Removal of vestibular schwannoma and facial nerve preservation using small suboccipital retrosigmoid craniotomy.

BACKGROUND: Vestibular schwannoma, the commonest form of intracranial schwannoma, arises from the Schwann cells investing the vestibular nerve. At present, the surgery for vestibular schwannoma remains one of the most complicated operations demanding for surgical skills in neurosurgery. And the trend of minimal invasion should also be the major influence on the management of patients with vestibular schwannomas. We summarized the microsurgical removal experience in a recent series of vestibular schwannomas and presented the operative technique and cranial nerve preservation in order to improve

Characteristics of repeated untethering pattern in patients with myelomeningocele from infancy to adolescence.

OBJECTIVE: Radiological evidence of secondary spinal cord tethering has been observed in patients with myelomeningocele (MMC), even after primary surgical treatment. A significant number of these patients may develop symptomatic tethered cord syndrome (TCS) several times during the clinical course, with worsening of preexisting neurological deficits, or bladder or bowel function. Early untethering is necessary to prevent further progression of symptoms. The aim of this study was to investigate factors influencing postoperative outcome of spinal cord untethering (SCU) on recurrent TCS. METHODS

Surgical resection and targeted therapy in a pediatric NTRK-rearranged low-grade spindle cell sarcoma: a case report.

BACKGROUND: Neurotrophic tropomyosin receptor kinase (NTRK) gene fusions have emerged as important oncogenic drivers across a wide range of malignancies, including soft tissue sarcomas. Early detection of these fusions facilitates precision treatment with TRK inhibitors, significantly improving clinical outcomes. However, diagnosing NTRK-rearranged spindle cell neoplasms (NTRK-RSCNs) remains challenging due to their histological heterogeneity and overlap with other soft tissue tumors. CASE PRESENTATION: We present the case of an 8-year-old boy with a history of infantile fibromatosis, who dev

Transcondylar Approach for Resection of Medullary Cavernous Malformation: 2-Dimensional Operative Video.

Cavernous malformations located within the brainstem present with a high rate of neurological symptoms and carry a more aggressive course in both pediatric and adult populations.1,2 Cavernomas within the medulla are the rarest form, representing only 5% of all brainstem lesions.3 Repeated hemorrhage of brainstem cavernomas is associated with significant and cumulative neurological deficits and thus requires treatment.4 Microsurgical resection has become the optimal mode of treatment with the aim of resecting the live malformation and not merely the multiaged, organized hematoma.4 This is best

Giant Multiloculated Cystic Trochlear Nerve Schwannoma Between the Tentorial Leaflets: Case Report and Systematic Review.

BACKGROUND: Trochlear nerve schwannomas (TNSs) are exceptionally rare benign tumors, accounting for a very small fraction of intracranial schwannomas. Due to their deep locations and slow growth, they often reach a considerable size before diagnosis. Giant or multiloculated cystic variants are exceptionally uncommon and present significant diagnostic and surgical challenges. METHODS: A 32-year-old female presented with headache and dizziness but no diplopia. Magnetic resonance imaging revealed a giant multiloculated cystic lesion extending from the posterior fossa to the right temporal lobe,

Occipital Plate Fixation in the Pediatric Population.

BACKGROUND: Occipital plate fixation has been shown to improve outcomes in cervical spine fusion. There is a paucity of literature describing occipital plate fixation, especially in the pediatric population. The authors reviewed a case series of 34 patients at a pediatric hospital who underwent cervical spine fusion with occipital plate fixation between 2003 and 2016. This study describes how occipital plates aid the cervical spine union in a case series of diverse, complex pediatric patients. METHODS: Our orthopaedic database at our institution was queried for patients undergoing an instrume

Neurologic and Neuropsychological Outcomes for Treatment of Unruptured Middle Cerebral Artery Aneurysms: Standard Pterional Versus Minipterional Approach in a Retrospective Single-Center Analysis.

BACKGROUND: In accordance with technique advancement and minimal invasiveness surgical approaches, the minipterional has progressively replaced the standard pterional approach for treatment of unruptured middle cerebral artery (MCA) aneurysms. Nowadays, multimodal intraoperative resources including microDoppler and microflow probes, indocyanine green videoangiography, and neurophysiologic monitoring constitute a fundamental prerequisite for increasing the safety of the clipping procedure. Our study investigated and compared in a single-center experience the effect of the evolution of a minimal

Clinical utility of intraoperative direct wave monitoring in patients with pre-operative motor deficits: Case series.

INTRODUCTION AND IMPORTANCE: D-wave (Direct waves) are Motor Evoked Potentials (MEPs) generated by a single transcranial stimulation and captured by attaching an epidural recording electrode caudal to the vulnerable area. Intraoperative neurophysiologic monitoring (IONM) is widely used in neurosurgery to recognize important neurological structures but can be challenging in the pediatric population due to incomplete neural development. CASE PRESENTATION: Case 1: A 48-year-old female presented to the outpatient department with complaints of difficulty walking for the past six months, numbness a

White cord syndrome following posterior decompression and fusion for severe OPLL and an acute traumatic cervical injury - A case report and review of literature.

BACKGROUND: White cord syndrome (WCS) refers to the observation of intramedullary hyperintensity due to edema/ischemia and swelling on postoperative T2-weighted MRI sequences in the setting of unexplained neurological deficits after cervical spinal cord decompression. Pathophysiologically, WCS/reperfusion injury (RPI) occurs due to oxygen derived free radicals as a result of acute reperfusion or direct trauma from blood flow itself. Intraoperative neurophysiologic monitoring (IONM) can give early warning and detect neurologic deficits. Here, we are presenting a case of a patient who had a chro

Microsurgical Resection of an Intramedullary Ependymoma at the Cervicomedullary Junction: A Two-Dimensional Operative Video.

Ependymomas are the most common adult intramedullary spinal tumors.1 Although uncommon in the brainstem, ependymomas make up a large proportion of tumors of this location.2-8 We present an operative video case report of an intrinsic ependymoma at the cervicomedullary junction. The purpose of this report is to present the clinical picture, operative setup, and surgical technique involved in resection of an intramedullary tumor of this region. For best outcome for intramedullary ependymomas, the goal should be gross total resection.1,9,10 These tumors have a relatively distinct plane between tum

Bilateral phrenic nerve palsy after posterior cervical decompression and fusion surgery: a rare event after surgery.

INTRODUCTION: Delayed C5 weakness is a known entity in cervical spine surgery, although with varied clinical presentation and poorly understood mechanism of action. We describe the first case in the literature of a bilateral C5 palsy leading to bilateral phrenic nerve dysfunction following a posterior cervical decompression and fusion. CASE REPORT: A 76-year-old male presented with low back pain and was diagnosed as myelopathic. On initial neurological examination, he could not ambulate without assistance and was unsteady on tandem gait. The initial cervical MRI and CT scan showed advanced mu

Microsurgical Resection of the IV Ventricle Subependymoma: 2-Dimensional Operative Video.

In this video, we demonstrate microsurgical resection of IV ventricle subependymoma. To the best of our knowledge, this is the first video case report of a microsurgical resection of subependymoma of the IV ventricle in the peer-reviewed English literature. Subependymomas are benign central nervous system tumors, typically arising in ventricular spaces, mostly in the IV and lateral ventricles.1-3 They are isointense on T1 and hyperintense on T2-weighted magnetic resonance imaging (MRI) with minimal or no enhancement.4 Microsurgery remains the mainstay treatment. Complete tumor resection is pos
