OBJECTIVE: To determine the efficacy of a new multilevel surgical protocol for obstructive sleep apnea (OSA).
STUDY DESIGN AND SETTING: Sixty patients with moderate to severe OSA because of multilevel pharyngeal obstruction were enrolled into this prospective, controlled clinical trial after clinical examination, endoscopy, and polysomnography. Surgery included uvulaflap, tonsillectomy, hyoid suspension, and radiofrequency treatment of the tongue base (group A). A second group did not receive hyoid suspension (group B). In both groups, nasal surgery was performed if necessary. Polysomnography

PURPOSE: Mandibular trauma can result in stylomandibular complex (SMC) fractures with clinical symptoms including glossopharyngeal neuralgia, globus pharyngeus, otalgia, and dysphonia. Fracture of the styloid process is noteworthy because of its proximity to numerous vital structures. The study objectives were 1) to determine the incidence of SMC fractures and develop a clinical grading system and 2) to institute and assess a treatment protocol.
MATERIALS AND METHODS: This prospective clinical trial involved 206 patients with mandibular trauma based on a set of inclusion and exclusion criteri

BACKGROUND: The transhyoid approach for the resection of squamous cell carcinoma (SCC) of the base of the tongue continues to evolve and remains controversial. We previously reported that the functional outcome of this operation is superior to that of the traditional transmandibular approaches.
OBJECTIVE: To report our long-term survival rates for T1, T2, and select T3 SCCs of the base of the tongue using the transhyoid approach.
PATIENTS AND METHODS: Twenty-eight patients with SCC of the base of the tongue were treated using a transhyoid approach at the University of California, Los Angeles

OBJECTIVES: To assess the diagnostic accuracy (ACC) of post-mortem computed tomography (PMCT) for fractures of the isolated larynx-hyoid complex (LHC) in comparison to post-mortem fine preparation (PMFP).
METHODS: This monocentric prospective study enclosed 54 LHCs that were extracted during autopsy, fixed in formalin, and underwent a PMCT scan (64-row multidetector CT, helical pitch). Two radiologists independently analyzed the LHC scans for image quality (IQ) and fractures (4-point Likert scales). A specialized forensic preparator dissected the specimens under the stereomicroscope. The PMFP

We explored the value of postmortem computed tomography (PMCT) to augment autopsy in evaluating strangulation fatalities. A literature search identified 16 studies describing autopsy findings in 576 deaths and two studies describing autopsy and PMCT findings in six deaths. Similar cases were identified from our institution, yielding 130 deaths with autopsy findings and 14 deaths with both autopsy and PMCT findings. The presence of laryngohyoid fracture and soft tissue hemorrhage was compared from autopsy and autopsy+PMCT cases. The detection rates of fractures in autopsy and autopsy+PMCT cases

Background/Objectives: In patients without clinically anticipated difficulty, an unexpected difficult airway carries the greatest potential for harm, yet routine bedside tests exclude difficulty poorly, and evidence for point-of-care ultrasonography remains heterogeneous, with difficult mask ventilation rarely studied as a distinct outcome. We evaluated three rapid anterior-neck ultrasonographic parameters-tongue thickness (TT), skin-to-hyoid-bone distance (SHBD), and cricothyroid membrane length (CTML)-for their diagnostic performance and independent association with a difficult airway in adu

BACKGROUND: Accurate preoperative prediction of difficult intubation is essential for perioperative safety. Ultrasonographic airway evaluation has emerged as a promising adjunct. However, heterogeneity in study populations has limited the identification of population-specific predictors. This study aimed to evaluate the diagnostic performance of four ultrasonographic airway parameters in non-obese, non-obstetric adult female patients.
METHODS: This prospective, single-center, single-blinded observational study included adult women undergoing elective surgery. Preoperative airway ultrasonograp

Oropharyngeal dysphagia in chronic post-stroke (PS-OD) patients is associated with impaired oropharyngeal sensory/motor function. We aimed to evaluate the effect of transcutaneous electrical stimulation (TES) on the biomechanics and kinetics of swallowing in PS-OD patients in a randomized controlled clinical trial with 1-year follow-up. 71 PS-OD patients with impaired safety of swallow (ISS) were randomized into 2 groups, (1) 26 with compensatory treatment (CT): thickeners and texture-modified diets, and (2) 45 with active treatment (TES + CT) with 2 cycles of TES (VitalStim; Enovis, USA), at

Obstructive sleep apnea (OSA) is closely associated with the development and chronicity of temporomandibular disorder (TMD). Given the intricate pathophysiology of both OSA and TMD, comprehensive diagnostic approaches are crucial. This study aimed to develop an automatic prediction model utilizing multimodal data to diagnose OSA among TMD patients. We collected a range of multimodal data, including clinical characteristics, portable polysomnography, X-ray, and MRI data, from 55 TMD patients who reported sleep problems. This data was then analyzed using advanced machine learning techniques. Thr

BACKGROUND: Pathogenesis of obstructive sleep apnoea (OSA) is complex and not yet fully understood. Several factors contribute to OSA severity. Obesity is believed to play an important role. Nevertheless, not all OSA patients are obese. Therefore, the different features that cause nocturnal upper airway obstruction in obese and non-obese OSA patients could be expected.
PURPOSE: To investigate the different components of cervico-craniofacial skeletal and upper airway soft tissue morphology among obese OSA, non-obese OSA patients and the controls.
PATIENTS: One hundred male OSA patients were c

BACKGROUND: New World monkeys (Platyrrhini), a highly diverse primate lineage endemic to Mexico, Central and South America, include howler monkeys (Alouatta spp.), one of the most ecologically successful genera in the region. They have evolved key adaptations including a specialized folivorous diet and distinctive howling calls that carry over long distances in tropical rainforests, conferring critical survival advantages. However, the genetic underpinnings of these adaptations-specifically, the genes involved in fiber digestion, fermentation, and metabolic adaptation for folivory, and those r

The exact mechanism of sarcopenic dysphagia following esophagectomy, which is associated with reduced pharyngeal contraction, remains unclear. This study measured the maximum pharyngeal constriction area normalized (MPCAn) as a potential indicator of post-esophagectomy sarcopenic dysphagia. Videofluoroscopic examination of swallowing study was conducted in 134 patients with suspected dysphagia following esophagectomy, defined as a score of ≥3 in the 8-point penetration-aspiration scale. The area under the receiver operating characteristic curve of MPCAn was applied to detect dysphagia in compa
