Determinants of long-term outcome in severe alcoholic hepatitis.

BACKGROUND: Although short-term outcome in severe alcoholic hepatitis (SAH) is well described, its long-term course remains uncharacterised. AIM: To assess determinants of long-term outcome in SAH. METHODS: Data were recorded from a cohort with SAH (admission Discriminant Function (DF) ≥32). Kaplan-Meier (KM) and Cox proportional hazards survival analyses were performed to determine predictors of outcome. RESULTS: One hundred and nine patients were included; 63.3% male, aged 49.6 ± 9.4 years with median follow-up of 40.7 months (95% CI 37.2-44.3). Median DF was 58, 86.2% had cirrhosis and 6

Readiness to change and the role of inpatient counseling for alcohol/substance abusing youth with major trauma.

BACKGROUND: The data regarding trauma recidivism among youth suggest that alcohol and/or substance abuse counseling should be an integral part of an injury prevention program in Level I trauma centers. Analysis of attitudes and motivation toward changing substance use is an important part of counseling. This study explores the willingness of a patient population at risk for recurrent injury to address their alcohol or substance use habits. METHODS: Young patients (ages 15-24) admitted with major trauma and positive alcohol/toxicology screens to a Level I trauma center were offered a previousl

The prevention paradox, traffic safety, and driving-while-intoxicated treatment.

BACKGROUND: In San Juan County, New Mexico, a 28-day jail/treatment program for first-time driving-while-intoxicated (DWI) offenders was established in 1994 to reduce both DWI recidivism and alcohol-related crashes. This paper assesses the impact of the program on both outcomes. METHODS: The data are composed of driving records of all people arrested for DWI in San Juan County from August 1994 through December 2001. Subsequent re-arrests and crashes were analyzed to compare people who had been sentenced to the jail/treatment program and those who had not. Kaplan-Meier survival curves and Cox

Effects of diversion on adults with co-occurring mental illness and substance use: outcomes from a national multi-site study.

This quasi-experimental non-equivalent comparison group study examines outcomes for participants in eight programs conducting criminal justice diversion for people with co-occurring serious mental illness and substance use disorders compared with jail detainees eligible for diversion, but who were processed through standard criminal justice methods without diversion. Nearly 2000 participants were interviewed at baseline, and 1500 at 3 month and 1300 at 12 month follow-up to baseline. In these interviews, outcome measures of re-arrest, mental health functioning, substance abuse, quality of life

Hospital-Based Violence Intervention Program: Impact on Post-Discharge Healthcare Engagement.

INTRODUCTION: Hospital-based violence intervention programs (HVIPs) aim to reduce reinjury and recidivism by addressing underlying determinants of violence. Despite proven benefits, their impact on post-discharge healthcare engagement remains underexplored. Our study sought to evaluate hospital system value of a newly implemented HVIP by assessing post-discharge healthcare engagement. METHODS: A retrospective before-and-after cohort study was conducted at a level I trauma center, comparing patients admitted with penetrating firearm injuries before HVIP implementation (2022) and after HVIP imp

Adverse childhood experiences, mental illness, HIV and offending among female inmates in Durban, South Africa.

BACKGROUND: Childhood adversities and adult trauma are common among female inmates. Associations have been documented with childhood adversities and mental illness, personality disorders, human immunodeficiency virus (HIV) and violent offending. However, no such study had been conducted in South Africa (SA), despite the high prevalence of HIV and trauma in SA. AIM: To measure the prevalence of childhood adversities and adult trauma; and to determine if there is a relationship between childhood adversities, mental illness, personality disorders, HIV and violent offending among female inmates.

The Risk Screener Violence (RS-V): retrospective prediction of violent and aggressive incidents within the prison setting.

INTRODUCTION: Physical and verbal violence toward staff or other detained individuals is a reoccurring problem within correctional facilities. Screening for violence risk within the prison setting could provide a valuable first step in the prevention of institutional violence. The brief and compact Risk Screener Violence (RS-V) has shown to be an efficient new method for assessing concerns regarding post-release violent offending for incarcerated persons. This study aimed to find out whether the RS-V is also able to predict future violent and aggressive incidents during imprisonment. METHODS:

Psychometric properties of the Trauma Checklist 2.0 and its predictive utility of felony re-offending among high-risk juvenile offenders.

BACKGROUND: Incarcerated youth are characterized by particularly high rates of childhood trauma, a significant risk factor for outcomes including risky behaviors and recidivism. Trauma-based interventions can ameliorate the negative effects of childhood trauma; however, a critical part of success is careful trauma screening. Due to the limitations associated with commonly used self-report trauma assessments, our team developed the Trauma Checklist (TCL), a trained-rater assessment of childhood trauma specifically created for use with forensic populations. The TCL is designed to provide a more

Successful Non-Operative Management of Adhesive Small Bowel Obstruction: Is it Really a Success?

BACKGROUND: In adhesive small bowel obstructions (ASBOs), literature has shown that passage of a water-soluble contrast challenge at either 8 hours or 24 hours is predictive of successful non-operative management (NOM) for an ASBO, but the long-term outcomes between these two groups are unknown. We hypothesized that patients who require longer transit times to the colon have a higher one-year recidivism of ASBO. METHODS: This was a 4-year review of patients with presumed ASBO undergoing successful NOM. Those requiring operation or those with an SBO due to something other than adhesions were e

A time to 'make amends and bring pieces together': A phenomenological study of family experiences and considerations when a parent returns home from incarceration.

Nearly 1 in every 14 youth have had a parent incarcerated at some point. In any given year, over 9 million adults from jail and 700,000 adults from prison return to their families and communities. However, few studies have explored the family experiences during re-entry or have provided suggestions on how to best support children during this time. The goal of this qualitative study was to describe family experiences and provide their considerations on how to better support children during the period of parent's re-entry. Qualitative data was gathered from 26 participants using semi-structured,

The mental health of ex-prisoners: analysis of the 2014 English National Survey of Psychiatric Morbidity.

PURPOSE: Prisoners experience extremely high rates of psychiatric disturbance. However, ex-prisoners have never previously been identified in representative population surveys to establish how far this excess persists after release. Our purpose was to provide the first community-based estimate of ex-prisoners' mental health in England using the data from the 2014 Adult Psychiatric Morbidity Survey (APMS). METHODS: APMS 2014 provides cross-sectional data from a random sample (N = 7546) of England's household population aged 16 or above. Standardised instruments categorised psychiatric disorder

Affective Disruption During Forced Ethanol Abstinence in C57BL/6J and C57BL/6NJ Mice.

BACKGROUND: In alcohol-dependent individuals, acute alcohol withdrawal results in severe physiological disruption, including potentially lethal central nervous system hyperexcitability. Although benzodiazepines successfully mitigate such symptoms, this treatment does not significantly reduce recidivism rates in postdependent individuals. Instead, persistent affective disturbances that often emerge weeks to months after initial detoxification appear to play a significant role in relapse risk; however, it remains unclear whether genetic predispositions contribute to their emergence, severity, an
