Comparison of Outcomes of Emergency Laparotomies Performed During Daytime Versus Nights and Weekends in Rwandan University Teaching Hospitals.

BACKGROUND: Emergency laparotomy is a common procedure with high morbidity and mortality. The aim of this study was to assess if the time of surgery (day versus night and weekend) affects the morbidity and mortality in a low-resource setting. METHODS: A retrospective study was conducted in 2 university teaching hospitals in Rwanda. Patient characteristics, time of laparotomy, operative details and postoperative outcomes were recorded. Chi-square and Wilcoxon rank sum tests were used to determine factors and outcomes associated with time of surgery. Logistic regression was used to determine fa

Time to scale up molecular surveillance for anti-malarial drug resistance in sub-saharan Africa.

Artemisinin resistance has emerged and spread in the Greater Mekong Sub-region (GMS), followed by artemisinin-based combination therapy failure, due to both artemisinin and partner drug resistance. More worrying, artemisinin resistance has been recently reported and confirmed in Rwanda. Therefore, there is an urgent need to strengthen surveillance systems beyond the GMS to track the emergence or spread of artemisinin and partner drug resistance in other endemic settings. Currently, anti-malarial drug efficacy is monitored primarily through therapeutic efficacy studies (TES). Even though essent

Are evidence-based vasectomy surgical techniques performed in low-resource countries?

Background:  Research evidence published 10 to 15 years ago has shown that the type of vasectomy surgical technique performed can influence the effectiveness and the safety of the procedure.  The objective of this study was to determine if evidence-based vasectomy surgical techniques are integrated in the vasectomy programs of selected low-resource countries. Methods: The surgical techniques recommended to perform the two steps of the vasectomy procedure (isolation/exposition and occlusion of the vas deferens) were extracted from current evidence-based clinical practice guidelines. Gastro-intestinal parasite infections of Ankole cattle in an unhealthy landscape: An assessment of ecological predictors.

The distribution of gastro-intestinal (GI) parasites across landscapes is closely related to the spatial distribution of hosts. In GI parasites with environmental life stages, the vitality of parasites is also affected by ecological and landscape-related components of the environment. This is particularly relevant for domestic livestock species that are often kept across habitats with varying degrees of degradation, exposing them to a wide range of environmentally robust parasite species. In our study, we examined the effect of environmental and anthropogenic factors on the prevalence and inte

Dermatophytes and dermatophytosis in the eastern and southern parts of Africa.

Dermatophytosis is currently a disease of global importance and a public health burden. It is caused by dermatophytes, which attack and grow on dead animal keratin. Dermatophytes belong to three genera, namely, Epidermophyton, Microsporum, and Trichophyton. The predominant clinical forms and causative agents vary from one region of the world to another. Poor socioeconomic status, high population densities, and poor sanitary conditions are some of the factors responsible for the high prevalence of dermatophytosis in many developing countries, which include countries in southern and eastern Afri

Case series of labor induction in twin gestations with an Intrauterine Balloon catheter.

Bunyamwera virus (BUNV) (Bunyamwera orthobunyavirus) has been found in Sub-Saharan Africa and demonstrated recently as cocirculating with Rift Valley Fever Virus (RVFV). Little is known regarding the breadth of transmission modalities of Bunyamwera. Given its co-occurence with RVFV, we hypothesized the transmission system of BUNV shared similarities to the RVFV system including transmission by Ae. aegypti mosquitoes and environmentally mediated transmission through fomites and environmental contamination. We exposed Ae. aegypti mosquitoes to BUNV and evaluated their ability to transmit both ve

Determinants and temporal trajectories of exclusive breastfeeding cessation in 16 sub-Saharan African countries: a pooled multivariable survival analysis of the demographic and health survey (2019-2024).

BACKGROUND: Exclusive breastfeeding (EBF) for the first six months of life is the globally recommended standard of infant nutrition, yet early cessation remains prevalent and highly variable across sub-Saharan Africa (SSA). Despite growing evidence on its determinants, few studies have applied pooled survival analysis with community-level contextual variables to examine both the timing and determinants of EBF cessation across multiple countries simultaneously. This study aimed to determine the time to EBF cessation and its associated factors among infants across 16 SSA countries, and to charac

Risk of selection and timelines for the continued spread of artemisinin and partner drug resistance in Africa.

The introduction of artemisinin combination therapies (ACTs) has significantly reduced the burden of Plasmodium falciparum malaria, yet the emergence of artemisinin partial resistance (ART-R) as well as partner drug resistance threatens these gains. Recent confirmations of prevalent de novo ART-R mutations in Africa, in particular in Rwanda, Uganda and Ethiopia, underscore the urgency of addressing this issue in Africa. Our objective is to characterise this evolving resistance landscape in Africa and understand the speed with which ART-R will continue to spread. We produce estimates of both AR

The geography of waiting: surveillance latency as the hidden driver of Ebola risk in East Africa.

Ebola virus disease remains a persistent public health threat in East Africa, yet preparedness strategies continue to emphasise response after outbreaks are recognised rather than the period preceding detection. This Perspective argues that surveillance latency the cumulative delay between the earliest clinically meaningful signal and organised public health action is a critical but insufficiently measured determinant of Ebola risk. Although diagnostic and reporting delays have been widely documented, they are often treated as isolated operational challenges. We introduce the concept of the ge

Canadian primary care needs community health workers as relational infrastructure.

Canada's primary care system faces persistent challenges related to access, continuity, and coordination. These pressures are commonly framed in terms of health workforce shortages, long waits, and growing demand associated with population aging and immigration. But capacity constraints do not fully account for the problem. Many access failures also arise before and between clinical encounters: needs go unrecognized, entry points are unclear or culturally misaligned, referrals break down across handoffs, and follow-up is not sustained. These breakdowns disproportionately affect people facing s

Functional outcomes and barriers of assistive devices for children with cerebral palsy: a cross-sectional study in Rwandan rehabilitation contexts.

BACKGROUND: Cerebral palsy (CP) is a leading cause of physical disability worldwide, affecting approximately 2 -3 per 1,000 live births. In sub-Saharan Africa, access to assistive devices supporting participation in occupation remains constrained by inadequate rehabilitation services and limited evidence. This study examined the utilisation, perceived benefits and challenges associated with assistive devices use among children with CP attending rehabilitation centres in Rwanda. METHODS: We conducted a cross-sectional study at Gatagara Orthopaedic and Rehabilitation Hospital and Gahini Rehabil

Wearable vital-sign sensors for early detection of severe dengue and sepsis: Clinical applications and implementation in resource-limited settings.

INTRODUCTION: Severe dengue and sepsis account for tens of millions of cases and deaths annually in low-resource settings, where spot-check vital sign monitoring misses early physiological deterioration. Affordable wearable sensors that stream continuous data and feed machine learning algorithms promise earlier detection and improved outcomes. METHODS: We conducted a structured narrative literature review of human studies on PubMed, Embase, Scopus, and Google Scholar that evaluated wearable vital-sign devices for predicting severe dengue or sepsis in resource-limited areas. Metabolic function and the prevalence of lipodystrophy in a population of HIV-infected African subjects receiving highly active antiretroviral therapy.

OBJECTIVE: This study measured the prevalence of lipodystrophy and the metabolic effects of highly active antiretroviral therapy (HAART) in HIV-infected African subjects. METHODS: Prevalence was measured in 571 Rwandans receiving HAART for > or = 6 months. Metabolic variables were measured in 100 HIV-positive adults with lipodystrophy, 50 HIV-positive nonlipodystrophic adults, and 50 HIV-negative controls. RESULTS: A HAART regimen of stavudine, lamivudine, and nevirapine was used by 81.6% of subjects; none received protease inhibitors. Lipodystrophy was observed in 34% (48.5% in urban groups

Hepatic safety and tolerability of cipargamin (KAE609), in adult patients with Plasmodium falciparum malaria: a randomized, phase II, controlled, dose-escalation trial in sub-Saharan Africa.

BACKGROUND: The novel anti-malarial cipargamin (KAE609) has potent, rapid activity against Plasmodium falciparum. Transient asymptomatic liver function test elevations were previously observed in cipargamin-treated subjects in two trials: one in malaria patients in Asia and one in volunteers with experimentally induced malaria. In this study, the hepatic safety of cipargamin given as single doses of 10 to 150 mg and 10 to 50 mg once daily for 3 days was assessed. Efficacy results, frequency of treatment-emerging mutations in the atp4 gene and pharmacokinetics have been published elsewhere. Fur
