Genetic characterization of antibiotic resistance in enteropathogenic Escherichia coli carrying extended-spectrum beta-lactamases recovered from diarrhoeic rabbits.

A total of 52 Escherichia coli strains isolated from diarrhoeic rabbits were investigated for their enteropathogenic E. coli (EPEC) pathotype by PCR amplification of eae and bfp virulence genes. A total of 22 EPEC isolates were identified, serotyped and studied for antibiotic resistance and screened for the detection of extended-spectrum beta-lactamases (ESBLs). The EPEC isolates belonged to three serogroups (O26, O92 and O103). The most common serogroup (O103:K-:H2) was observed among 17 EPEC strains, the O92:K-serogroup in three isolates (the antibiotic sensitive ones) and the remaining O26:

Catalytic properties of an endogenous beta-lactamase responsible for the resistance of Azospirillum lipoferum to beta-lactam antibiotics.

Azospirillum lipoferum RG20, a nitrogen-fixing bacterium found in all kind of soils, was found to be naturally resistant to penicillins and cephalosporins. 6-beta-Bromopenicillanic acid, an irreversible inhibitor of serine-beta-lactamases, completely abolished this resistance. A beta-lactamase was purified 518-fold from a cell-free extract of A. lipoferum RG20. A single band on SDS-PAGE (apparent molecular mass 31000 Da) and on isoelectric focussing (pI9.35) was observed with the purified protein. The enzyme hydrolysed benzylpenicillin, ampicillin, cephalothin and cephaloridine with comparable

Comparison of five different methods for detection of SHV extended-spectrum beta-lactamases.

Five different methods for detection of different types of SHV extended-spectrum beta-lactamases (ESBL) were compared: minimum inhibitory concentration (MIC) determination of beta-lactam with and without clavulanic acid, double-disk synergy test (DDST), inhibitor potentiated disk diffusion test (IPDDT), three-dimensional test (TDT) and PCR/Nhe I test. MIC determination of beta-lactam with and without clavulanic acid was the most sensitive method regardless of the type of beta-lactamase. However the specificity of this method was a little above 90%. IPDDT turned out to be a very sensitive metho

Which prophylactic regimen for which surgical procedure?

For optimal prevention of infection subsequent to a surgical intervention, it is necessary to follow a series of general principles, including the classification of the type of surgical intervention, the characteristics of the antibiotic used, and the route and the time of its administration. Moreover, with reference to the different types of surgery, other factors assume importance: the etiology of the infection and the ability of the antibiotic to achieve adequate levels in the tissues at the beginning of the infective process. In general abdominal, biliary, and obstetric-gynecologic surgery

The role of the newer antimicrobial agents in obstetrics and gynecology.

The new antibiotics include interesting compounds--extended-spectrum cephalosporins and penicillins, combinations of older antibiotics plus beta-lactamase inhibitors, and new classes such as the monobactams and fluoroquinolones. In addition to extended spectrums, some of these compounds offer more favorable kinetics, less toxicity, or decreased cost. Several general conclusions might help place this array of new antibiotics in a useful clinical perspective. The newer antibiotics discussed here are no more effective than what is currently used. However, several have very good in-vitro activity

Successful Treatment of Bloodstream Infection Due to Metallo-β-Lactamase-Producing Stenotrophomonas maltophilia in a Renal Transplant Patient.

Stenotrophomonas maltophilia is an emerging multidrug-resistant (MDR) opportunistic pathogen for which new antibiotic options are urgently needed. We report our clinical experience treating a 19-year-old renal transplant recipient who developed prolonged bacteremia due to metallo-β-lactamase-producing S. maltophilia refractory to conventional treatment. The infection recurred despite a prolonged course of colistimethate sodium (colistin) but resolved with the use of a novel drug combination with clinical efficacy against the patient's S. maltophilia isolate.

Correlation of type 3 secretion system genes with antimicrobial resistance in Pseudomonas aeruginosa: A unique clinical signature.

OBJECTIVES: Pseudomonas aeruginosa is a well-known opportunistic pathogen with intrinsic multidrug resistance, extensive drug resistance, and multiple virulence factors, making hospital-acquired infections therapy resistant. The type 3 secretion system (T3SS) and its four effector proteins, i.e., ExoS, ExoT, ExoU, and ExoY, have invasive and cytotoxic effects that lead to host cell death. The significant correlations of exo genes with the resistance attributes of P. aeruginosa in disease progression were evaluated in the current study. METHODS: In total, 102 random clinical strains of Pseudom

Addressing therapeutic options for KPC-3-producing ST307-Klebsiella pneumoniae: insights from in vitro evolution and mutant prevention strategies.

Ceftazidime-avibactam, meropenem-vaborbactam, and imipenem-relebactam are key therapeutic options for infections caused by KPC-producing Klebsiella pneumoniae (KPC-Kp). Resistance may emerge during therapy, but knowledge on mutant prevention and collateral effects remains limited. Killing kinetics assays of ceftazidime-avibactam, meropenem-vaborbactam, imipenem-relebactam, and the combination of ceftazidime-avibactam with meropenem (CZA + MER) were performed against clinical KPC-3-ST307-Kp isolates and ceftazidime-avibactam-resistant KPC-ST307-Kp mutants (KPC-31/KPC-46/KPC-53/KPC-61/KPC-62/KPC
