Neuraxial Analgesia for Scoliosis Correction: A Case Series in a Specialised Children's Centre.

Surgical correction of spinal deformities in children presents a challenge to the anaesthetist due to the extensive nature of the surgery, the co-morbidities of the patients and the constraints on aesthetic techniques of intraoperative neurophysiological monitoring of the spinal cord. Patients undergoing scoliosis surgery are considered to suffer severe pain, which may lead to a negative impact on patient psychology and physical well-being. By using effective postoperative pain regimens to enhance recovery after surgery, pain can be significantly reduced, leading to patient satisfaction, facil

Assessment of Neurone-Specific Enolase, Glial Fibrillary Acidic Protein and S100 B as Spinal Cord Ischemia Biomarkers in Patients Undergoing Open and Endovascular Complex Aortic Surgery: A Single-Center Experience.

BACKGROUND: Despite all efforts, spinal cord ischemia (SCI) is a relevant and feared complication after open and endovascular thoracoabdominal aortic aneurysm (TAAA) repair. Besides the established correlation of motor evoked potentials (MEPs) and SCI, the usage of biomarkers for early detection of SCI intraoperatively and postoperatively after TAAA surgery is scarcely described in literature. METHODS: The methods include retrospective assessment of 33 patients (48.48% male) undergoing open and endovascular TAAA repair between January 2017 and January 2018. Levels of the biomarkers neurone-sp

Symptomatic epilepsy in children with poroencephalic cysts secondary to perinatal middle cerebral artery occlusion.

BACKGROUND: Perinatal cerebral artery occlusion is responsible for ischemic cerebral infarction leading to brain cavitation and gliosis; the territory of the middle cerebral artery is most frequently involved. The resulting poroencephalic cysts are frequently associated with hemiplegia and epilepsy; that can be managed medically in most cases, only 6-7% of them being refractory to medical treatment. This particular subset of congenitally hemiplegic children will be possible candidates for electrophysiological investigation and eventually for resective surgery. Whatever the kind of surgical tre

[On the occasion of my retirement as head of the Neurochirurgische Universitätsklinik Zürich--changing aspects of treatment modality in modern neurosurgery and of neuroscience research. Presentation of our experience and historical backgrounds].

On the occasion of my retirement as head of the Neurochirurgische Universitätsklinik Zürich on 31. May 2007, the changing aspects of neurosurgery in diseases dealt with and in their treatments during the last 70 years (from the foundation of the department by Prof. Krayenbuehl in 1937 successed by Prof. Yaşargil and later on by myself.) were presented. Number of surgical procedures were 1,000 times per year during the interval and the busiest era was that of Prof. Krayenbühl, in which it amounted to as much as 1,500 at the maximum. Lumbar disc surgery was the most frequent one (around 250/year

Management of Spinal Bone Metastases With Radiofrequency Ablation, Vertebral Reinforcement and Transpedicular Fixation: A Retrospective Single-Center Case Series.

Spine is a frequent site of bone metastases, with a 8.5 months median survival time after diagnosis. In most cases treatment is only palliative. Several advanced techniques can ensure a better Quality of Life (QoL) and increase life expectancy. Radiofrequency ablation (RFA) uses alternating current to produce local heating and necrosis of the spinal lesion, preserving the healthy bone. RFA is supported by vertebral reinforcement through kyphoplasty and vertebroplasty in order to stabilize the fracture with polymethylmethacrylate (PMMA) injection, restoring vertebral body height and reducing th

Beyond surgical radicality in intramedullary spinal cord metastases: neurological function and systemic disease burden drive patient outcomes.

PURPOSE: Intramedullary spinal cord metastases (ISCM) are rare, clinically challenging lesions with limited evidence-based guidance. Optimal surgical management remains controversial, particularly regarding the ideal extent of resection (EOR) and associated prognostic factors. This study systematically evaluates perioperative outcomes, neurological function, and short-term survival according to biopsy-only, subtotal, or gross total resection (GTR) approaches. METHODS: This retrospective single-center study included 16 patients treated surgically for histologically confirmed ISCM between 2015

A Comprehensive Review of the Role of the Latest Minimally Invasive Neurosurgery Techniques and Outcomes for Brain and Spinal Surgeries.

Minimally invasive neurosurgical techniques offer reduced surgical trauma, lower complication rates, and faster recovery compared to traditional open procedures. Approaches such as keyhole craniotomy, endoscopic skull base surgery, endovascular neurosurgery, and percutaneous spinal techniques have advanced the management of cranial and spinal pathologies. These methods aim to minimize tissue disruption, blood loss, and hospital stay while maintaining surgical efficacy. This narrative review explores their evolution, applications, and advantages in modern neurosurgery.

The erectile and ejaculatory implications of the surgical management of rectal cancer.

Colorectal cancer is a significant cause of cancer-related deaths worldwide. Although advances in surgical technology and technique have decreased mortality rates, surviving patients often experience sexual dysfunction as a common complication. The development of the lower anterior resection has greatly decreased the use of the radical abdominoperineal resection surgery, but even the less radical surgery can result in sexual dysfunction, including erectile and ejaculatory dysfunction. Improving the knowledge of the underlying causes of sexual dysfunction in this context and developing effectiv

Towards a New Dawn for Neuro-Oncology: Nanomedicine at the Service of Drug Delivery for Primary and Secondary Brain Tumours.

(1) Background/Objectives: Primary and secondary brain tumours often hold devastating prognoses and low survival rates despite the application of maximal neurosurgical resection, and state-of-the-art radiotherapy and chemotherapy. One limiting factor in their management is that several antineoplastic agents are unable to cross the blood-brain barrier (BBB) to reach the tumour microenvironment. Nanomedicine could hold the potential to become an effective means of drug delivery to overcome previous hurdles towards effective neuro-oncological treatments. (2) Methods: A scoping review following th
