Lemierre's syndrome is a suppurative thrombophlebitis involving the internal jugular vein, most commonly associated with Fusobacterium necrophorum, usually a complication of oropharyngeal infections. This syndrome is rare and is often overlooked. We present a case of sepsis mimicking initially severe leptospirosis (Weil's disease) due to acute febrile illness with multiorgan failure and hyperbilirubinemia. Group C streptococci rarely cause severe disease in humans. The authors report a 15-year-old girl with group C streptococcal pneumonia. No immune deficiencies were detected in our patient and investigation of virulence factors including emmL, sag A, spegg and smeZ genes failed to reveal insight into why the isolated group C streptococcal strain was so virulent. Idiopathic thrombocytopenic purpura is an autoimmune disorder causing thrombocytopenia. The authors present the case of a patient who presented with spontaneous bruising. A complete blood cell count revealed the patient was severely thrombocytopenic with a platelet count of 4000. Further evaluation failed to reveal a cause for thrombocytopenia and the patient was subsequently diagnosed with idiopathic thrombocytopenic purpura.

Establishing the underlying diagnosis in acute renal failure is essential in guiding patient management. To our knowledge, this is the first reported case with concurrent diagnoses of Wegener's granulomatosis and non-steroidal anti-inflammatory drug (NSAID)-induced interstitial nephritis. Renal biopsy should be performed in renal insufficiency patients where several concurrent conditions are suspected to obtain the correct diagnosis and institute appropriate treatment.

Diffuse pachymeningitis is an uncommon presentation of tuberculous meningitis (TBM). We present a 78-year-old woman patient with a 1-year history of progressive headache and MRI of the brain compatible with diffuse pachymeningitis. Without strong evidence to support a diagnosis, she subsequently underwent dural biopsy which revealed caseous granulomatous inflammation and was positive for Mycobacterium tuberculosis complex by PCR. The syndrome of inappropriate secretion of antidiuretic hormone (SIADH) is a well-known complication in malignancy, especially small cell carcinoma of the lung. However its association with pancreatic carcinoma is rare. To the best of our knowledge, only 9 cases have been reported in the literature. Eosinophilic fasciitis (EF) is a rare connective tissue disorder characterized by symmetrical sclerodermatous skin changes primarily affecting the extremities and histologically, by thickening of the fascia with chronic inflammatory infiltrate containing eosinophils. EF is associated with peripheral eosinophilia, hypergammaglobulinemia, and an elevated ESR. Reported is a case of EF in a 57-year-old Japanese-American woman who refused treatment with prednisone, review of other treatment options, and discussion of key differences between this disease and scleroderma.

Cryptococcus neoformans is the most common cryptococci causing infection in humans. Non-neoformans cryptococci have generally been regarded as saprophytes and rarely reported as human pathogens. Since vaccination is critical in responding to smallpox exposure, vaccination strategies must be evaluated during bioterrorism preparedness. Information on historical factors, smallpox characteristics, public health capabilities and hypothetical attack scenarios was used to evaluate major vaccination strategies. In event of a smallpox attack, the optimal strategy is situational, mass vaccination may be best for dense island populations such as Oahu.

Talc is a commonly used sclerosing agent for pleurodesis. However, there have been recent reports of a number of complications associated with the use of talc. Although there is significant data regarding the respiratory complications associated with talc, reports of bleeding complications are extremely rare. We present the case of a 78 year-old man who was treated with talc pleurodesis for recurrent pleural effusion and subsequently developed a massive hemothorax, a rare complication.
