Risk Factors for Postoperative Bleeding after Adenoidectomy.

IMPORTANCE: Postoperative bleeding is a common and potentially life-threatening complication. Precise identification of risk factors in addition to the basic ones, such as coagulation parameters, is certainly very desirable. OBJECTIVE: The aim of this study was to identify other possible risk factors for bleeding after adenoidectomy in children. DESIGN: This observational prospective study enrolled children undergoing adenoidectomy from October 2019 to February 2020, then evaluated the influence of possible risk factors for bleeding. SETTING: Tertiary pediatric otorhinolaryngology center.

Percutaneous Mechanical Aspiration vs Valve Surgery for Tricuspid Valve Endocarditis in People Who Inject Drugs.

BACKGROUND: Valve surgery in tricuspid valve infective endocarditis (TVIE) is controversial in people who inject drugs (PWID) due to perceived risks of reinfection because of recidivism. The study objective was to compare outcomes of percutaneous mechanical aspiration (PMA) using the Penumbra Indigo system to valve surgery in PWID with TVIE. METHODS: Retrospective cohort of adult PWID hospitalized with definite TVIE and received PMA or valve surgery from January 2014 to April 2019. Primary endpoint was all-cause 12-month mortality; secondary endpoints included in-hospital mortality and all-ca

Coordinating across correctional, community, and VA systems: applying the Collaborative Chronic Care Model to post-incarceration healthcare and reentry support for veterans with mental health and substance use disorders.

BACKGROUND: Between 12,000 and 16,000 veterans leave incarceration annually. As is known to be the case for justice-involved populations in general, mental health disorders (MHDs) and substance use disorders (SUDs) are highly prevalent among incarcerated veterans, and individuals with MHDs and SUDs reentering the community are at increased risk of deteriorating health and recidivism. We sought to identify opportunities to better coordinate care/services across correctional, community, and VA systems for reentry veterans with MHDs and SUDs. METHODS: We interviewed 16 veterans post-incarceratio

Cost-utility analysis of low-intensity case management to increase contact with health services among ex-prisoners in Australia.

OBJECTIVES: The economic burden of incarceration is substantial in Australia. People released from prison are at high risk of poor health and this is an important predictor of recidivism. The 'Passports Study' was a randomised controlled trial of an intervention designed to increase health service utilisation after release from prison. The aim of this study is to conduct a cost-utility analysis of this transitional programme. SETTING: Australia DESIGN: A hybrid simulation model was developed to estimate the changes to total economic costs and effectiveness expressed as quality-adjusted life-y

Improving empathy with motivational strategies in batterer intervention programmes: Results of a randomized controlled trial.

OBJECTIVES: Empathy (i.e., the ability to decode emotions, as well as cognitive and emotional empathy) is involved in moral reasoning, prosocial behaviour, social and emotional adequacy, mood and behaviour regulation. Hence, alterations in these functions could reduce behaviour control and the adoption of specific types of violence such as intimate partner violence (IPV). Although interventions for IPV perpetrators focus on reducing IPV risk factors and increasing protective factors to prevent this kind of violence, the study of the effectiveness of these programmes in promoting changes in emp

Correlates of Hepatitis C Virus Infection in the Targeted Testing Program of the New York City Jail System.

OBJECTIVE: The objective of this study was to understand predictors of hepatitis C virus (HCV) antibody positivity in a large urban jail system in New York City. METHODS: We examined demographic characteristics, risk behaviors, and HCV antibody prevalence among 10 790 jail inmates aged 16 to 86 who were screened from June 13, 2013, to June 13, 2014, based on birth cohort or conventional high-risk criteria. We used logistic regression analysis to determine predictors of HCV antibody positivity. RESULTS: Of the 10 790 inmates screened, 2221 (20.6%) were HCV antibody positive. In the multivaria

Nursing case management, peer coaching, and hepatitis a and B vaccine completion among homeless men recently released on parole: randomized clinical trial.

BACKGROUND: Although hepatitis A virus (HAV) and hepatitis B virus (HBV) infections are vaccine-preventable diseases, few homeless parolees coming out of prisons and jails have received the hepatitis A and B vaccination series. OBJECTIVES: The study focused on completion of the HAV and HBV vaccine series among homeless men on parole. The efficacy of three levels of peer coaching (PC) and nurse-delivered interventions was compared at 12-month follow-up: (a) intensive peer coaching and nurse case management (PC-NCM); (b) intensive PC intervention condition, with minimal nurse involvement; and (

[The randomized controlled trial of the prison-based Japanese Matrix Program (J-MAT) for methamphetamine abusers].

Methamphetamine use is subject to severe criminal punishment in Japan and approximately 22% of the prison population were confined for violations of the stimulants control law in 2009. Although the high recidivism rate is also a problem, no systematic treatment has been conducted in prison. Therefore, the development of the prison-based treatment program is necessary. In this study, the prison-based program was developed based on the Matrix Model, which is the cognitive-behavioral treatment for amphetamine users developed in the US. The program was tailored in order to address the treatment ne

Emergency uncrossmatched transfusion effect on blood type alloantibodies.

BACKGROUND: Trauma patients receive emergency transfusions of unmatched Type O Rh-negative (Rh-) blood until matched blood is available. We hypothesized that patients given uncrossmatched blood may develop alloantibodies, placing them at risk for hemolytic transfusion reactions (HTRs). METHODS: Data regarding alloantibody profiles and HTR occurrence were collected from the records of trauma patients at our university-based trauma center who received emergency uncrossmatched blood from July 2008 to August 2010. RESULTS: A total of 132 patients received 1,570 units of packed red blood cells. M

Medroxyprogesterone and paraphiles: do testosterone levels matter?

We examine the associations between pretreatment testosterone (TTS) levels and sociodemographic, clinical, and sexual behavioral characteristics. Two groups, low and normal pretreatment TTS, were treated with medroxyprogesterone acetate (MPA) and compared on clinical response (deviant and nondeviant sexual behaviors; recidivism) and length of time to return to pretreatment TTS after discontinuing MPA. Thirteen paraphilic men who were treated with MPA and had TTS levels monitored at approximately three-month intervals during and after MPA were followed naturalistically. The principal outcome me

Improving early detection of mental disorders & emergencies in resource-limited prison systems: development of a structured screening and risk stratification instrument in Indonesia.

BACKGROUND: Mental health screening in prisons is vital to uphold prisoners' right to mental health services, ensure safety, improve quality of life and reduce recidivism among incarcerated individuals. As frontline staff, correctional officers need a guideline for this screening to refer cases to the appropriate medical services. The currently available screening tool in Indonesia is unsuitable for detecting a number of relevant mental health issues and lacks sensitivity. Therefore, this study aimed to develop a new screening tool for mental disorders among inmates in Indonesia through an evi

Recovery Capital and Family Support Among Incarcerated Men with a History of Psychoactive Substance Use in a Single Romanian Penitentiary: A Qualitative Study.

Background/Objectives: Substance use disorders are common in the prison population and are associated with relapse and criminal recidivism, yet qualitative evidence on recovery capital and family support within Eastern European penitentiary systems remains limited. This study explored how incarcerated men with a history of substance use perceive initiation, withdrawal, recovery, and the role of family support as a recovery capital resource. Methods: A qualitative descriptive study was conducted in a Romanian penitentiary with 50 definitively sentenced men with self-reported substance use histo

Fecal Microbiota Transplantation Is Associated with Better Survival Compared to Standard of Care in Severe Alcoholic Hepatitis Not Responding to Corticosteroids: A Systematic Review and Meta-Analysis.

Background: Alcohol-related liver disease (ALD) affects 4.8% of the global population. Among these patients, between 13.4% and 19.6% suffer from alcoholic hepatitis (AH), which, in its severe form, is associated with significant short- and long-term mortality. Current therapeutic options are limited, offering only modest short-term survival benefits. Recent studies suggest that microbiota-based therapies may offer a novel therapeutic opportunity for patients with ALD. Methods: Databases including Embase, Medline, and CENTRAL were searched until 4 February 2026. The pre-registered protocol on P

Investigating Key Elements of Peer Support Programs Focused on Recovery and Reentry in Community-Based Organizations: A Qualitative Implementation Science Study.

BACKGROUND: Peer support programs are recognized as an evidence-based practice to effectively support individuals in recovery from substance use and reduce recidivism. Peer support is provided by trained individuals with lived experience who guide individuals navigating similar life challenges. Organizations design and structure peer support programs differently; some have developed unique approaches to meet the needs of the individuals they serve. This study provides guidance for implementing peer support programs within community based organizations with a focus on recovery or reentry after

Population ageing, incarceration and the growing digital divide: Understanding the effects of digital literacy inequity experienced by older people leaving prison.

BACKGROUND: Digital inequity refers to the inequality and exclusion experienced by those who lack the same opportunities or circumstances to support the development of digital skills as the rest of modern society. One rapidly growing and highly vulnerable group to digital inequity is older people attempting to reintegrate into society after release from prison, where technology access is limited. Inadequate support for digital skills in this population entails widespread consequences for public health, human rights, social welfare and recidivism. This qualitative study is the first to: examine
