Current management of thyroglossal-duct remnant.

OBJECTIVE: Thyroglossal-duct remnant (TGDR) should be differentiated from ectopic thyroid prior to surgical excision. The purpose of this study is to discuss our experience with TGDR and review the role of technetium 99m-labelled sodium pertechnetate (99mTc), thyroid function test (TFT), ultrasound (US), and computerized tomography (CT) scan in the management of TGDR. METHODS AND RESULTS: This retrospective study comprised 24 (18 M:6 F) patients with TGDR, whose ages ranged between 3 and 52 years, 83.3% of whom were less than 30 years of age. The most common complaints were anterior neck swel

[Interdisciplinary therapeutic concept in severe bacterial infections of the central respiratory tract in childhood].

BACKGROUND: Apart from all advances made in the management of central airway infections, Acute Epiglottitis (AE) and Bacterial Tracheitis (BT) continue to be causes of life-threatening airway obstruction in children. The aim of this retrospective study was to evaluate deficiencies in the diagnostical protocol, to clarify the role of airway endoscopy in acute childhood stridor, and to identify current reasons for fatalities in these diseases. MATERIAL: In the observation period between 1980-92, we found 12 patients suffering from BT and 21 from AE managed in close cooperation of the involved d

Growth and Development of Human Laryngeal Functions.

OBJECTIVES: Human laryngeal functions are respiration (airway), local immunity, airway protection (swallowing) and phonation. Newborn larynges resemble those of non-human primates and lack mature functional structures like adults. This study summarizes the development of laryngeal functions, focusing particularly on their morphological and functional interrelationships. METHODS: Human larynges from autopsy cases were examined using whole-organ serial section studies, integrating findings from our previous studies. RESULTS: In newborns, the posterior glottis (intercartilaginous portion) occup

Laryngeal chondromas: Current knowledge and future directions.

OBJECTIVE: Cartilaginous tumors of the larynx are rare, representing less than 1% of all laryngeal tumors. Chondromas are benign mesenchymal tumors characterized by a slow-paced growth, primarily originated in the cricoid cartilage, followed by the thyroid, arytenoid, and epiglottic cartilages. This scoping review aims to understand the extent of evidence on the epidemiology, clinical characteristics, morbidity, and recurrence of the laryngeal chondroma (LC). DATA SOURCES: MEDLINE (Ovid), Embase (Elsevier), Web of Science (Clarivate), Cochrane Central Register of Controlled Trials and Systema

Training Requirements in Point-of-Care Ultrasonography of the Upper Airway: A Feasibility Study.

OBJECTIVES: Point-of-care ultrasonography of the upper airway can be a useful supplement to conventional pre-anesthetic clinical evaluations. However, the reliability of such examinations is highly operator-dependent and proper training in sonoanatomy and ultrasound operational skills are required. The objective of this study aims to assess the minimum training requirements for applying a predefined upper airway scanning protocol in healthy volunteers by anesthesia trainees. METHODS: Twenty-two healthy volunteer members of the Operating Room staff participated in the study. A predefined scann

The classical Starling resistor model often does not predict inspiratory airflow patterns in the human upper airway.

The upper airway is often modeled as a classical Starling resistor, featuring a constant inspiratory airflow, or plateau, over a range of downstream pressures. However, airflow tracings from clinical sleep studies often show an initial peak before the plateau. To conform to the Starling model, the initial peak must be of small magnitude or dismissed as a transient. We developed a method to simulate fast or slow inspirations through the human upper airway, to test the hypothesis that this initial peak is a transient. Eight subjects [4 obstructive sleep apnea (OSA), 4 controls] slept in an "iron

Free jejunal diversionary conduit flaps following radiation damage to the pharynx: a technique for regaining oral feeding while preserving vocal function.

UNLABELLED: Radiotherapy is an accepted primary treatment modality for head and neck malignancies. However, in severe cases, the chronic radiation damage has resulted in dysphagia, aspiration and choking. Failure in conservative therapeutic strategies for this swallowing dysfunction will result in either preservation of voice with loss of oral feeding, or vice versa. We introduce our surgical technique based on the free jejunal diversionary conduit flaps, which helps patients to resume oral feeding and preserves vocal function, while reducing the risk of aspiration. METHOD: Six patients suffe

Impaired upper airway integrity by residual neuromuscular blockade: increased airway collapsibility and blunted genioglossus muscle activity in response to negative pharyngeal pressure.

BACKGROUND: Residual neuromuscular blockade increases the risk to develop postoperative complications. The authors hypothesized that minimal neuromuscular blockade (train-of-four [TOF] ratio 0.5-1) increases upper airway collapsibility and impairs upper airway dilator muscle compensatory responses to negative pharyngeal pressure challenges. METHODS: Epiglottic and nasal mask pressures, genioglossus electromyogram, respiratory timing, and changes in lung volume were measured in awake healthy volunteers (n = 15) before, during (TOF = 0.5 and 0.8 [steady state]), and after recovery of TOF to uni

Acute airway obstruction during spinal anaesthesia for caesarean section.

A 30-year-old primiparous Caucasian woman with known placenta praevia required an emergency caesarean section for a mild antepartum haemorrhage at the onset of spontaneous term labour. Following intravenous prehydration with 500 ml gelatin colloid (Haemaccel trade mark ), spinal anaesthesia was induced in the sitting position with 2.6 ml of 0.5% hyperbaric bupivacaine (13 mg). The patient was then placed in the recumbent position with left lateral tilt, whereupon she suddenly became dyspnoeic. A generalized erythematous urticarial rash with associated facial, periorbital, glossal and perioral
