Survivor, family and professional experiences of psychosocial interventions for sexual abuse and violence: a qualitative evidence synthesis.

BACKGROUND: It is well-established that experiencing sexual abuse and violence can have a range of detrimental impacts; a wide variety of interventions exist to support survivors in the aftermath. Understanding the experiences and perspectives of survivors receiving such interventions, along with those of their family members, and the professionals who deliver them is important for informing decision making as to what to offer survivors, for developing new interventions, and enhancing their acceptability. OBJECTIVES: This review sought to: 1. identify, appraise and synthesise qualitative stud

Effectiveness of a culturally appropriate intervention to prevent intimate partner violence and HIV transmission among men, women, and couples in rural Ethiopia: Findings from a cluster-randomized controlled trial.

BACKGROUND: Intimate partner violence (IPV) is associated with increased HIV risk and other adverse health and psychosocial outcomes. We assessed the impact of Unite for a Better Life (UBL), a gender-transformative, participatory intervention delivered to men, women, and couples in Ethiopia in the context of the coffee ceremony, a traditional community-based discussion forum. METHODS AND FINDINGS: Villages (n = 64) in 4 Ethiopian districts were randomly allocated to control, men's UBL, women's UBL, or couples' UBL, and approximately 106 households per village were randomly selected for inclus

A scoping review of the determinants of foetal alcohol spectrum disorder in South Africa: an intersectional perspective.

Foetal alcohol spectrum disorder (FASD) affects babies born to mothers who consume alcohol while pregnant. South Africa has the highest prevalence of FASD in the world. We review the social determinants underpinning FASD in South Africa and add critical insight from an intersectional feminist perspective. We undertook a scoping review, guided by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses for Scoping Reviews guidelines. Drawing from EBSCOhost and PubMed, 95 articles were screened, with 21 selected for analysis. We used the intersectionality wheel to conceptualize how

Theorizing "Big Events" as a potential risk environment for drug use, drug-related harm and HIV epidemic outbreaks.

Political-economic transitions in the Soviet Union, Indonesia, and China, but not the Philippines, were followed by HIV epidemics among drug users. Wars also may sometimes increase HIV risk. Based on similarities in some of the causal pathways through which wars and transitions can affect HIV risk, we use the term "Big Events" to include both. We first critique several prior epidemiological models of Big Events as inadequately incorporating social agency and as somewhat imprecise and over-generalizing in their sociology. We then suggest a model using the following concepts: first, event-specif

Functional/dissociative seizures in a single women's rehabilitation center in Addis Ababa, Ethiopia: a single-center case series and thematic analysis of neuropsychiatric presentation, cultural meaning, and structural barriers to care.

BACKGROUND: Functional/dissociative seizures (FDS; previously termed psychogenic non-epileptic seizures, PNES) are disabling functional neurological events associated with trauma, dissociation, psychiatric comorbidity, impaired functioning, and increased mortality. In sub-Saharan Africa, limited access to video-electroencephalography (video-EEG), limited specialist services, and culturally embedded explanatory models complicate recognition and treatment. To our knowledge, there are no published Ethiopian case-based studies of FDS employing structured clinical formulation and thematic analysis.

Systematic review of facility-based sexual and reproductive health services for female sex workers in Africa.

BACKGROUND: Several biological, behavioural, and structural risk factors place female sex workers (FSWs) at heightened risk of HIV, sexually transmitted infections (STIs), and other adverse sexual and reproductive health (SRH) outcomes. FSW projects in many settings have demonstrated effective ways of altering this risk, improving the health and wellbeing of these women. Yet the optimum delivery model of FSW projects in Africa is unclear. This systematic review describes intervention packages, service-delivery models, and extent of government involvement in these services in Africa. METHODS:

Mental health of university students twenty months after the beginning of the full-scale Russian-Ukrainian war.

BACKGROUND: Russia's full-scale military invasion of Ukraine on February 24, 2022 led to an increase of anxiety and depressive states, psychosomatic manifestations, and a tendency to abuse alcohol and psychoactive substances in the population. The aim of this paper is to examine the mental health burden among university students twenty months after war and to identify risk and protective factors for mental health problems. METHODS: A cross-sectional study was conducted among Ukrainian students in October 2023 (n = 1398). The online survey included sociodemographic data collection, evaluation

Identifying longitudinal patterns of HIV treatment (dis)engagement and re-engagement from oral histories of virologically unsuppressed persons in Uganda: A thematic trajectory analysis.

BACKGROUND: There is limited study of persons deemed "harder to reach" by HIV treatment services, including those discontinuing or never initiating antiretroviral therapy (ART). We conducted narrative research in southern Uganda with virologically unsuppressed persons identified through population-based sampling to discern longitudinal patterns in HIV service engagement and identify factors shaping treatment persistence. METHODS: In mid-2022, we sampled adult participants with high-level HIV viremia (≥1000 RNA copies/mL) from the prospective, population-based Rakai Community Cohort Study. Usi

Associations between childhood threat and deprivation experiences, self- and other-mentalizing, and adult psychopathology: evidence from a community sample.

INTRODUCTION: Childhood adversity is a well-established transdiagnostic risk factor for psychopathology, yet the mechanisms linking specific adversity types to mental health remain unclear. The Dimensional Model of Adversity and Psychopathology (DMAP) distinguishes deprivation, defined as the absence of expected social and cognitive inputs, from threat, defined as exposure to violence or danger, and links them to impairments in cognitive and socio-emotional development. These forms of adversity may critically impact socio-cognitive mechanisms such as mentalization, the capacity to interpret on

Trauma, empowerment, and resilience: understanding the mental health concerns of people living with human immunodeficiency virus in Manitoba.

INTRODUCTION: Canada has endorsed the Joint United Nations Programme on HIV/AIDS (UNAIDS) target to end the HIV epidemic by 2030. However, Canada has reported increases in HIV diagnoses for five consecutive years since 2019, with Manitoba's provincial rates three times the national average. This study examines mental health factors that may impact engagement in care among people living with HIV in Manitoba. METHODS: For this qualitative study, 32 Manitoban women, men and gender-diverse people living with HIV completed semi-structured interviews on barriers and facilitators to HIV care; as wel

Forced vaginal sex and genital immune correlates of HIV risk: a prospective study of female sex workers in Kenya.

BACKGROUND: The likelihood of HIV acquisition is increased following forced vaginal sex. This relates in part to epidemiological and behavioural factors; however, the biological effects of forced vaginal sex, including impacts on immune parameters linked to HIV susceptibility, are poorly understood. Here, we examine biological mediators of HIV susceptibility among female sex workers (FSWs) in Nairobi, Kenya, who recently experienced forced vaginal sex. METHODS: The Maisha Fiti study was a longitudinal cohort study of FSWs from Nairobi, Kenya. At up to three visits, HIV-uninfected participants

A qualitative exploration of contraceptive use and discontinuation among women with an unmet need for modern contraception in Kenya.

BACKGROUND: Addressing the unmet need for modern contraception underpins the goal of all family planning and contraception programs. Contraceptive discontinuation among those in need of a method hinders the attainment of the fertility desires of women, which may result in unintended pregnancies. This paper presents experiences of contraceptive use, reasons for discontinuation, and future intentions to use modern contraceptives. METHODS: Qualitative data were collected in two rural counties in Kenya in 2019 from women with unmet need for contraception who were former modern contraceptive users

Addressing multiple modifiable risks through structured community-based Learning Clubs to improve maternal and infant health and infant development in rural Vietnam: protocol for a parallel group cluster randomised controlled trial.

INTRODUCTION: Optimal early childhood development is an international priority. Risks during pregnancy and early childhood have lasting effects because growth is rapid. We will test whether a complex intervention addressing multiple modifiable risks: maternal nutrition, mental health, parenting capabilities, infant health and development and gender-based violence, is effective in reducing deficient cognitive development among children aged two in rural Vietnam. METHODS AND ANALYSIS: The Learning Clubs intervention is a structured programme combining perinatal stage-specific information, learn
