Retrospective antimicrobial consumption surveillance at health facility level in Dodoma Region, Tanzania.

INTRODUCTION: Antimicrobial resistance (AMR) is a major threat in sub-Saharan Africa (SSA), but assessments of antimicrobial consumption (AMC) are limited. This study aimed to investigate regional AMC and resistance patterns in a representative area of Tanzania and to introduce a method for determining AMC in low-resource settings. DESIGN AND METHODS: We conducted a retrospective study using prescription data collected over 5 years (2013-2017) from multiple hospitals and selected primary health facilities in the Dodoma Region of Tanzania. The study employed the WHO's Anatomical Therapeutic Ch

Scoliosis detection, patient characteristics, referral patterns and treatment in the absence of a screening program in Norway.

BACKGROUND: Early diagnosis of idiopathic scoliosis allows for observation and timely initiation of brace treatment in order to halt progression. School scoliosis screening programs were abolished in Norway in 1994 for lack of evidence that the programs improved outcome and for the costs involved. The consequences of this decision are discussed. OBJECTIVES: To describe the detection, patient characteristics, referral patterns and treatment of idiopathic scoliosis at a scoliosis clinic during the period 2003-2011, when there was no screening and to compare treatment modalities to the period 19

Impact on child acute malnutrition of integrating small-quantity lipid-based nutrient supplements into community-level screening for acute malnutrition: A cluster-randomized controlled trial in Mali.

BACKGROUND: Community-based management of acute malnutrition (CMAM) has been widely adopted to treat childhood acute malnutrition (AM), but its effectiveness in program settings is often limited by implementation constraints, low screening coverage, and poor treatment uptake and adherence. This study addresses the problem of low screening coverage by testing the impact of distributing small-quantity lipid-based nutrient supplements (SQ-LNSs) at monthly screenings held by community health volunteers (CHVs). Screening sessions included behavior change communication (BCC) on nutrition, health, an

Protocol for the evaluation of the population-level impact of Zimbabwe's prevention of mother-to-child HIV transmission program option B+: a community based serial cross-sectional study.

BACKGROUND: WHO recommends that HIV infected women receive antiretroviral therapy (ART) minimally during pregnancy and breastfeeding ("Option B"), or ideally throughout their lives regardless of clinical stage ("Option B+") (Coovadia et al., Lancet 379:221-228, 2012). Although these recommendations were based on clinical trials demonstrating the efficacy of ART during pregnancy and breastfeeding, the population-level effectiveness of Option B+ is unknown, as are retention on ART beyond the immediate post-partum period, and the relative impact and cost-effectiveness of Option B+ compared to Opt

Dependency of sanitation infrastructure on the discharge of faecal coliform and SARS-CoV-2 viral RNA in wastewater from COVID and non-COVID hospitals in Dhaka, Bangladesh.

The detection of severe acute respiratory syndrome coronavirus-2 (SARS-CoV-2) RNA in wastewater can be used as an indicator of the presence of SARS-CoV-2 infection in specific catchment areas. We conducted a hospital-based study to explore wastewater management in healthcare facilities and analyzed SARS-CoV-2 RNA in the hospital wastewater in Dhaka city during the Coronavirus disease (COVID-19) outbreak between September 2020-January 2021. We selected three COVID-hospitals, two non-COVID-hospitals, and one non-COVID-hospital with COVID wards, conducted spot-checks of the sanitation systems (i.

The future of open vascular neurosurgery: perspectives on cavernous malformations, AVMs, and bypasses for complex aneurysms.

Despite the erosion of microsurgical case volume because of advances in endovascular and radiosurgical therapies, indications remain for open resection of pathology and highly technical vascular repairs. Treatment risk, efficacy, and durability make open microsurgery a preferred option for cerebral cavernous malformations, arteriovenous malformations (AVMs), and many aneurysms. In this paper, a 21-year experience with 7348 cases was reviewed to identify trends in microsurgical management. Brainstem cavernous malformations (227 cases), once considered inoperable and managed conservatively, are

Integrated community-based versus facility-based care for people with HIV, diabetes, and hypertension in sub-Saharan Africa (INTE-COMM): an open-label, multicountry, cluster-randomised trial.

BACKGROUND: In sub-Saharan Africa, the burden of diabetes and hypertension is high, alongside a high prevalence of HIV. Whether these conditions can be managed in an integrated way in the community is unknown. We aim to compare integrated community-based care with integrated facility-based care for people with HIV, diabetes, and hypertension in Tanzania and Uganda. METHODS: This open-label, multicountry, cluster-randomised trial was conducted in 14 primary care facilities across Tanzania and Uganda. Adults aged 18 years or older with a diagnosis of HIV, type 2 diabetes, or hypertension (or a

Determinants of Length of Stay After Vaginal Deliveries in the Friuli Venezia Giulia Region (North-Eastern Italy), 2005-2015.

Although length of stay (LoS) after childbirth has been diminishing in several high-income countries in recent decades, the evidence on the impact of early discharge (ED) on healthy mothers and term newborns after vaginal deliveries (VD) is still inconclusive and little is known on the characteristics of those discharged early. We conducted a population-based study in Friuli Venezia Giulia (FVG) during 2005-2015, to investigate the mean LoS and the percentage of LoS longer than our proposed ED benchmarks following VD: 2 days after spontaneous vaginal deliveries (SVD) and 3 days post instrument

Pretreatment Lost to Follow-up Tuberculosis Patients, Lusaka, Zambia, 2024: A Retrospective Cohort Study.

BACKGROUND: Treatment outcomes, particularly lost to follow-up (LTFU) of tuberculosis (TB) patients, are one of the most fundamental indicators related to TB control. However, national TB programs often ignore those who once come to a health facility and were diagnosed with TB but never come back to the facility to commence anti-TB treatment (i.e. pretreatment [PT] LTFU). We conducted a study to determine the proportions of bacteriologically confirmed PT-LTFU patients with TB found at four facilities in Lusaka, Zambia, in 2024. This is the first multifacility 2024 cohort assessing PT-LTFU tren

The preterm child in dentistry. Behavioural aspects and oral health.

BACKGROUND: Advancements in medical care have enabled more children born preterm to survive and develop as healthy individuals alongside their full-term peers. However, a higher frequency of medical health problems, cognitive and behavioural disturbances, including problems in school, has been reported. There is limited knowledge about how this affects preterm children (PT) in dentistry, and few studies have evaluated behavioural problems related to dental treatments and oral health in PT as compared with full-term control children (C). AIMS: The overall objective was to investigate behaviour

Indoor residual spraying of distinct insecticide formulations in a household-scale micro-mosaic format: Feasibility and effectiveness for malaria vector control in rural Zambia.

Zambia recently adopted mosaic application patterns as an insecticide deployment strategy for managing resistance in malaria vector populations. While macro-mosaic applications of indoor residual spraying (IRS), a commonly used supplement to long-lasting insecticidal nets (LLINs), vary the active ingredients used from one village to the next, micro-mosaics vary them between households. Here, a convenient procedure for micro-mosaic deployment of two different IRS formulations at household level was piloted under routine programmatic conditions in south-eastern Zambia, nested within a village-sc

Post-mortem characterisation of pathogen-specific causes of infection-related deaths in African and south Asian neonates: a prospective, observational, multicentre study.

BACKGROUND: Post-mortem biological investigation of causes of deaths is uncommon in low-income and middle-income countries (LMICs). The primary objective of Child Health and Mortality Prevention Surveillance (CHAMPS) is to determine the underlying and contributing causes of death for stillbirths and children younger than 5 years, including identification of specific pathogens and risk factors associated with mortality. We report on the role of infections, including pathogen-specific causes, in the overall causal pathway to death in neonates (aged <28 days) across CHAMPS sites in Ethiopia, Keny

Effect of mass drug administration on malaria incidence in southeast Senegal during 2020-22: a two-arm, open-label, cluster-randomised controlled trial.

BACKGROUND: In Africa, the scale-up of malaria-control interventions has reduced malaria burden, but progress towards elimination has stalled. Mass drug administration (MDA) is promising as a transmission-reducing strategy, but evidence from low-to-moderate transmission settings is needed. We aimed to assess the safety, coverage, and effect of three cycles of MDA with dihydroartemisinin-piperaquine plus single, low-dose primaquine on Plasmodium falciparum incidence and prevalence in southeast Senegal. METHODS: We conducted a two-arm, open-label, cluster-randomised controlled trial in villages
