Assessing risk factors for postoperative nausea and vomiting: a retrospective study in patients undergoing retromastoid craniectomy with microvascular decompression of cranial nerves.

We have observed that patients, after retromastoid craniectomy (RMC) with microvascular decompression (MVD) of cranial nerves, frequently experienced postoperative nausea and vomiting (PONV). The authors conducted this study to track the incidence of PONV and to identify potential factors for PONV after RMC with MVD. Medical records from 185 adults, who underwent elective RMC with MVD, were identified and reviewed from January 2000 to December 2004. Extracted data included patient, anesthesia, and surgery related variables that were considered to have a possible effect on the patients experien

Some patients with multiple sclerosis have neurovascular compression causing their trigeminal neuralgia and can be treated effectively with MVD: report of five cases.

The role of trigeminal ganglion percutaneous injection and radio-frequency lesioning procedures for the treatment of trigeminal neuralgia (TGN) in multiple sclerosis (MS) is well established. There is general acceptance that microvascular decompression (MVD) cannot be an appropriate treatment due to the view that the underlying aetiology is a demyelinating plaque affecting the root entry zone of the trigeminal pathway. Recently, MR-imaging has been used in the preoperative investigation of this group of patients demonstrating that neurovascular compression can occasionally be the responsible m

Sensitivity and specificity of MRA in the diagnosis of neurovascular compression in patients with trigeminal neuralgia. A correlation of MRA and surgical findings.

The published rates of operatively confirmed neurovascular compression as the cause for trigeminal neuralgia range from 10% to nearly 100%. High-definition magnetic resonance angiography (MRA) was performed in 27 consecutive patients (in 6 cases with 3D reconstructions) to show neurovascular compression preoperatively. The MRA findings were compared with the relationship between the Vth nerve and the surrounding vessels at surgery. In 23 patients MRA showed present neurovascular compression in accordance with surgical findings (18/27 in complete accordance of type and side of vessel, site and

Preventive and therapeutic effects of Tanshinone IIA on spinal cord injury without radiographic abnormality by regulating microglial phenotype polarization.

Spinal cord injury without radiographic abnormality (SCIWORA) occurs in patients with cervical spinal stenosis (CSS) following minor trauma, often leading to significant neurological deficits. While surgical decompression reduces SCIWORA risk, some patients are ineligible or reluctant to undergo surgery. Consequently, reducing the risk of SCIWORA in the absence of surgical intervention and optimizing spinal cord function recovery remain pivotal challenges. This study explores the potential of Tanshinone IIA (TIIA) in preventing and treating SCIWORA by modulating microglial polarization and att

Microvascular decompression for glossopharyngeal neuralgia using intraoperative neurophysiological monitoring: Technical case report.

BACKGROUND: Glossopharyngeal neuralgia (GN) is a rare functional disorder representing around 1% of cases of trigeminal neuralgia. Lancinating throat and ear pain while swallowing are the typical manifestations, and are initially treated using anticonvulsants such as carbamazepine. Medically refractory GN is treated surgically. Microvascular decompression (MVD) is reportedly effective against GN, superseding rhizotomy and tractotomy. METHODS: We encountered three patients with medically refractory GN who underwent MVD using intraoperative neurophysiological monitoring (IONM). The offending ve

Gamma knife radiosurgery as the first surgery for trigeminal neuralgia.

To evaluate the role of Gamma Knife radiosurgery as the first surgical therapy in the management of medically refractory trigeminal neuralgia, we reviewed outcomes on our first 23 patients who had radiosurgery as primary surgical therapy. These patients represented 19% of our overall series. Mean patient age was 66 years, and mean follow-up after radiosurgery was 12 months (range 5-33 months). For most patients, radiosurgery was performed because the patient had medical contraindications to open surgery. 14 patients had 70-Gy radiosurgery, and 9 patients, 80 Gy. Radiosurgery was performed usin

Large persistent trigeminal artery variant that supplied the subtotal cerebellar hemisphere and caused trigeminal neuralgia, which was diagnosed by magnetic resonance (MR) angiography and MR cisternography.

PURPOSE: To describe a case of large persistent trigeminal artery (PTA) variant that caused trigeminal neuralgia, which was diagnosed by magnetic resonance (MR) angiography and MR cisternography. CASE REPORT: An 82-year-old woman with left trigeminal neuralgia underwent cranial MR imaging, MR angiography and MR cisternography. MR imaging revealed no significant abnormality. MR angiography showed that a relatively large artery arose from the precavernous segment of the left internal carotid artery (ICA) without connection to the basilar artery, which is indicative of a PTA variant. This artery

Targeting the cranial nerve: microradiosurgery for trigeminal neuralgia with CISS and 3D-flash MR imaging sequences.

OBJECT: The authors undertook a study to identify magnetic resonance (MR) imaging techniques that can be used reliably during gamma knife surgery (GKS) to identify the trigeminal nerve, surrounding vasculature, and areas of compression. METHODS: Preoperative visualization of the trigeminal nerve and surrounding vasculature as well as targeting the area of vascular compression may increase the effectiveness of GKS for trigeminal neuralgia. During the past years our gamma knife centers have researched different MR imaging sequences with regard to their ability to visualize cranial nerves and va

Measurement of pulse pressure profiles in patients with trigeminal neuralgia.

Applanation tonometry is a non-invasive method of assessing the arterial blood pressure profiles in both the peripheral and systemic circulation. In this study the authors examined whether there were differences in these profiles in patients with trigeminal neuralgia. The carotid artery and derived aortic blood pressure waveforms were obtained using a pulse wave analysis system. The ratio of the pressure wave amplitude above the systolic shoulder to the total systolic blood pressure (augmentation index, AIx) was recorded. Thirty two patients with trigeminal neuralgia (16 male and 16 female) an

Comparative safety and efficacy of percutaneous radiofrequency thermocoagulation and percutaneous balloon compression in CT-guided and local anesthesia for recurrent trigeminal neuralgia.

BACKGROUND: There are several ways to treat trigeminal neuralgia (TN); however, TN may recur after treatment. Although microvascular decompression (MVD) is considered an effective treatment for trigeminal neuralgia, patients with recurrence may not be willing to undergo craniotomy. OBJECTIVE: This study compared the safety and efficacy of percutaneous radiofrequency thermocoagulation and percutaneous balloon compression for treating recurrent trigeminal neuralgia. METHODS: This was a prospective non-randomized controlled study. A total of 52 with recurrent TN were scheduled to undergo surger

Primary somatosensory cortex oscillations in trigeminal neuralgia: laser-evoked signatures and their potential relevance to microvascular decompression.

Classical trigeminal neuralgia (TN) is a severe chronic pain disorder characterized by sudden, intense facial pain attacks and represents a major burden for affected individuals. Microvascular decompression (MVD) can provide pain relief, yet not all patients benefit equally. A key challenge in selecting candidates for MVD lies in the limited predictive accuracy of current diagnostics, which mainly rely on subjective pain history and structural MRI findings. Since many asymptomatic individuals show neurovascular contact on imaging, its prognostic value remains limited. Electrophysiological meas

Glossopharyngeal neuralgia and radiosurgery.

OBJECT: Glossopharyngeal neuralgia is difficult to treat. On the basis of results obtained by using Gamma Knife surgery (GKS) to treat trigeminal neuralgia, the authors have used GKS to treat glossopharyngeal neuralgia in a series of patients since 2007. Their objectives with this study were to demonstrate the usefulness and safety of GKS for treating glossopharyngeal neuralgia and to describe a simple treatment method. METHODS: From 2007 through 2013, the authors treated glossopharyngeal neuralgia in 5 patients (4 women and 1 man), who ranged in age from 36 to 74 years. One patient had previ
