BACKGROUND: Leprosy or Hansen's disease (HAD) undoubtedly remains an emergency in certain countries. It is an ancient deforming disease caused by Mycobacterium leprae. The countries with the highest endemic leprosy rate in 2000 were Brazil, India and Madagascar. In Italy, the old epidemic has been defeated and there are approximately 400 patients under constant monitoring with three to four new cases per year involving Italian residents. The kidney is one of the target organs during the splanchnic localization of leprosy. The histopathological renal lesion spectrum includes glomerulonephritis

The present distribution of animals and plants throughout the world is the result of an evolutionary process involving tectonic, climatic and biotic factors. Humans, since their appearance, have contributed to the spreading of many species including disease vectors and pests. When humans left their native African home, they brought with them ectoparasites such as lice and acarids. During the neolithic era, humans were leading domesticated animals which carried their own parasites into new areas. Dwelling commensals, flea, bugs, triatomids, flies, and cockroaches followed human migrations. In t

Authors record the manifestations on the child of the severe malaria epidemic that raged in Antananarivo. Using the hospitalisation registers of the Debré room, Pediatric Service "A", at the Hospital of Befelatanana as source documents, they underline some points: Concerning morbidity, the epidemic started in 1984, reached its highest point in 1988 (26.7%, of in patients) and began to fade away in 1992. Boys and girls were equally affected, with a majority among children of 5 years old and more (51 to 58% of cases). All patients came from the center of Antananarivo (33.5%) or mainly from the s

BACKGROUND: Zero-dose (ZD) children remain concentrated in low- and lower-middle-income countries (LLMICs) and are a major focus of the Immunization Agenda 2030 (IA2030), which aims to halve their number by 2030. Despite this commitment, progress has stalled in many high-burden settings. This study aimed to examine progress in reaching ZD children, trends in wealth-related inequity, and key population-modifiable risk factors across LLMICs over the past decade.
METHODS: We analysed two rounds of the most recent Demographic and health survey (DHS) datasets from ten LLMICs to assess progress in

BACKGROUND: Orofacial clefts impose a substantial burden on low- and middle-income countries (LMICs), yet evidence on preventable environmental determinants, including agrochemicals, remains sparse.
OBJECTIVE: To quantify the association between maternal agrochemical exposure during pregnancy and nonsyndromic orofacial clefts (NSOFCs) across eight LMICs.
METHODS: This population-sampled, unmatched case-control study included questionnaires administered to mothers of children with NSOFCs (cases) and mothers of unaffected children (controls) in the Democratic Republic of Congo (DRC), Guatemala

IMPORTANCE: Surgical site infections (SSI) are a leading cause of morbidity and mortality from surgery, with higher rates in low- and middle-income countries (LMICs). Clean Cut is a multimodal, adaptive quality improvement program that aims to reduce SSI by improving compliance with perioperative infection prevention standards. The program has been successfully implemented in Ethiopia at 12 hospitals with an associated 35% reduction in SSI.
OBJECTIVE: To assess whether this surgical infection prevention program implemented in Ethiopia can be effectively scaled to a variety of geographical and

BACKGROUND: Low birth weight, defined as less than 2.5 kg (5.5 lbs) at birth, remains a critical global public health challenge. It significantly increases the risk of neonatal mortality and immediate complications such as sepsis and hypothermia, along with lifelong consequences including childhood disabilities and adult-onset chronic diseases. However, there was a limited study that described the spatial distribution and predictors of low birth weight in sub-Saharan Africa. The study aimed to assess geospatial variations and predictors of low birth weight in sub-Saharan Africa.

BACKGROUND: Plague is a high-consequence infectious disease with epidemic potential. Current treatment guidelines are based on weak evidence.
METHODS: We enrolled persons (excluding pregnant persons) in Madagascar who had clinically suspected bubonic plague during 2020-2024. Using an open-label noninferiority design, we compared two treatments included in the national plague guidelines: oral ciprofloxacin for 10 days (ciprofloxacin monotherapy) or injectable aminoglycoside for 3 days followed by oral ciprofloxacin for 7 days (aminoglycoside-ciprofloxacin). The primary end point was treatment

BACKGROUND: Anopheles stephensi is an invasive malaria vector in Africa that threatens to put an additional 126 million people at risk of malaria if it continues to spread. The island nation of Mauritius is highly connected to Asia and Africa and is at risk of introduction due to this connectivity. For early detection of An. stephensi, the Vector Biology and Control Division under the Ministry of Health in Mauritius, leveraged a well-established Aedes program, as An. stephensi is known to share Aedes habitats. These efforts triggered multisectoral coordination and cascading benefits of integra

BACKGROUND: Malaria caused by Plasmodium vivax was long considered to have a low mortality, but recent reports from some geographical areas suggest that severe and complicated vivax malaria may be more common than previously thought.
METHODS: The primary objective of this systematic review and meta-analysis was to describe the reported clinical characteristics and the geographical variation in prevalence of reported severe vivax malaria and its change over time derived from English-language articles published since 1900. Medline and Scopus databases were searched for original papers on severe

BACKGROUND: Smoking has a major influence on the development and worsening of low back pain (LBP). However, the effect of smoking on the burden of LBP has not been thoroughly examined at the global, regional, and national levels. This study aims to analyze the trends in smoking-related LBP from 1990 to 2021. It uses data from the 2021 Global Burden of Disease (GBD), providing scientific evidence for policy-making.
METHODS: This observational study, based on population data, used epidemiological information on LBP due to smoking from the GBD 2021 study. The study categorized the disability-adj

BACKGROUND: Computer-aided detection (CAD) algorithms for automated chest X-ray (CXR) reading have been endorsed by the World Health Organization for tuberculosis (TB) triage, but independent, multi-country assessment and comparison of current products are needed to guide implementation.
METHODS: We conducted a head-to-head evaluation of five CAD algorithms for TB triage across seven countries. We included CXRs from adults who presented to outpatient facilities with at least two weeks of cough in India, Madagascar, the Philippines, South Africa, Tanzania, Uganda, and Vietnam. BACKGROUND: Leprosy is an ancient infectious disease with an annual global incidence of around 200,000 over the past decade. Since 2018, the World Health Organization (WHO) recommends single-dose rifampicin as post-exposure prophylaxis (SDR-PEP) for contacts of leprosy patients. The Post ExpOsure Prophylaxis for Leprosy (PEOPLE) trial evaluated PEP with a double dose of rifampicin in Comoros and Madagascar. Preliminary results of this trial show some reduction in leprosy incidence in intervention villages but a stronger regimen may be beneficial. The objective of the current Bedaquiline Enhanc

The authors report the results of a randomized epidemiological survey aiming to assess the sero-prevalence of plague in the general population > or = 2-year-old in Mahajanga. In 656 sera tested (by ELISA), the prevalence of anti-F1 antibodies was found to be 6.1%@1000 inhabitants, close to the expected prevalence in the area, where plague reappeared in 1991 after 62 years of absence. The study also demonstrated that the shrew, Suncus murinus, is an important reservoir in the plague transmission in Mahajanga.

Madagascar periwinkle [Catharanthus roseus (L.) G. Don] produces vindoline and catharanthine, key precursors used in the semi-synthesis of the high-value anticancer alkaloids vinblastine and vincristine. Closed plant production systems (CPPS) allow precise application of environmental stimuli, including light spectrum and plasma-activated water (PAW). In a previous study, we demonstrated the potential of combining artificial light (white and red) and PAW to enhance the production of these alkaloids in periwinkle grown under controlled CPPS conditions. The present study expands this approach by
