Review of blast noise and the auditory system.

The auditory system is particularly vulnerable to blast injury due to the ear's role as a highly sensitive pressure transducer. Over the past several decades, studies have used a variety of animal models and experimental procedures to recreate blast-induced acoustic trauma. Given the developing nature of this field and our incomplete understanding of molecular mechanisms underlying blast-related auditory disturbances, an updated discussion about these studies is warranted. Here, we comprehensively review well-established blast-related auditory pathology including tympanic membrane perforation

Post-traumatic peripheral vestibular disorders (excluding positional vertigo) in workers following head injury.

Benign paroxysmal positional vertigo has typically been reported to be the most common cause of post-traumatic dizziness. There is however paucity in the literature about other peripheral vestibular disorders post-head injury. This article provides an overview of other causes of non-positional dizziness post-head trauma from our large institutional experience. The UHN WSIB Neurotology database (n = 4291) between 1998 and 2018 was retrospectively studied for those head-injured workers presenting with non-positional peripheral vestibular disorders. All subjects had a detailed neurotological hist

Inner-Ear Disorders Presenting with Air-Bone Gaps: A Review.

Air-bone gaps (ABGs) are commonly found in patients with conductive or mixed hearing loss generally due to outer- and/or middle-ear diseases such as otitis externa, tympanic membrane perforation, interruption or fixation of the ossicular chain, and chronic suppurative otitis media. ABGs can also be found in correlation with inner-ear disorders, such as endolymphatic hydrops, enlarged vestibular aqueduct syndrome, semicircular canal dehiscence, gusher syndrome, cochlear dehiscence, and Paget disease's as well cerebral vascular anomalies including dural arteriovenous fistula. The typical clinica

Dizziness and vestibular function before and after cochlear implantation.

Vestibular problems after cochlear implantation (CI) were explored by categorizing them according to clinical course and changes in objective vestibular function. The changes in vestibular function of 62 patients (66 ears) were analyzed and vestibular symptoms were divided into three categories by their time course and nature. Etiologies were determined by analyzing the symptoms in combination with changes in objective vestibular function, measured using the caloric and/or video head impulse test. Before surgery, vestibular function was normal in 31 cases (47.0 %), unilaterally hypofunctional

Endolymphatic space size in patients with Meniere's disease and healthy controls.

CONCLUSIONS: The incident ratios of cochlear and/or vestibular endolymphatic hydrops (EH) were significantly higher in the affected ears of patients with Meniere's disease (MD) than in the ears of healthy controls. There were no significant differences between controls and the contralateral ears of MDs. OBJECTIVES: The aim was to compare the incidence ratios of EH between unilateral/bilateral MD and controls using 3T magnetic resonance imaging (MRI) with intravenous gadolinium. METHODS: A total of 41 patients were diagnosed with MD: 32 with unilateral MD (uMD) and nine with bilateral MD (bMD

Time course changes of vasopressin-induced enlargement of the rat intrastrial space and the effects of a vasopressin type 2 antagonist.

CONCLUSION: The intrastrial space was enlarged remarkably at 20 min after vasopressin (VP) injection, and this enlargement of the intrastrial space was reduced by administration of OPC-31260 before VP injection. These results suggest that VP increases the influx of water from the perlymph to the basal cells via aquaporin (AQP) 2 and causes the formation of endolymphatic hydrops. OBJECTIVES: To investigate a time course of changes of the stria vascularis after VP injection and the influence of OPC-31260 on experimentally induced enlargement of the intrastrial space caused by VP injection. Vestibular nerve section: long-term follow-up.

Vestibular nerve section is considered an effective modality in the treatment of refractory and incapacitating vertigo. Typically nerve section results are described on the basis of short-term follow-up. We have reviewed 41 cases of vestibular nerve section spanning an 18-year period. Although the majority of cases involved classic Meniere's disease, delayed endolymphatic hydrops, vestibular neuritis, and Meniere's syndrome secondary to head trauma were also included. Surgical approaches included translabyrinthine (20 cases), retrolabyrinthine (14 cases), retrosigmoid (six cases), and middle f

Otitis media with perforation of the tympanic membrane: a longitudinal experimental study.

Despite the high incidence and prevalence of otitis media, its pathogenesis is not thoroughly understood. In an effort to provide a better understanding of this disease, experimental animal models have been developed which corroborate the changes observed in humans. In this study a new factor was added: tympanic membrane perforation 1 week after Eustachian tube obstruction. Twelve cats were divided in 4 even groups, and sacrificed at 1 and 2 weeks, 1 and 3 months after perforation, and their temporal bones were studied. Findings revealed an early massive reaction of the mucoperiosteum with gra

A model of viscoelastoplasticity in the cochleo-saccular membranes.

INTRODUCTION: Variations in the distensile behavior of the cochleo-saccular vestibular membranes may contribute to lesion evolution of endolymphatic hydrops in Meniere's disease. Such variation may be mediated through membrane viscoelastoplasticity. This feature may provide insight into the distensile process at work in these membranes. HYPOTHESIS: A precipitated collagen matrix can provide a suitable in vitro model of viscoelastoplasticity in cochleo-saccular vestibular membranes. METHODS: An in vitro extra-cellular matrix of precipitated collagen was evaluated as a model of suspected visco

Some Remarks on Imaging of the Inner Ear: Options and Limitations.

The temporal bone has a highly complex anatomical structure, in which the sensory organs of the cochlea and the vestibular system are contained within a small space together with the sound-conducting system of the middle ear. Detailed imaging is thus required in this anatomical area. There are a great many clinical aims for which the highest-possible spatial resolution is required. These include the localization of cerebrospinal fluid fistulas, the detection of malformations of the middle and inner ear and the vestibulocochlear nerve, an aberrant course of the facial nerve and anomalies of the

The furosemide test and vestibular status in Meniere's disease.

A total of 62 ears of patients with typical Meniere's disease was examined by the furosemide test to detect endolymphatic hydrops. In 95% of the normal control group, the per cent change in the maximum velocity of the slow phase of caloric nystagmus (MVS) after injection of furosemide was under 10%. Therefore, a positive furosemide test was defined as a change in MVS of more than 10%. Thirty-five (56%) of the 62 ears with typical Meniere's disease showed a positive furosemide test. When the affected ears were divided into two groups according to vestibular symptoms, only 11 (38%) of 29 inactiv

Obstructive sleep apnea syndrome as a novel cause for Ménière's disease.

PURPOSE OF REVIEW: Several recent reports have described the relation between sleep disorders and inner ear function. There are also many reports that insomnia is observed in Ménière's patients. However, the possibility that obstructive sleep apnea syndrome (OSAS) might affect Ménière's disease or other neurotological consequences was not noticed, until studies using polysomnography for these patients. RECENT FINDINGS: OSAS may cause not only vestibular but also auditory dysfunction. Several reports suggest that insufficient supply of blood via the vertebral basilar artery, which supplies the

Relationship of vestibular aqueduct and inner ear pressure in Ménière's disease and the normal population.

OBJECTIVE: Etiopathogenesis of Ménière's disease has not been resolved. The principal histopathologic finding in this disease is endolymphatic hydrops. The majority of radiologic and histopathologic studies demonstrated a narrow vestibular aqueduct in Ménière's disease. There is no study in the literature investigating the relationship between inner ear pressure and vestibular aqueduct dimensions. Static acoustic compliance is a noninvasive procedure that is thought to measure perilymphatic pressure at the footplate. An increase in mechanical fluid pressure in the inner ear is transmitted to t

Investigations into the cause of vertigo in sudden sensorineural hearing loss.

In this light microscopic study of the temporal bone, an attempt has been made to find a morphologic correlate of vertigo associated with idiopathic sudden sensori-neural hearing loss (ISSHL). Hair cell densities of the three cristae and both maculae, as well as vestibular ganglion cell (neuronal) count estimation, was done in nine ears that had documented histories of ISSHL. There were five ears with vertigo and four without. These quantitative data--i.e., hair cell densities and neuronal counts, of the vertiginous ears (group I) and nonvertiginous ears (group II)--was compared by histograms

Exposure to high-altitude hypobaric hypoxic environment induces low-frequency hearing loss in C57BL/6J mice: Mediated by slowing down the postsynaptic electrical signal transmission speed in the cochlear-inferior colliculus auditory signaling pathway.

OBJECTIVE: To reveal the pattern of effects of exposure to high-altitude hypobaric hypoxia environment on the auditory function of C57BL/6J mice, so as to provide a theoretical basis for the targeted prevention and treatment of hearing loss, tinnitus, and otogenic vertigo in high-altitude areas. METHODS: Forty male C57BL/6J mice were randomly divided into a control group and an experimental group. The experimental group was further subdivided into 9 subgroups according to the exposure duration (3, 5, 7, 10, 15, 20, 25, 30, and 35 days), with 4 mice in each subgroup. Mice in the experimental g

What diagnosis should we make for long-lasting vertiginous sensation after acute peripheral vertigo?

BACKGROUND: Differential diagnosis of persistent vertigo/dizziness in patients with a past history of vestibular neuritis (VN) and sudden deafness with vertigo (SDV) could sometimes be difficult for physicians due to variable vertiginous symptoms from rotatory to floating sensation. OBJECTIVES: The main purpose of the present study was to examine the associations between the findings of otology/neurotology examinations in patients at the chronic stage after VN and SDV. MATERIAL AND METHODS: We encountered 1789 successive vertigo/dizziness patients at the Vertigo/Dizziness Center in Nara Medi
