Microvascular Decompression in Patients Aged 30 Years or Younger.

OBJECTIVE: The aim of this study was to identify the etiology of hemifacial spasm (HFS) and trigeminal neuralgia (TN) in patients aged 30 years or younger and to examine the efficacy of microvascular decompression (MVD). PATIENTS AND METHODS: Between 1996 and 2012, 228 HFS and 190 TN patients underwent MVD at Atsuchi Neurosurgical Hospital. Of these, 7 patients were 30 years of age or younger at the time of treatment (HFS: n = 6, TN: n = 1). Assessments were based on their medical history and on magnetic resonance imaging, magnetic resonance angiography, surgical, and follow-up findings. RES

Bilateral trigeminal neuralgia and charcot-marie-tooth disease: diagnosis and successful microsurgical treatment of bilateral neurovascular compression.

BACKGROUND: The association of Charcot-Marie-Tooth (CMT) disease and trigeminal neuralgia (TN) is rare. CMT and bilateral TN is even rarer. Our literature review has revealed only 4 previous cases with CMT and bilateral TN. We report the case of a 23-year-old man with a prior diagnosis of CMT and unilateral deafness who initially presented with excruciating and lancinating right-sided facial pain. PATIENT AND METHOD: Initially treated with percutanous ablation elsewhere with only short-lasting relief, the patient was on toxic doses of medication when he presented to us. Neuro-imaging using ma

Pain-free survival after vagoglossopharyngeal complex sectioning with or without microvascular decompression in glossopharyngeal neuralgia.

OBJECTIVE: Glossopharyngeal neuralgia (GN) is a rare pain condition in which patients experience paroxysmal, lancinating throat pain. Multiple surgical approaches have been used to treat this condition, including microvascular decompression (MVD), and sectioning of cranial nerve (CN) IX and the upper rootlets of CN X, or a combination of the two. The aim of this study was to examine the long-term quality of life and pain-free survival after MVD and sectioning of the CN X/IX complex. METHODS: A combined retrospective chart review and a quality-of-life telephone survey were performed to collect

Clinical analysis of trigeminal neuralgia caused by vertebrobasilar dolichoectasia.

Our objective is to explore the clinical manifestations, imaging features, and therapy of trigeminal neuralgia (TN) caused by vertebrobasilar dolichoectasia (VBD). Clinical and imaging data of 11 cases with trigeminal neuralgia caused by VBD were retrospectively analyzed, and relevant literatures were reviewed. Of these 11 patients, 8 were male, and 10 suffered from hypertension. Imaging findings revealed that the vertebrobasilar arteries were pathologically enlarged and tortuous. Facial pain disappeared or was relieved after the microvascular decompression (MVD) in all 11 patients; no recrude

Efficacy of subcutaneous sumatriptan in postcraniotomy pain and opioid consumption.

OBJECTIVE: Traditional pain management pathways following craniotomy are predicated on opioids. However, narcotics can confound critical neurological examination, contribute to respiratory depression, lower the seizure threshold, and lead to medication habituation, dependence, and/or abuse. Alternative medications to better address postoperative pain while mitigating opioid-related adverse effects remain insufficiently studied. Preliminary studies suggest sumatriptan, a 5-HT (1B/1D) receptor agonist known to regulate dural vasoactivity and inflammation, may moderate pain following trigeminal m

Preliminary Report of Fully Endoscopic Microvascular Decompression.

OBJECTIVE: Microscopic microvascular decompression (MVD) has been considered to be a useful treatment modality for medically refractory hemifacial spasm (HFS) and trigeminal neuralgia. But, the advent of the endoscopic era has presented new possibilities to MVD surgery. While the microscope remains a valuable tool, the endoscope offers several advantages with comparable clinical outcomes. Thus, fully endoscopic MVD (E-MVD) could be a reasonable alternative to microscopic MVD. This paper explores the safety and efficacy of the fully E-MVD technique. METHODS: A single-center retrospective study

A Clinical Analysis of Secondary Surgery in Trigeminal Neuralgia Patients Who Failed Prior Treatment.

OBJECTIVE: Although many treatment modalities have been introduced for trigeminal neuralgia (TN), the long-term clinical results remain unsatisfactory. It has been particularly challenging to determine an appropriate treatment strategy for patients who have responded poorly to initial therapies. We analyzed the surgical outcomes in TN patients who failed prior treatments. METHODS: We performed a retrospective analysis of 37 patients with recurrent or persistent TN symptoms who underwent surgery at our hospital between January 2010 and December 2014. Patients with follow-up data of at least on

Stereotactic radiosurgery for idiopathic trigeminal neuralgia.

OBJECT: Each year a greater number of patients with trigeminal neuralgia (TN) undergo radiosurgery, including a large number of patients who are candidates for microvascular decompression (MVD). METHODS: The case characteristics and outcomes of 117 consecutive patients who underwent radiosurgery were retrieved from a prospectively maintained database. The mean patient age was 67.8 years; and the majority (58%) of patients had undergone surgery previously. The dependent variable for all analyses of facial pain was complete pain relief without medication (excellent outcome). Median follow-up du

The Location of the Parasympathetic Fibres within the Vagus Nerve Rootlets: A Case Report and a Review of the Literature.

The vagus nerve has motor, sensory, and parasympathetic components. Understanding the nerve's internal anatomy, its variations, and relationship to the glossopharyngeal nerve are crucial for neurosurgeons decompressing the lower cranial nerves. We present a case report demonstrating the location of the parasympathetic fibres within the vagus nerve rootlets. A 47-year-old woman presented with a 1-year history of medically refractory left-sided glossopharyngeal neuralgia and a more recent history of left-sided hemi-laryngopharyngeal spasm. magnetic resonance imaging showed her left posterior inf

Potential Role of Turbulent Cerebrospinal Fluid Flow in Type 1 Trigeminal Neuralgia Without Neurovascular Compression: Insights From a Cerebrospinal Fluid Dynamics Study.

BACKGROUND AND OBJECTIVES: Trigeminal neuralgia (TN) is a severe facial pain disorder typically caused by neurovascular compression (NVC) of the trigeminal nerve. However, a subset of patients with Type 1 TN presents without identifiable NVC on preoperative imaging. The pathophysiology in these cases remains unclear, and optimal surgical strategies are not well established. This study investigates the role of cerebrospinal fluid (CSF) flow dynamics in NVC-negative TN and explores its implications for surgical intervention. METHODS: This single-center prospective study included 14 patients wit

Hybrid Microvascular Decompression for Cranial Neurovascular Compression Syndromes: A Retrospective Case Series and Surgical Nuances.

BACKGROUND: Microvascular decompression (MVD) remains the gold standard for the treatment of neurovascular compression syndromes (NVCS). Adjunctive visualization tools have been introduced to improve visualization beyond the direct microscopic line-of-sight. This study evaluated the technical feasibility, safety, and short-to mid-term clinical outcomes of QEVO-assisted microscopic MVD. METHODS: This retrospective series included 43 patients who underwent QEVO-assisted MVD for trigeminal neuralgia (TN, 31), hemifacial spasm (HFS, 11), and cochleo-vestibular nerve compression syndrome (1) betwe

A cost effectiveness analysis of two treatment strategies for trigeminal neuralgia in Ontario.

BACKGROUND: Trigeminal neuralgia (TN) is a debilitating disease with an annual incidence of approximately 4-27/100,000. In Ontario, over 2000 patients receive interventions for profound pain, including medical and surgical therapies. The global expected cost of these approaches is unknown. This study aims to analyze the cost-effectiveness of one surgical therapy, microvascular decompression (MVD), compared with the best medical therapy (carbamazepine) as first-line therapy. METHODS: Costs were gathered from the Canadian Institute for Health Information, Ontario Drug Benefit Formulary, and Ont

The role of biomarkers in drug-resistant trigeminal neuralgia: a prospective study in patients submitted to surgical treatment.

BACKGROUND: Molecular mechanisms underlying trigeminal neuralgia (TN) have been poorly understood. Recently, different biomarkers have been studied in several chronic neuropathic diseases or in neuronal damage, but their role in TN has not yet been investigated. Here, we firstly analyzed the serum levels of the neuron-specific enolase (NSE) (as an index of neuronal tissue damage) in TN patients submitted to surgical treatment. Different cytokines and interleukins related to inflammation were also studied. METHODS: Blood samples from 40 patients were prospectively collected preoperatively and

Prevention of Superior Petrosal Vein Injury during Microvascular Decompression for Trigeminal Neuralgia: Operative Nuances.

Background  The superior petrosal vein (SPV) often obscures the surgical field or bleeds during microvascular decompression (MVD) for the treatment of trigeminal neuralgia. Although SPV sacrifice has been proposed, it is associated with multiple complications. We have performed more than 4,500 MVDs, including approximately 400 cases involving trigeminal neuralgia. We aimed to describe our operative technique and nuances to avoid SPV injury. Methods  We have provided a detailed description of our institutional protocol, including the anesthesia technique, neurophysiologic monitoring, patient po

Impact of US state government regulation on patient access to elective surgical care.

PURPOSE: Rising health care costs in the United States have led to government regulation of services via a Certificate of Need (CON) law in many states. Such regulation may decrease access to elective surgical procedures. This study describes the impact of CON laws on elective surgical care. METHODS: This retrospective cohort trial used data from the Health Care Utilization Project, a publicly available, inpatient database. Rates of six elective procedures were compared between 21 CON states and 5 non-CON states (2004-2005). Further, facility type (non-profit versus for-profit), facility teac

Outcomes of Microvascular Decompression for Young-Onset Trigeminal Neuralgia.

BACKGROUND AND OBJECTIVES: The surgical treatment of trigeminal neuralgia (TN) in young patients is challenging, and there is mixed evidence regarding the efficacy of microvascular decompression (MVD) in this patient population. We examined long-term clinical outcomes after MVD in patients with pediatric and young-onset TN. METHODS: Between 2007 and 2023, 22 patients with classical TN symptom onset before age 30 years underwent MVD for TN at or before age 30 years. A retrospective chart review was used to gather information on clinical characteristics, imaging, and long-term outcomes in these

Radiological characteristics predicting early poor drug response in patients with hemifacial spasm.

OBJECTIVES: Patients with hemifacial spasm (HFS) often resort to botulinum toxin injections or microvascular decompression surgery when medication exhibits limited effectiveness. This study aimed to identify MRI and demographic factors associated with poor drug response at an early stage in patients with HFS. METHODS: We retrospectively included patients with HFS who underwent pre-therapeutic MRI examination. The presence, location, severity, and the offending vessels of neurovascular compression were blindly evaluated using MRI. Drug responses and clinical data were obtained from the medical
