Neurophysiologic intraoperative monitoring in children with Down syndrome.

OBJECTIVE: Neurophysiological monitoring during complex spine procedures may reduce risk of injury by providing feedback to the operating surgeon. This tool is a well-established and important surgical adjunct in adults, but clinical data in children are not well described. Moreover, to the best of our knowledge, neurophysiologic intraoperative monitoring data have not been reported in children with neurodevelopmental disorders, such as Down syndrome, who commonly present with craniocervical instability requiring internal fixation. The purpose of this study is to determine the reliability and

[Pharmacotherapy of cancer pain : 2. Use of opioids.].

The adequate use of opioids in the treatment of chronic cancer pain requires sound knowledge of selection criteria for the various opioids, the routes of administration, dosages, dosing schemes and possible side effects. Drug selection depends on the intensity of pain rather than on the specific pathophysiology. Mild to moderate pain can often be treated effectively by so-called "weak" opioids. These include codeine, dihydrocodeine and dextropropoxyphene. Non-opioid analgesics, like acetylsalicylic acid or paracetamol can be added according to the "analgesic ladder" proposed by the World Healt

Cerebral subarachnoid blood migration consecutive to a lumbar haematoma after spinal anaesthesia.

We report the case of a woman who received spinal anaesthesia for peripheral vascular surgery of the lower limbs and subsequently developed a spinal subarachnoid haematoma. Interestingly, low back pain was the only symptom of this spinal subarachnoid haemorrhage. During the following days, blood migrated from the spinal haematoma towards the cerebral subarachnoid spaces. The patient presented with stupor, nausea and vomiting that resolved within 2 weeks with conservative treatment.

[Combined subarachnoid-epidural technique for obstetric analgesia].

Combined spinal-epidural blockade for labor pain has enjoyed increasing popularity in obstetric anesthesia. The usual procedure is to use a single space and a single needle for dural puncture, inserting a spinal needle through an epidural needle followed by insertion of a catheter. A small dose of one or several substances (usually a lipophilic opioid and a local anesthetic) is first injected in the intrathecal space to provide rapid, effective analgesia with minimal muscle blockade. The epidural catheter is used if labor lasts longer than the spinal block, if the spinal block is insufficient,

Refining the anesthesia management of the face-lift patient: lessons learned from 1089 consecutive face lifts.

BACKGROUND: The importance of anesthetic technique is often underappreciated in face-lift procedures and is sparsely written about in the literature. Appropriate control of blood pressure, anxiety, pain, and nausea is essential for reducing the complications of face lift, primarily, hematoma risk. This study discusses the standard anesthetic protocol provided at the authors' institution and describes the preoperative, intraoperative, and postoperative management of face-lift patients resulting in low hematoma and complication rates. METHODS: One thousand eighty-nine patients who underwent fac

Physiological pattern differences in non-intubated vs. intubated thoracic anesthesia based on time series analysis.

BACKGROUND: It is still weak with evidence of the targeted anesthesia management for non-intubated thoracic anesthesia, mainly based on the professional experience and information on the anesthesia sheet. Our study elucidates the differences in dynamic physiological parameters, including arterial oxygen saturation (SpO2), end-tidal carbon dioxide (ETCO2), bispectral index (BIS), mean arterial pressure (MAP), and heart rate (HR), as revealed by the intraoperative real-time database during one-lung ventilation with reduced anesthetics in non-intubated compared with intubated thoracic anesthesia.

Impact of Posterior Quadratus Lumborum Block on Acute Pain Relief and Chronic Pain Prevention in Breast Cancer Surgery.

INTRODUCTION: Breast cancer surgery is a common surgical procedure often associated with acute and chronic postoperative pain. As part of multimodal analgesia, the erector spinae plane block (ESPB) has been shown to effectively alleviate pain after breast cancer surgery. This study is the first to apply the posterior quadratus lumborum block (posterior QLB) for perioperative analgesia in breast cancer surgery. The aim of this research was to evaluate whether ESPB and QLB2 can relieve acute and chronic pain following breast cancer surgery. METHODS: A total of 120 female patients undergoing bre

Cholinergic Switch between Two Types of Slow Waves in Cerebral Cortex.

Sleep slow waves are known to participate in memory consolidation, yet slow waves occurring under anesthesia present no positive effects on memory. Here, we shed light onto this paradox, based on a combination of extracellular recordings in vivo, in vitro, and computational models. We find two types of slow waves, based on analyzing the temporal patterns of successive slow-wave events. The first type is consistently observed in natural slow-wave sleep, while the second is shown to be ubiquitous under anesthesia. Network models of spiking neurons predict that the two slow wave types emerge due

Evaluating sedation efficacy and perioperative safety of fospropofol disodium versus propofol in elderly patients undergoing elective TURP: study protocol for a randomized controlled trial.

BACKGROUND: Fospropofol disodium is the first novel water-soluble propofol prodrug in China. After administration, it is mainly converted to propofol by alkaline phosphatase on the surface of endothelial cells to exert its pharmacological effects. Compared with propofol, fospropofol disodium has the characteristics of longer duration of action, lower incidence of injection pain and adverse events related to the respiratory and circulatory systems, and no adverse reactions related to lipid metabolism. At present, there are few reports on the safety and efficacy of fospropofol disodium in the in

Association between continuous phenylephrine infusion and hemodynamic stability in patients undergoing gastrointestinal endoscopy under remimazolam sedation: a retrospective cohort study.

BACKGROUND: Hypotension is an important complication in the procedural sedation for the gastrointestinal endoscopy despite the use of remimazolam, which has superior hemodynamic stability compared to propofol. Prophylactic vasopressor therapy may preserve cerebral perfusion and enhance the outcome of recovery. This study evaluated the association between continuous infusion of phenylephrine and hemodynamic stability and recovery of cognition in patients undergoing endoscopy under remimazolam sedation. METHODS: This retrospective cohort study analyzed clinical records of 108 adult patients (Am

[Stroke volume and pulse pressure variation are good predictors of fluid responsiveness in sepsis patients].

UNLABELLED: Stroke volume variation (SVV) and pulse pressure variation (PPV) are dynamic preload indicators. Specific interactions of the cardiovascular system and lungs under mechanical ventilation cause cyclic variations of SVV and PPV. Real time measurement of SVV and PPV by arterial pulse contour analysis is useful to predict volume responsiveness in septic patients. Results of a prospective, 2-year observational study conducted at Department of Anesthesiology, Resuscitation and Intensive Care, Zagreb University Hospital Center, are presented. Volume responders and non-responders were defi
