Superior Labral Anterior to Posterior Tears: Repair, Tenodesis, or Both?

Superior labrum anterior to posterior (SLAP) tears are highly prevalent in overhead athletes and heavy laborers. Biomechanical adaptations as well as anatomic factors make the area prone to injury with a diminished healing potential. Previously, SLAP tears were repaired frequently with often poor postoperative function secondary to increased stiffness. Recently, the trend has shifted toward addressing the bicep anchor with a bicep tenodesis. Return to sports is multifactorial and depends on type of activity; however, about 79.5% of athletes are able to return to sport after an isolated SLAP re

Slow-pitch softball injuries.

A prospective investigation of slow-pitch softball injuries incurred in Hawaii was undertaken to study the nature of these injuries and analyze their causes. The injured players were involved in league softball with referees. There were 83 athletes and 93 injuries. A retrospective review of all Army softball-related admissions was also done. Thirty-five athletes or 42% were injured while sliding, 29 "foot first" and the remainder "hand first". Twenty-five of 27 ankle injuries caused by sliding included 20 fractures, 3 sprains, and 2 complete closed posterior dislocations. Analysis of these inj

Non-Time-Loss and Time-Loss Softball Injuries in Secondary School Athletes: A Report From the National Athletic Treatment, Injury and Outcomes Network (NATION).

CONTEXT: Most studies of injury trends associated with softball focus on injuries requiring at least 24 hours of missed participation time (time-loss [TL] injuries), with little focus on those that do not (non-time-loss [NTL] injuries). A better understanding of injury trends associated with softball will improve athlete care. OBJECTIVE: To describe NTL and TL injuries experienced by secondary school girls' softball players. DESIGN: Descriptive epidemiology study. SETTING: Secondary school athletic training clinics. PATIENTS OR OTHER PARTICIPANTS: Secondary school girls' softball players.

Predictors of Unsuccessful Nonoperative Management of Capitellar Osteochondritis Dissecans.

BACKGROUND: Osteochondritis dissecans of the humeral capitellum (capitellar OCD) is a common injury among adolescent throwing athletes. Some younger patients with incomplete maturity of the epiphysis and early-stage capitellar OCD are good candidates for nonoperative treatment. However, during initial examination, predicting the need for surgical treatment in patients with capitellar OCD is difficult. PURPOSE: To perform multivariate ordered logistic regression analysis of data obtained from patients' medical records and images on initial examination and identify the predictors of unsuccessfu

Persistent Vagal Activation Caused by a Large Mesenteric Hematoma After Blunt Abdominal Trauma: A Case Report.

In patients with trauma, decreased circulating blood volume and pain increase sympathetic nerve activation, which increases the heart rate (HR) and vasoconstriction. When this compensatory mechanism is disrupted, the effects of trauma are more severe. Stimulation via the afferent vagal nerves from the gastrointestinal tract causes vagal activation, inducing a cardioinhibitory response that sometimes causes shock and bradycardia. A 51-year-old man sustained an abdominal injury while playing baseball. His blood pressure was normal; however, examination revealed persistent sinus bradycardia (HR:

Auditory Verb Generation Performance Patterns Dissociate Variants of Primary Progressive Aphasia.

Primary progressive aphasia (PPA) is a clinical syndrome in which patients progressively lose speech and language abilities. Three variants are recognized: logopenic (lvPPA), associated with phonology and/or short-term verbal memory deficits accompanied by left temporo-parietal atrophy; semantic (svPPA), associated with semantic deficits and anterior temporal lobe (ATL) atrophy; non-fluent (nfvPPA) associated with grammar and/or speech-motor deficits and inferior frontal gyrus (IFG) atrophy. Here, we set out to investigate whether the three variants of PPA can be dissociated based on error pat

Outcomes and failure factors in surgical treatment for osteochondritis dissecans of the capitellum.

BACKGROUND: Osteochondritis dissecans (OCD) of the capitellum is an intra-articular lesion and one of the leading causes of permanent elbow disability. The treatment of advanced capitellar OCD remains challenging because of the limited potential of the articular cartilage for self-repair. The purpose of this study was to investigate the outcome of surgical treatment for OCD of the capitellum. METHODS: From 2000 to 2010, 32 male patients who had advanced lesions of capitellar OCD were treated operatively. The mean age of the patients was 14.4 years at the time of surgery. Twenty-nine patients

A retrospective evaluation of magnetic resonance imaging effectiveness on capitellar osteochondritis dissecans among overhead athletes.

BACKGROUND: Magnetic resonance imaging (MRI) has been widely used to characterize osteochondritis dissecans (OCD) lesions. However, the usefulness of MRI for predicting fragment stability in OCD of the humeral capitellum (capitellar OCD) remains unclear. HYPOTHESIS: Preoperative MRI cannot accurately diagnose fragment instability of capitellar OCD in overhead athletes. STUDY DESIGN: Cohort study (diagnosis); Level of evidence, 1. METHODS: Twenty-seven male overhead athletes who had undergone surgery for capitellar OCD were included in the study. A single senior musculoskeletal radiologist b

Elbow range of motion is stable or improves following ulnar collateral ligament repairs and reconstructions.

BACKGROUND: The increasing incidence of ulnar collateral ligament (UCL) injuries in throwing athletes has resulted in a concomitant increase in annual UCL repair and reconstruction procedures. While elbow stiffness remains a small but significant postoperative complication, little data have been published regarding range of motion (ROM) trajectories postoperatively and factors that impact final recovery. This study aimed to determine the trajectory of elbow ROM recovery following UCL repairs and reconstructions and to identify the factors that impact a patient's final motion. METHODS: For our

Treatment of partial ulnar collateral ligament tears in the elbow with platelet-rich plasma.

BACKGROUND: Studies have demonstrated the potential of platelet-rich plasma (PRP) to heal damaged tissue. To date, there are no published reports of clinical outcomes of partial ulnar collateral ligament (UCL) tears of the elbow treated with PRP. HYPOTHESIS: Platelet-rich plasma will promote the healing of partial UCL tears and allow a return to play. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: Thirty-four athletes with a partial-thickness UCL tear confirmed on magnetic resonance imaging were prospectively followed. All patients had failed at least 2 months of nonoperative tre

Impact of Spencer Technique on Pain, Range of Motion, and Functional Disability in Patients With Frozen Shoulder: A Pilot Study.

Background and objective The shoulder is the most flexible ball- and socket-type joint in the human body. The pathological condition that can commonly affect this joint is the frozen shoulder. This condition is marked by pain and stiffness in the area surrounding the shoulder complex. This leads to difficulty in doing the daily activities of living. Exercise and physical therapy are mostly recommended to decrease pain and improve and maintain the range of motion (ROM). Mainly traditional techniques such as Mulligans, Maitland, and Kaltenborn are used, along with electrotherapy and exercises, f

Arthroscopic Repair of Type II SLAP Lesions in Overhead Athletes.

For overhead athletes and, in particular, baseball pitchers, the rates of success and return to play for those who have undergone arthroscopic repair of type II SLAP lesions are poor, ranging from 7% to 62%. The reasons for the poor results and high failure rates in overhead athletes with type II SLAP repairs are multifactorial and are a combination of many factors. These factors include the failure to establish the diagnosis and treat these athletes preoperatively; the inability of the operating surgeon to differentiate normal anatomic variants from pathologic SLAP lesions at the time of surg

The neuropathology of chronic traumatic encephalopathy.

Chronic traumatic encephalopathy (CTE) is a neurodegenerative tauopathy associated with repetitive head trauma, including concussion and subconcussion. CTE was first recognized in boxers nearly a century ago as "dementia pugilistica" or "punch drunk," but has been recently identified in contact sports athletes (including American football, ice hockey, soccer, baseball, rugby, boxing, and wrestling) and military veterans exposed to blast. Similar to many other neurodegenerative diseases, CTE is diagnosed conclusively only by neuropathologic examination of brain tissue. CTE is characterized by t

Histological evaluation of low-intensity pulsed ultrasound on osteochondritis dissecans of the humeral capitellum.

BACKGROUND: The clinical use of low-intensity pulsed ultrasound (LIPUS) was recently evaluated in cases of osteochondritis dissecans of the humeral capitellum (elbow OCD). However, the mechanism underlying the effect of LIPUS in elbow OCD is not well understood. The aim of this study was to histopathologically evaluate the effect of LIPUS irradiation on elbow OCD. METHODS: Fifteen patients with elbow OCD were enrolled in this study. All patients were juvenile baseball players (average age, 13.1 years). LIPUS was performed under the same conditions as the fracture treatment for an average leng
