Females demonstrate lower levels of activity, psychological readiness and strength symmetry after anterior cruciate ligament reconstruction than males, and also recovery of quadriceps strength and hop symmetry is delayed in females undergoing reconstruction with a quadriceps tendon autograft.

PURPOSE: To investigate sex-based recovery differences in patients undergoing anterior cruciate ligament reconstruction (ACLR) with a hamstring (HT) or quadriceps (QT) tendon autograft. METHODS: This study included 97 patients, including 50 females (HT = 25, QT = 25) and 47 males (HT = 24, QT = 23), assessed presurgery and at 12- and 24-month postoperatively via surveys, laxity, isokinetic knee extensor and flexor torque and a 6-hop performance battery. Limb symmetry indices (LSIs) were calculated. Outcomes were compared between males and females, as well as within each graft type. RESULTS:

Retrospective Study on the Risk of Nerve Injury After Distal Biceps Tendon Repair Using Cortical Button.

BACKGROUND: Distal biceps tendon ruptures are relatively rare injuries that typically require surgical intervention to restore flexion and supination strength. Concerns have been raised regarding the risk of nerve injuries, particularly the posterior interosseous nerve (PIN), associated with the use of cortical buttons in distal biceps repair. This study aimed to estimate the incidence of PIN injury as well as injuries to the lateral cutaneous nerve of the forearm and superficial branch of the radial nerve following distal biceps repair using cortical buttons. METHODS: A retrospective review

Press-fit fixation using autologous bone in the tibial canal causes less enlargement of bone tunnel diameter in ACL reconstruction--a CT scan analysis three months postoperatively.

BACKGROUND: Bone tunnel enlargement is a phenomenon present in all anterior cruciate ligament (ACL)- reconstruction techniques. It was hypothesized that press-fit fixation using a free autograft bone plug reduces the overall tunnel size in the tibial tunnel. METHODS: In a prospective cohort study twelve patients who underwent primary ACL reconstruction using an autologous quadriceps tendon graft and adding a free bone block for press-fit fixation (PF) in the tibial tunnel were matched to twelve patients who underwent ACL reconstruction with a hamstring graft and interference screw fixation (I

Cross-Talk Between Human Tenocytes and Bone Marrow Stromal Cells Potentiates Extracellular Matrix Remodeling In Vitro.

Tendon and ligament (T/L) pathologies account for a significant portion of musculoskeletal injuries and disorders. Tissue engineering has emerged as a promising solution in the regeneration of both tissues. Specifically, the use of multipotent human mesenchymal stromal cells (hMSC) has shown great promise to serve as both a suitable cell source for tenogenic regeneration and a source of trophic factors to induce tenogenesis. Using four donor sets, we investigated the bidirectional paracrine tenogenic response between human hamstring tenocytes (hHT) and bone marrow-derived hMSC. Cell metabolic

Short-/intermediate-term outcomes after medial patellofemoral ligament reconstruction in the treatment of chronic lateral patellofemoral instability.

PURPOSE: Disruption of the medial patellofemoral ligament (MPFL) is now considered the essential lesion of recurrent lateral patellar dislocation in patients with normal lower extremity alignment. Reconstruction of the MPFL is a technique gaining significant success in the treatment of patients with this disabling condition. HYPOTHESIS: Reconstruction of the MPFL in patients with chronic patellar instability and normal lower extremity alignment will improve knee function and symptoms, with a high percentage of patients achieving good to excellent results at early follow-up. Knee pain associated with bone-patellar tendon-bone autografts does not limit activity levels, sports participation or quality of life after ACL reconstruction.

PURPOSE: Bone-patellar tendon-bone (BPTB) and Hamstring (HT) autografts are commonly used for anterior cruciate ligament reconstruction (ACLR). Concerns exist regarding postoperative anterior knee pain (AKP) and kneeling discomfort with BPTB grafts. However, many studies solely report the presence/absence of anterior knee pain, without assessing its clinical significance in terms of functional limitation or impact on quality of life. METHODS: This study prospectively analysed 1407 patients undergoing primary ACLR with BPTB or HT autografts. Knee pain prevalence, severity, and location were me

Twelve-month magnetic resonance imaging after anterior cruciate ligament reconstruction can identify risk factors for subsequent graft rupture and used to guide the return to sport. Results from a high-volume institution.

INTRODUCTION: Retear after anterior cruciate ligament reconstruction (ACLR) has been reported between 6% and 31% of cases, resulting in worse outcomes and increased risk of post-traumatic osteoarthritis. This study investigated whether postoperative magnetic resonance imaging (MRI) assessment, clinical outcomes, and return-to-sport test findings can help identify patients at higher risk of early graft retear. METHODS: Retrospective analysis of 430 patients who underwent primary ACLR using hamstring autograft between 2017 and 2022, with a minimum follow-up of 12 months. Baseline characteristic

Incorporating BMP-2 and skeletal muscle to a semitendinosus autograft in an oversized tunnel yields robust bone tunnel ossification in rabbits: Toward single-step revision of failed anterior cruciate ligament reconstruction.

BACKGROUND: Bone tunnel widening after anterior cruciate ligament (ACL) reconstruction is a known complication that can lead to graft failure. Subsequent revision surgery typically involves a two-stage procedure. The aim of this study was to test a novel autologous tendon graft retaining muscle tissue combined with Human Recombinant Bone Morphogenetic Protein-2 (rh-BMP-2) leading to rapid ossification of the muscle tissue, simultaneously replenishing bone stock and producing a mechanically stable bone-tendon insertion. METHODS: In 12 skeletally mature New Zealand rabbits, the ACL was resected

Double femoral-double tibial tunnel configuration improves long-term return to activity compared to anatomic single bundle and double femoral-single tibial tunnel configurations in anterior cruciate ligament reconstruction: a 12-year retrospective cohort study.

INTRODUCTION/OBJECTIVES: Double-bundle (DB) anterior cruciate ligament (ACL) reconstruction has demonstrated biomechanical advantages over anatomic single-bundle (ASB) techniques; however, long-term clinical superiority remains controversial, and the potential influence of tibial tunnel configuration has not been well established. This study aims to compare long-term clinical outcomes of ASB ACL reconstruction with two DB configurations-double femoral-single tibial tunnel (DFST) and double femoral-double tibial tunnel (DFDT)-and to evaluate return to preinjury activity. METHODS: Patients unde

Factors affecting return to sport following hamstrings anterior cruciate ligament reconstruction in non-elite athletes.

BACKGROUND: Anterior cruciate ligament (ACL) rupture is a common injury affecting non-elite and elite athletes. Although most ACL reconstructions restore the mechanical stability of the injured knee, the rate of return to sport (RTS) is varied. HYPOTHESIS/PURPOSE: We aimed to identify why non-elite athletes fail to return to pre-injury sport level. The primary objective was to assess the frequency of RTS (including type of sport, timing, level, and duration), and the secondary objective was to assess the surgical (injury, type of surgery, re-injury) and patient (demographics, function, sympto

Impact of graft selection and return to sport timing on reinjury rates following paediatric anterior cruciate ligament reconstruction: A systematic review.

IMPORTANCE: Anterior cruciate ligament (ACL) injuries in paediatric and adolescent athletes are increasingly common, and ACL reconstruction (ACLR) carries a high risk of reinjury in this population. Graft selection and return to sport (RTS) timing are potentially modifiable determinants of reinjury risk and the rapid accumulation of new evidence continues to challenge established dogma, warranting an updated synthesis of their combined influence in skeletally immature patients. OBJECTIVES: This systematic review will establish the impact of autograft type and RTS timing on reinjury rates incl
