Transplantation of amputated bony phalanges as a salvage thumb reconstruction method - a preliminary report.

BACKGROUND: Every traumatic amputation of the thumb is an indication for an attempt of replantation. Due to local state of the wound not every case qualifies for this procedure. Such patients may be offered a multi-step salvage reconstruction of the amputated finger; the first step consists of an emergency implantation of the bony phalanx under the skin of the abdominal wall. The osseous graft was covered with skin using a pedicled tubular flap or a "cutaneous pocket". The aim of this paper was to evaluate the effectiveness of this reconstructive method. MATERIAL AND METHODS: Over a period of

Quantitative ultrasound of phalanges of adults with end-stage renal disease or who have undergone renal transplantation.

In patients with end-stage renal disease (ESRD), bone disturbances are common. The aim of this study was to compare the bone mineral status in patients with ESRD, in patients post renal transplantation and in healthy controls. The groups were composed of 218 males and 126 females (ESRD), 43 males and 23 females (renal transplantation) and 614 males and 927 females (healthy controls). Skeletal status was evaluated by quantitative ultrasound measurements of the phalanges using a DBM 1200 (IGEA, Carpi, Italy), which measures the amplitude-dependent speed of sound (Ad-SoS) in m/s. Data analyses we

Ten-year probabilities of clinical vertebral fractures according to phalangeal quantitative ultrasonography.

The objectives of the present study were to estimate 10-year probabilities of clinical vertebral fractures in women, according to age and bone mineral assessment using phalangeal quantitative ultrasound (QUS). Risks were computed from UK derived data on the incidence of a first symptomatic vertebral fracture and mortality rates for each year of age using Poisson models. The 10-year probability of vertebral fracture was determined as the proportion of individuals fracture-free at that site from the age of 45 years. We assumed that the risk of fracture increased with decreasing QUS as assessed b

Long-term results after vascularised joint transfer for finger joint reconstruction.

INTRODUCTION: Vascularised complete joint transfer from the finger or the second toe offers the unique possibility of reconstructing a joint defect on the thumb or fingers using autologous tissue, which fully preserves its growth potential. Indications for vascularised joint transfer on the finger in children are set because of lack of therapy options offering normal growth potential. In adults vascularised joint transfer is indicated in case of contraindication for prosthetic joint replacement or arthrodesis. PATIENTS AND METHODS: In a retrospective clinical study 16 vascularised joint trans

Conservative tissue sparing management of pediatric pyogenic granuloma with preserved joint function: A narrative review with an illustrative case.

The pediatric population presents specific management challenges with pyogenic granuloma (PG), particularly when located near joints, as surgical treatment may compromise mobility. While surgical excision remains the gold standard treatment, emerging reports suggest that more conservative approaches may be suitable in select cases, offering less invasive treatment options. We report the preservation of joint function while successfully resolving pyogenic digital granuloma with topical steroid-antibiotic preparations. A healthy 7-year-old girl presented with progressive left middle and ring fin

Thumb injuries in intercollegiate men's lacrosse.

BACKGROUND: Men's intercollegiate lacrosse is played at a fast pace and with significant force. Glove protection is required. However, the thumb is at risk because of contact with opponents' sticks, the ball, other players, and the ground or artificial surface. PURPOSE: To characterize patterns of hand injuries in men's intercollegiate lacrosse and to compare them with those in similar intercollegiate stick-handling sports that require gloves. STUDY DESIGN: Descriptive epidemiology study. METHODS: The National Collegiate Athletic Association (NCAA) Injury Surveillance System was utilized to

Standardized protocol for artery-only fingertip replantation.

PURPOSE: Artery-only fingertip replantation can be reliable if low-resistance flow through the replant is maintained until venous outflow is restored naturally. Injuring the tip of the replant to promote ongoing bleeding augmented with anticoagulation usually accomplishes this; however, such management results in prolonged hospitalization. In this study, we analyzed the outcomes of artery-only fingertip replantation using a standardized postoperative protocol consisting of dextran-40, heparin, and leech therapy. METHODS: Between 2001 and 2008, we performed 19 artery-only fingertip replants fo

Atypical squamous cell carcinoma of the nail bed with phalangeal involvement.

We report an atypical presentation of squamous cell carcinoma (SCC) of the distal phalanx of the left thumb. On physical examination, there was no clinical evidence of a local lesion, but magnetic resonance imaging (MRI) showed multiple intraosseous cavities filled with fluid. A biopsy specimen showed a well-differentiated SCC, which was treated by amputation of the distal phalanx.

Prospective evaluation of risk of vertebral fractures using quantitative ultrasound measurements and bone mineral density in a population-based sample of postmenopausal women: results of the Basel Osteoporosis Study.

OBJECTIVE: Prospective studies have shown that quantitative ultrasound (QUS) techniques predict the risk of fracture of the proximal femur with similar standardised risk ratios to dual-energy x-ray absorptiometry (DXA). Few studies have investigated these devices for the prediction of vertebral fractures. The Basel Osteoporosis Study (BOS) is a population-based prospective study to assess the performance of QUS devices and DXA in predicting incident vertebral fractures. METHODS: 432 women aged 60-80 years were followed-up for 3 years. Incident vertebral fractures were assessed radiologically.

The Use of External Fixator and Iliac Crest Bone Graft in Traumatic Metacarpal Bone Defect.

INTRODUCTION: Metacarpal bone fractures often lead to significant shortening in the phalanges; it can be caused by variable conditions, ranging from idiopathic, and infective to traumatic causes. There are very rare reports of metacarpals shortening in idiopathic primary hypoparathyroidism cases in the literature. Traumatic metacarpal shortening differs from congenital agenesis [2]. This condition leads to both functional and cosmetic limitations which affect the patient's quality of life. CASE REPORT: A 22-year-old male patient was admitted to our outpatient clinic with a traumatic left-hand

Tips Under the Skin: A Simple Modification of Extension Block Pinning for Mallet Fractures.

Pins exposed out of the skin after surgery for mallet fractures keep patients from washing their hands. The authors buried the tips of all pins under the skin while performing extension block pinning for 14 patients with mallet fractures. The patients were allowed to wash their hands 4 to 5 days postoperatively, without any dressing or splinting. The pins were removed at a mean of 8 weeks postoperatively. Solid union was achieved in all 14 fractures. No pull-out or subsidence of the pin was observed. No patient developed infection or other pin-related complications. Mean extension lag at final
