Cefiderocol-resistant Aeromonas with expanded Resistomes in German hospital wastewater: Phenotypic and genomic evidence from the environment-clinical Interface.

Hospital wastewater is a key interface between clinical and environmental reservoirs of antimicrobial resistance, fostering selection and horizontal gene transfer. Aeromonas spp. are aquatic opportunistic pathogens with highly plastic genomes and are increasingly recognized as potential intermediaries in resistance dissemination. We compared 72 cefiderocol-selected Aeromonas isolates recovered from untreated hospital wastewater collected at six tertiary care hospitals across Germany with 62 clinical isolates from patients with intestinal and extraintestinal infections, to characterize cefidero

Phenotypic and biochemical characterization of the class C β-lactamase, PAC-1.

BACKGROUND: The class C β-lactamase, PAC-1, was originally described as acquired in Pseudomonas aeruginosa clinical isolates from south Asia, the corresponding gene being chromosomally located and embedded within a Tn1721-like transposon. Despite its association with high-level resistance to cephalosporins and β-lactam/β-lactamase inhibitor combinations, data on its full hydrolytic profile and kinetic parameters have not yet been comprehensively characterized. OBJECTIVES: To characterize the susceptibility profile and hydrolytic kinetics of PAC-1, with a particular focus on cefiderocol hydrol

In Silico Mutational Analysis of Two-Component System Genes Associated with Colistin Resistance in Clinical Pseudomonas aeruginosa Isolates from Peshawar.

Pseudomonas aeruginosa is an opportunistic pathogen causing healthcare-associated infections. Colistin is a last-resort antibiotic for multidrug-resistant Gram-negative bacteria. Resistance arises through mutations in two-component systems (TCS) regulating the arn operon. Data on colistin resistance in P. aeruginosa from Pakistan remain limited. A total of 3189 clinical samples (urine, blood, sputum, pus, wound swabs) were cultured. P. aeruginosa was identified by Gram staining, biochemical tests (catalase, oxidase, API 20E), and oprL gene amplification. Antibiotic susceptibility was determine

Newer antibiotics for drug-resistant Gram-negative infections in immunocompromised hosts: from pivotal trials to high-risk practice.

Newer antibiotics have expanded treatment options for drug-resistant Gram-negative infections, but registration evidence is dominated by syndrome-based trials and post-approval evidence by heterogeneous observational cohorts. Translation to immunocompromised patients remains uncertain because profound neutropenia, transplantation, cell-targeted therapy, organ dysfunction, impaired source control, and limited immune-mediated clearance change both the probability and consequences of treatment failure. This critical narrative Review integrates 27 pivotal-trial protocol identifiers included in a p

Multidrug-resistant and virulence traits in genetically distinct vesicular Leptospira inhabiting farmland and sewage in Southern India.

The occurrence, taxonomic identity, drug resistance pattern and virulence traits of cultivable environmental Leptospira inhabiting the Indian subcontinent remain largely unexplored. Here, we describe the isolation and characterization of six novel strains of Leptospira from environmental samples and decode their drug resistance and virulence traits using in silico and in vitro approaches. Among six new isolates of Leptospira, two strains were identified to represent L. interrogans (SA-L3 and SA-L4), originating from sewage that flows proximal to a wildlife enclosure in Mangalore. We also found

Emerging resistance mechanisms to newer β-lactams in Pseudomonas aeruginosa.

BACKGROUND: Although the introduction of novel β-lactams and/or combinations with β-lactamase inhibitors over the last decade is helping to mitigate the threat of extensively drug-resistant/difficult-to-treat-resistant (XDR/DTR) Pseudomonas aeruginosa infections, the problem is far from being solved, due to the capacity of this pathogen for developing resistance. OBJECTIVES: This study aims to provide a comprehensive analysis of the emerging/evolving resistance mechanisms to the antipseudomonal β-lactams introduced over the last decade. SOURCES: Sources include literature review of published
