry, cVEMP and oVEMP. RESULTS: In MD patients, EH was observed in 18 (90%) out of

Rate influences on tone burst summating potential amplitude in electrocochleography: clinical(a) and experimental(b) data.

Electrocochleographic recordings of action and summating potentials are widely used in the electrophysiological assessment of endolymphatic hydrops (ELH). Increased amplitudes of the summating potential (SP) in response to tone burst stimuli are indicative of positive ELH. This study reports the effect of repetition rate of tone burst stimulation on the SP amplitude. Using transtympanic electrocochleography (ECochG), the SP in response to 1 kHz tone bursts was recorded in both a Ménière and a non-Ménière population. Absolute values of the SP were systematically higher in the Ménière group. Mor
