Dyke-davidoff-masson syndrome: A case report of an adolescent boy at a tertiary hospital in Adamawa, North-Eastern Nigeria.

Dyke-Davidoff-Masson Syndrome (DDMS) is a rare clinical condition in which atrophy or hypoplasia of one cerebral hemisphere occurs secondary to brain insult during fetal or early childhood, which results in variable clinical manifestations like hemiparesis, seizures, expressive aphasia, and mental retardation. This rare entity mainly presents in childhood and is unusual in adults. DDMS is a rare cause of epilepsy and should be considered and excluded in cases of refractory seizures. Few cases have been reported from a developing nation like Nigeria but not from the north-eastern part of Nigeri

Fungal Endophthalmitis in a Case of Rhino-Orbito-Cerebral Mucormycosis: Successfully Treated With Amphotericin B Colloidal Dispersion.

BACKGROUND: Rhino-orbito-cerebral mucormycosis (ROCM) is an acute, fulminant, opportunistic fungal infection that usually occurs in diabetes or immunocompromised patients. Amphotericin B combined with surgical debridement remains the standard treatment, although it is controversial due to its lager nephrotoxicity. Thus far, no studies have reported the treatment for ROCM-associated fungal endophthalmitis because the exact pathogenesis and transmission routes in ROCM remain unclear. Here, we reported a case of ROCM complicated with fungal endophthalmitis treated favorably with amphotericin B co

Primary and Secondary/ Metastatic Salivary Duct Carcinoma Presenting within the Sinonasal Tract.

Traditionally, sinonasal adenocarcinomas have been subdivided into intestinal (ITAC) and non-intestinal (non-ITAC) categories. The latter encompasses salivary-type adenocarcinomas originating from the seromucinous glands of the sinonasal mucosa and non-salivary adenocarcinomas. The non-salivary adenocarcinoma category is further subdivided into low-and high-grade variants. Among salivary-type sinonasal adenocarcinomas, tumors recapitulating salivary duct carcinoma (SDC) are exceedingly rare, but some might have been lumped into the high-grade non-ITAC category. To date, only three primary SDCs

Surgical Management of Rhabdomyosarcoma of the Nasal Cavity and Paranasal Sinuses: Analysis of Operative Indications, Settings, and Outcomes.

Objectives  The role of surgery in management of sinonasal rhabdomyosarcoma (SNRMS) has traditionally been limited, owing to anatomic and technological challenges and the established role of systemic therapy. Herein, we report our institutional experience with surgical management of SNRMS, with a particular focus on operative approaches, extent and outcomes. Design  This study is a retrospective cohort study. Setting  This study was conducted at a single-institution, academic center. Participants  Patients of any age with histologically confirmed RMS of the nasal cavity, maxillary, ethmoid, fr

The Analysis of Patients Operated for Frontal Sinus Osteomas.

OBJECTIVE: Paranasal sinus osteomas are benign tumors that are smooth-walled, slow-growing, and induced by bone tissue. Although their most common localization is the frontal sinus, some osteomas are seen in the ethmoid, maxillary, and sphenoid sinuses. Frontal sinus osteomas are often asymptomatic; however, when they become symptomatic, headache is the most common complaint. In this study, we aimed to analyze the postoperative results of patients who were diagnosed with frontal sinus osteoma and were operated with appropriate surgical techniques. METHODS: We retrospectively evaluated 14 pati

Rhinosinusitis and atopy in patients infected with HIV.

HYPOTHESIS: Rhinosinusitis is common during HIV infection; its prevalence is uncertain and could probably be related to clinical features, immunoallergological status, and diagnostic criteria METHODS: Seventy-four patients hospitalized with HIV infection were prospectively evaluated for the presence of rhinosinusitis based on clinical findings, nasal endoscopy, or paranasal sinus computed tomography (CT). Immune status, nasal smear, features of atopy (based on the prick test), and its contribution to sinusal inflammatory pathology were also evaluated. RESULTS: Most patients were severely imm

[The value of radiotherapy in treating paranasal sinus tumors].

A multidisciplinary treatment conception is necessary for carcinomas of the paranasal sinuses because of the generally very advanced stage (90% bone destruction). Surgery is the most important therapy part. However, it is difficult to obtain complete recovery by surgery alone, because in most cases the tumor has infiltrated into adjacent tissues (nasopharynx, pterygoid muscles, ethmoidal plate, sphenoidal sinus, base of the crane), where a radical operation is not possible. Therefore radiotherapy plays an important role: it shall destroy malignant cells which could not be eliminated by surgery

The pathology of nonfunctional pituitary adenomas.

The nonfunctional adenomas also known as undifferentiated, null cells, or nonsecretory adenomas are endocrinologically silent neoplasms of the anterior lobe of the pituitary gland. They constitute 50% of all pituitary adenomas in the present report. Most tumors are large, often growing beyond the confines of the sella turcica and characterized anatomically by displacement and compression of the adjacent sellar structures including the optic chiasm, hypothalamus, and third ventricle. Others may be truly invasive when they locally or diffusely infiltrate the adjacent structures. They may grow th

[Neuroradiological examinations of extracranial extension of brain tumors].

Before the application of CT scan, the exact identification of extracranial extension of brain tumors was difficult. In the present paper, neuroradiological findings of eight brain tumors with extracranial extension were reviewed. These eight cases were composed of two sphenoidal meningiomas, one CP angle meningioma, one CP angle neurofibroma, two pituitary adenomas, and two jugular foramen chemodectomas. After the first application of CT scan on May, 1976, 841 cases of primary intracranial tumors have been experienced in our clinic. So the incidence of brain tumors with extracranial extension

Cavernous sinus thrombosis secondary to aspergillus granuloma: A case report and review of the literature.

Cavernous sinus thrombosis is a rare but serious complication of sphenoid aspergillosis. The rarity of this pathology makes its diagnostic very difficult on a clinical, biological and radiological sense. The authors present a case of cavernous sinus thrombosis with ipsilateral internal carotid artery thrombosis secondary to a non-invasive sphenoid aspergillosis in an immunocompetent host, responsible of a cavernous syndrome associated to a Claude Bernard Horner syndrome. One year after surgery, the patient is still asymptomatic without recurrence. Diagnostic modalities are detailed and several

Endoscopic frontal sinus obliteration: a new technique for the treatment of chronic frontal sinusitis.

OBJECTIVES: Patients who fail endoscopic drainage procedures for chronic frontal sinusitis often require obliteration of the frontal sinus with abdominal fat. The purpose of this study was to evaluate an endoscopic technique for frontal sinus obliteration. STUDY DESIGN AND SETTING: Retrospective case-control. Thirty-five patients underwent frontal sinus obliteration using either an endoscopic (n=10) or conventional osteoplastic flap (n=25) technique from 1994 to 2004 at an academic medical center. RESULTS: Patients undergoing endoscopic obliteration had less blood loss (P = 0.006), decreased

Skull-base foramina of the middle cranial fossa: reassessment of normal variation with high-resolution CT.

PURPOSE: To evaluate by means of high-resolution CT the anatomic variations of the middle cranial fossa foramen. METHODS: We examined 123 CT studies of the temporal bone in patients with no evidence of disease that might alter foraminal anatomy. A checklist of known variants and suspected structures was used as each case was systematically examined for the presence or absence of these foramina; variations in size, shape, and location; and relationship of structures to each other. Inclusion criteria were established to eliminate error. RESULTS: The foramen rotundum had a constant appearance.

[Fibrous dysplasia of the scull discovered accidently on CT from different indication].

UNLABELLED: Fibrous dysplasia (dysplasia fibrosa -DF) is a local disorder of spongy bone structure caused by local pathological metabolism. Transformation of trabecular bony structure into connective tissue takes place. Inside the transformed bone scattered connective tissue elements, cartilaginous elements and pathological density are found. The disease proliferates either in the medullary bone or in subperiosteal region, and the development of fibrous tissue causes bone expansion with thinning of overlying cortex. The radiological appearance depends on the stage of the disease. The margins o

Nasal septal abscess and facial cellulitis caused by community-acquired methicillin-resistant Staphylococcus aureus.

OBJECTIVE: Community-acquired methicillin-resistant Staphylococcus aureus is emerging as an important pathogen. However, methicillin-resistant Staphylococcus aureus rarely causes nasal septal abscess. CASE REPORT: We present a case of severe, community-acquired, methicillin-resistant Staphylococcus aureus infection causing rapidly progressing sinusitis, nasal septal abscess and facial cellulitis. CONCLUSION: This report serves to remind the clinician of the expanding spectrum of severe infections caused by methicillin-resistant Staphylococcus aureus, all requiring prompt diagnosis and approp

Toxic shock syndrome after functional endonasal sinus surgery: an all or none phenomenon?

Reported cases of toxic shock syndrome (TSS) following nasal surgery or functional endonasal sinus surgery (FESS) are uncommon. Classic TSS is a serious multisystem disorder resulting from Staphylococcus aureus phage I toxic shock syndrome toxin 1 (TSST-1), and it is characterized by fever, rash, hypotension, mucosal hyperemia, vomiting, diarrhea, and laboratory evidence of multisystem organ dysfunction. TSS cases following nasal surgery have been associated with nasal packing, mucosal barrier violation, prior S aureus phage I colonization, as well as low antitoxin antibody levels. Of the 1700

Management of locally aggressive sinus disease using craniofacial exposure and the galeal frontalis fascia-muscle flap.

Histologically, benign sinus diseases can behave aggressively, potentially causing sight- and life-threatening conditions requiring surgical treatment. Four patients illustrating acute and chronic infection, single and multiple sinus polyposis, and mucoceles are presented and discussed to illustrate how the principles of craniofacial exposure, resection, and reconstruction may be employed to treat aggressive sinus disease. The bicoronal and buccal sulcus approaches minimize visible scarring; wide exposure of the orbital contents and dura reduce the risk of excessive bleeding, neurologic compli
