Outcomes in pacemaker-dependent patients upgraded from conventional pacemakers to cardiac resynchronization therapy-defibrillators.

BACKGROUND: Pacemaker-dependent patients with left ventricular dysfunction benefit from upgrade to cardiac resynchronization therapy (CRT). Those at low risk for ventricular tachyarrhythmias may benefit similarly from upgrade to a CRT-defibrillator or CRT-pacemaker. OBJECTIVE: To determine whether coronary artery disease (CAD), because of associated scar that supports reentry, predicts higher risk of appropriate shocks in pacemaker-dependent patients upgraded to a CRT-defibrillator. METHODS: We grouped 157 pacemaker-dependent patients with left ventricular ejection fraction (LVEF) ≤35%, no p

Thoracoscopic right atrial access for septal lead implantation: a novel minimally invasive technique (case report).

BACKGROUND: Transvenous lead implantation remains the standard of cardiac pacing; however, alternative strategies are required in patients with limited or absent venous access. Video-assisted thoracoscopic surgery (VATS) has previously been used for epicardial left ventricular lead placement when transvenous approaches fail. Direct thoracoscopic access to the right atrium for endocardial septal pacing has not been previously described. We report a novel, minimally invasive technique allowing right atrial access and implantation of a septal pacing lead targeting the left bundle branch (LBB) are

Real-world performance of intrinsic antitachycardia pacing: Primary results from a global prospective postapproval registry study.

BACKGROUND: Antitachycardia pacing (ATP) can prevent defibrillator shocks and associated adverse outcomes. Opportunities to enhance ATP performance exist, particularly for rapid monomorphic ventricular tachycardia (MVT). Intrinsic ATP (iATP) is the first closed-loop algorithm that designs and auto-adjusts ATP therapy in real time. OBJECTIVE: This study aimed to evaluate iATP performance in a large, real-world cohort. METHODS: This global, prospective, observational study enrolled patients with an iATP-enabled implantable cardioverter-defibrillator or cardiac resynchronization therapy-defibri

Complications of left bundle branch area pacing compared with biventricular pacing in candidates for resynchronization therapy: Results of a propensity score-matched analysis from a multicenter registry.

BACKGROUND: Cardiac resynchronization therapy (CRT) with biventricular pacing (BVP) is a well-established therapy in patients with reduced left ventricular ejection fraction, heart failure, and left bundle branch block. Left bundle branch area pacing (LBBAP) has recently been shown to be a feasible and effective alternative to BVP. Comparative data on the risk of complications between LBBAP and BVP among patients undergoing CRT are lacking. OBJECTIVE: The aim of this study was to compare the long-term risk of procedure-related complications between LBBAP and BVP in a cohort of patients underg

Upgrade of right ventricular pacing to cardiac resynchronization therapy in heart failure: a randomized trial.

BACKGROUND AND AIMS: De novo implanted cardiac resynchronization therapy with defibrillator (CRT-D) reduces the risk of morbidity and mortality in patients with left bundle branch block, heart failure and reduced ejection fraction (HFrEF). However, among HFrEF patients with right ventricular pacing (RVP), the efficacy of CRT-D upgrade is uncertain. METHODS: In this multicentre, randomized, controlled trial, 360 symptomatic (New York Heart Association Classes II-IVa) HFrEF patients with a pacemaker or implantable cardioverter defibrillator (ICD), high RVP burden ≥ 20%, and a wide paced QRS com

Characterization of super-response to cardiac resynchronization therapy.

BACKGROUND: In patients with chronic systolic heart failure who undergo cardiac resynchronization therapy (CRT), improvements in left ventricular ejection fraction (LVEF) and reductions in left ventricular volume are generally modest. A minority of patients experience a dramatic response to CRT (super-responders), but the attributes associated with these patients have not been fully characterized. OBJECTIVE: The purpose of this study was to identify baseline clinical attributes of super-responders and to assess the survival benefit associated with this response. METHODS: We reviewed clinical

Size Matters: Normalization of QRS Duration to Left Ventricular Dimension Improves Prediction of Long-Term Cardiac Resynchronization Therapy Outcome.

BACKGROUND: In patients with left bundle branch block (LBBB), QRS duration (QRSd) depends on left ventricular (LV) dimension. Previously, we demonstrated that normalizing QRSd to LV dimension, to adjust for variations in LV size, improved prediction of hemodynamic response to cardiac resynchronization therapy (CRT). In addition, sex-specific differences in CRT outcome have been attributed to normalized QRSd. The present study evaluates the effect of normalization of QRSd to LV dimension on prediction of survival after CRT implantation. METHODS: In this 2-center study, we studied 250 heart fai

Ertugliflozin to Reduce Arrhythmic Burden in Patients with ICDs/CRT-Ds.

BACKGROUND: Sodium-glucose cotransporter 2 inhibitors (SGLT2is) have beneficial pleiotropic effects, contributing to improved cardiovascular and renal outcomes for patients with and without diabetes. The impact of SGLT2is on arrhythmic burden remains largely unexplored through randomized trials. METHODS: In this multicenter, double-blind, randomized, placebo-controlled trial, we investigated the effects of ertugliflozin on arrhythmic burden among patients with heart failure with an ejection fraction less than 50%. All patients had an implantable cardioverter-defibrillator (ICD) with or withou

Bleeding complications during cardiac electronic device implantation in patients receiving antithrombotic therapy: is there any value of local tranexamic acid?

BACKGROUND: The perioperative use of antithrombotic therapy is associated with increased bleeding risk after cardiac implantable electronic device (CIED) implantation. Topical application of tranexamic acid (TXA) is effective in reducing bleeding complications after various surgical operations. However, there is no information regarding local TXA application during CIED procedures. The purpose of our study was to evaluate bleeding complications rates during CIED implantation with and without topical TXA use in patients receiving antithrombotic treatment. METHODS: We conducted a retrospective

Paradoxical embolism from a migrated right atrial 'lead ghost' through an unrecognized patent foramen ovale after cardiac resynchronization therapy-defibrillator lead extraction: a case report.

BACKGROUND: A 'lead ghost' is a fibrotic or thrombotic cast that remains after transvenous lead extraction (TLE) and typically resides within the right heart. This case describes a paradoxical embolism through an unrecognized patent foramen ovale (PFO), most likely due to a transient right-to-left shunt that allowed the 'lead ghost' to enter the systemic circulation. CASE SUMMARY: A 55-year-old man with ischaemic cardiomyopathy (ICM) underwent cardiac resynchronization therapy-defibrillator lead extraction for right-ventricular (RV) lead fracture. Clinical phenotypes and outcomes associated with improved left ventricular ejection fraction after biventricular pacing.

BACKGROUND: In some patients who receive a cardiac resynchronization therapy (CRT) device, the left ventricular ejection fraction (LVEF) does not improve. METHODS: We analyzed patients enrolled in the REVERSE, MADIT-CRT, and BLOCK-HF trials, restricting the analysis to those who received CRT. Characteristics of patients with or without improved LVEF were compared using two sample t-tests and Pearson's chi-square tests. Kaplan-Meier survival curves were constructed to display time-to-event data. A log-rank test was used to compare event rates for patients with or without improved LVEF. Mixed e
