Therapeutic intraspinal microstimulation improves forelimb function after cervical contusion injury.

OBJECTIVE: Intraspinal microstimulation (ISMS) is a promising method for activating the spinal cord distal to an injury. The objectives of this study were to examine the ability of chronically implanted stimulating wires within the cervical spinal cord to (1) directly produce forelimb movements, and (2) assess whether ISMS stimulation could improve subsequent volitional control of paretic extremities following injury. APPROACH: We developed a technique for implanting intraspinal stimulating electrodes within the cervical spinal cord segments C6-T1 of Long-Evans rats. Beginning 4 weeks after a

The genetic basis of addictive disorders.

Addictions are common, chronic, and relapsing diseases that develop through a multistep process. The impact of addictions on morbidity and mortality is high worldwide. Twin studies have shown that the heritability of addictions ranges from 0.39 (hallucinogens) to 0.72 (cocaine). Twin studies indicate that genes influence each stage from initiation to addiction, although the genetic determinants may differ. Addictions are by definition the result of gene × environment interaction. These disorders, which are in part volitional, in part inborn, and in part determined by environmental experience,

Physiological responses and perceived exertion during cycling with superimposed electromyostimulation.

The goal of the study was to evaluate and to quantify the effects of local electromyostimulation (EMS) during cycling on the cardiorespiratory system, muscle metabolism, and perceived exertion compared with cycling with no EMS. Ten healthy men (age: 24.6 ± 3.2 years, V[Combining Dot Above]O2max: 54.1 ± 6.0 ml·min·kg) performed 3 incremental cycle ergometer step tests, 1 without and 2 with EMS (30 and 85 Hz) until volitional exhaustion. Lactate values and respiratory exchange ratio were significantly higher at intensities ≥75% peak power output (PPO) when EMS was applied. Bicarbonate concentrat

Cardiac functional reserve is diminished in growth hormone-deficient adults.

Various studies have shown that patients with severe growth hormone deficiency (GHD) have diverse changes in left ventricular (LV) size or performance but so far there is no direct indication of cardiac reserve ability to maintain the circulation during peak exercise. We tested the hypothesis that patients with severe GHD have reduced cardiac reserve function compared with healthy controls. Eighteen patients with severe GHD were studied and compared with 18 age-, sex-, and body mass index (BMI)-matched healthy controls. Peak cardiac power and cardiorespiratory fitness were investigated using n

Effect of a splint on measures of sustained grip exertion under different forearm and wrist postures.

Despite the facts that gripping tasks have been found to be highly correlated with CTS and that splints are gaining popularity as personal protective equipment, the influence of splints on grip performance has not been determined adequately. The present study intends to investigate the influence of splints without the volar parts as well as of forearm and wrist postures on grip performances including maximal volitional contraction (MVC), maximum acceptable sustained time (MAST), cumulated exertion output (CEO), and normalized exertion level (NEL). Twenty college-student volunteers, 10 males an

Automatic postural response systems in individuals with congenital total blindness.

This study examined the effects of the absence of vision from birth on automatic postural responses to platform displacements during stance. Postural responses were induced by producing randomly four types of perturbations which consisted of forward and backward translations, and toe up and down rotations. Nine congenitally totally blind and nine sighted adults served as subjects. EMG signals were recorded from four muscles in the right leg, and reaction time to somatosensory stimuli generated by platform displacements was measured by pushing a hand-held button. To assess the ability to contro

A validation of techniques using surface EMG signals from dynamic contractions to quantify muscle fatigue during repetitive tasks.

The purpose of the current study was to determine the validity of quantifying biceps brachii fatigue with dynamic measures of surface electromyo-graphic (EMG) mean power frequency (MPF) through comparisons with the well-established isometric methodology. Subjects performed repetitive elbow flexion-extension movements with a hand held load of 7 kg until volitional exhaustion. Elbow joint angle and biceps brachii EMG signals were recorded continuously during the fatiguing movement (in 250-ms segments) and during isometric, isotonic contractions (in 1000-ms segments) performed at a 90 degrees fle

Effects of topical skin protectant on heat exchange in humans.

BACKGROUND: The application of a Topical Skin Protectant (TSP) under chemical protective clothing may impair heat exchange and/or decrease tolerance time during exercise. HYPOTHESIS: The extent to which TSP might act as a barrier to heat transfer was unknown. Since TSP may be permeable to water vapor, we hypothesized that there would be no significant differences between treatments on variables effecting heat exchange. METHODS: There were 10 subjects who walked (3.5 mph, 3% grade) until volitional exhaustion in an environmental chamber (TA = 36.0 +/- 0.5 degrees C; TDP = 27.0 +/- 1.0 degrees

Developing physical activity interventions for adults with spinal cord injury. Part 1: a comparison of social cognitions across actors, intenders, and nonintenders.

OBJECTIVE: This article is the first in a three-part series focused on designing theory-based interventions to increase leisure time physical activity (LTPA) in persons with spinal cord injury (SCI). The purpose of this first study was to compare social cognitions for LTPA between people classified as LTPA actors, intenders, and nonintenders, as per Schwarzer's Health Action Process Approach (HAPA) model. METHOD: Participants were 238 men and women living with a SCI (M age = 44.14, SD = 12.74; 44.5% paraplegic) who were subsequently classified as LTPA actors (n = 105), intenders (n = 73), or

Detecting consciousness in a total locked-in syndrome: an active event-related paradigm.

Total locked-in syndrome is characterized by tetraplegia, anarthria and paralysis of eye motility. In this study, consciousness was detected in a 21-year-old woman who presented a total locked-in syndrome after a basilar artery thrombosis (49 days post-injury) using an active event-related paradigm. The patient was presented sequences of names containing the patient's own name and other names. The patient was instructed to count her own name or to count another target name. Similar to 4 age- and gender-matched healthy controls, the P3 response recorded for the voluntarily counted own name was

Neural mechanisms underlying immediate and final action goals in object use reflected by slow wave brain potentials.

Event-related brain potentials were used to study the neural mechanisms underlying goal-directed object use distinguishing between processes supporting immediate and final action goals during action planning and execution. Subjects performed a grasping and transportation task in which actions were cued either with the immediate action goal (the part of the object to grasp) or with the final action goal of the movement (the end position for transportation). Slow wave potentials dissociated between processes supporting immediate and final goals: reaching for the object was accompanied by the dev

Discordant mental and physical efforts in an autistic patient.

We investigated whether there was mental effort in response to verbal commands in a 16-year old girl with autism, a high degree of non-compliance with commands and symptoms of autonomic dysfunction by monitoring the brainstem autonomic tone during an attempt to perform isometric exercise. An index of cardiac vagal tone (CVT), cardiac sensitivity to baroreflex (CSB), heart rate (HR) and mean arterial blood pressure (MAP) were measured simultaneously. Physical non-compliance with our commands meant there was no force applied by the patient during the attempted exercise, but CVT and CSB were both

A wheelchair modified for leg propulsion using voluntary activity or electrical stimulation.

A commercially available wheelchair has been modified for propulsion by movements of the lower legs. The feet are attached securely to a foot rest that can rotate around the knee joint. Movement is generated either with residual voluntary activation of the quadriceps (knee extensor) and hamstring (knee flexor) muscles, or with electrical stimulation of these muscles, if voluntary control is absent. Either a chain or a lever can couple the movements through a gearbox to the wheel to propel the wheelchair forward. Control of a wheelchair with the legs is more efficient than using the arms and ha

Feasibility of a Modified Exertional Bike Test to Threshold Supervised Virtual Aerobic Exercise in Community-Dwelling Individuals with Mild Traumatic Brain Injury.

Mild traumatic brain injury (mTBI) often creates a sequela of symptoms that complicate safely participating in aerobic exercise. Although exertional assessments have been used to evaluate exercise tolerance, data on the feasibility of using test-derived metrics to guide the intensity of aerobic exercise interventions in community-dwelling adults is yet to be presented. This secondary analysis of a wider pilot randomized control trial examined whether a modified exertional bike test (MEBT) could be used to effectively individualize supervised virtual aerobic exercise without exacerbating sympto

Pre-exercise chickpea milk consumption supports glycemic stability during endurance exercise in female runners.

PURPOSE: This study investigated the effects of pre-exercise chickpea milk consumption on glycemic stability and physiological responses during endurance exercise in trained female runners. METHODS: Fourteen eumenorrheic female half-marathon runners (25-35 years) completed a randomized, double-blind, counterbalanced crossover trial. Participants consumed 250 mL of chickpea milk or soy milk or a maltodextrin placebo 30 min before exercise, with each trial conducted during the follicular phase and separated by a washout period of at least one week. The exercise protocol consisted of treadmill r

The Interplay Between Neuromodulation and Stem Cell Therapy for Sensory-Motor Neuroplasticity After Spinal Cord Injury: A Perspective View.

Spinal Cord Injury (SCI) rehabilitation is undergoing a transformative shift with the emergence of new treatment strategies. Historically, treatment options were limited, and few offered meaningful recovery. Recent work in human models has shown that neuromodulation specifically with spinal cord epidural stimulation (SCES) paired with task-specific training (TsT) can partially restore motor function such as the ability to stand, step, and perform volitional movements. Despite these advances, the recovery has been shown to plateau even with the combination of therapies. The recovery process typ

Motor synergy organization during fatiguing single-leg heel-rise in young ballet dancers and non-dancers.

BACKGROUND: The uncontrolled manifold (UCM) framework decomposes inter-trial kinematic variability into task-stabilizing (VUCM) and task-destabilizing (VORT) components to quantify motor synergy organization. Whether long-term ballet training influences motor synergy structure during fatiguing lower-limb tasks remains unclear. RESEARCH QUESTION: To what extent is long-term ballet practice related to the motor synergy organization that stabilizes the center of mass (COM) during a repetitive, fatiguing single-leg heel-rise task, compared with non-dancers? METHODS: Fourteen female ballet dancer

Acute effects of dietary whey protein supplementation after endurance exercise on serum osteokine and inflammatory cytokine concentrations in endurance runners.

Endurance athletes have an increased risk of bone stress injuries due to high training and energy demands. Endurance training with inadequate recovery triggers a pro-inflammatory response, which can upregulate bone resorption for hours to days. Combined dietary protein and carbohydrate supplementation post-exercise has shown to attenuate bone resorption and pro-inflammatory cytokine responses, though the effect of dietary protein intake alone remains to be elucidated. This randomized cross-over, double-blinded study examined the acute effects of post-exercise protein supplementation on serum c
