[A case of pycnodysostosis--observation of the skull by CT scan].

A 13-year-old boy was presented to the Department of Neurosurgery, Saiseikai Fukuoka General Hospital for further examinations concerning abnormal findings in the skull radiogram taken when he struck his head. His physical features showed some characteristics the same as those of pycnodysostosis as follows--proportionate dwarfism, prominent forehead, short spoon-shaped fingers, bilateral exophthalmos. A skull radiogram revealed widely open cranial sutures with no healing of the fracture and craniotomy which was performed for an acute epidural hematoma 6 years ago. Furthermore, the mandible was

[Three cases of Eagle's syndrome].

Recently microvascular decompression surgery has been performed for trigeminal neuralgia or glossopharyngeal neuralgia with remarkable success. In differential diagnosis of such neuralgia, a lot of atypical facial pain must be taken into consideration, one of which is so-called Eagle's syndrome. This syndrome is characterized by elongated styloid process or calcified stylohyoid ligament compressing the Vth and/or IXth cranial nerve. In the present paper we report three cases of Eagle's syndrome and discuss the mechanism, differential diagnosis and surgical treatment of the facial pain in this

Respiratory and airway disorders in children with Down Syndrome: a review of the clinical challenges and management.

Down Syndrome (DS), or Trisomy 21, is a common inherited chromosomal disorder, caused by an extra copy of chromosome 21, with features including intellectual disability, hearing and vision disorders, hypotonia, hypothyroidism, cardiac and gastrointestinal structural abnormalities. The characteristic features of flattened nasal bridge, mandibular and maxillary hypoplasia, relative macroglossia, and a narrow nasopharyngeal region all predispose to airway complications and structural abnormalities can extend to the lower airways and lung parenchyma. Congenital airway stenoses and malacia are pres

Nociceptor Neurons Magnify Host Responses to Aggravate Periodontitis.

Periodontitis is a highly prevalent chronic inflammatory disease that progressively destroys the structures supporting teeth, leading to tooth loss. Periodontal tissue is innervated by abundant pain-sensing primary afferents expressing neuropeptides and transient receptor potential vanilloid 1 (TRPV1). However, the roles of nociceptive nerves in periodontitis and bone destruction are controversial. The placement of ligature around the maxillary second molar or the oral inoculation of pathogenic bacteria induced alveolar bone destruction in mice. Chemical ablation of nociceptive neurons in the

Association Between Trigeminal Neuralgia And Unnecessary Tooth Extraction.

BACKGROUND: Trigeminal neuralgia is a very painful condition which is presented as severe pain corresponding to spastic shock. This is caused by trigeminal nerve's vascular compression at root entry zone. The pain is induced due to ipsilateral neurovascular conflict. In spite of the condition being known since centuries, it still continues to frustrate the clinician and its pathogenesis remains as enigma to the medical profession. It is very common for these patients to undergo unnecessary dental extraction without any relief in pain. This study was conducted to assess the association between

Morphometric evaluation and clinical implications of the greater palatine foramen, greater palatine canal and pterygopalatine fossa on CBCT images and review of literature.

INTRODUCTION: The pterygopalatine fossa (PPF) infiltration is performed to reduce blood flow during endoscopic sinus surgery and septorhinoplasty, as well as to control posterior epistaxis and provide regional anesthesia in dental procedures. PPF infiltration performed with consideration of the morphometrics of greater palatine foramen (GPF), greater palatine canal (GPC) and PPF would increase the success of the procedure and reduce the risk of complications. The aim of this study is to investigate the GPF, GPC, lesser palatine foramen (LPF), lesser palatine canal (LPC) and PPF morphology via

Iridogoniodysgenesis syndrome: a case report.

A rare case is presented of iridogoniodysgenesis syndrome, an autosomal dominant inheritance disorder that includes abnormalities in the differentiation of the anterior segment structures and increased values of intraocular pressure, which at long run increases the risk of glaucomatous optic neuropathy. The syndrome is diagnosed when ocular changes are accompanied by extraocular structure anomalies such as maxillary hypoplasia, micro- and anodontia, redundant periumbilical skin, inguinal hernia and hypospadias (males). A 44-year-old male patient presented for treatment of retinal detachment on

Types of psychosocial job demands and adverse events due to dental mismanagement: a cross sectional study.

BACKGROUND: A harsh work environment including psychosocial job demands might cause adverse events due to medical mismanagement, but the association has not been explored. The purpose of the study was to investigate whether some types of psychosocial job demands are associated with adverse events due to dental mismanagement experienced by general dental practitioners. METHODS: A self-administered questionnaire was mailed to members of a local branch of the Japan dental association. A total of 261 dental practitioners responded anonymously (response rate 53%). Psychosocial job demands were mea

CT and MRI features in Bipolaris fungal sinusitis.

Bipolaris is an increasingly recognized cause of fungal sinusitis. Reports of imaging features are sparse. Our purpose was to review the imaging features in patients with Bipolaris fungal sinusitis. A review of our data showed seven patients with culture-proven Bipolaris fungal sinusitis. Computed tomography of the paranasal sinuses in all the patients and MRI in five patients were analysed for the location, nature, extent of the disease and density/signal characteristics on CT/MRI. The sphenoid and posterior ethmoid sinuses were most often involved (six of seven), followed by the anterior eth

Fibrous dysplasia of the zygomaticomaxillary region: outcomes of surgical intervention.

BACKGROUND: Fibrous dysplasia is the most common craniofacial tumor, presenting in both monostotic and polyostotic forms with varying degrees of severity. No consensus exists regarding the surgical management of craniofacial fibrous dysplasia, particularly in the zygomaticomaxillary region. The present study compared long-term outcomes of limited reduction burring versus radical resection of zygomaticomaxillary fibrous dysplasia. METHODS: Patients with craniofacial fibrous dysplasia at the University of California, Los Angeles, Craniofacial Center from 1982 to 2008 were studied based on demog

The importance of the palatine bone for endoscopic endonasal skull base surgery.

Endoscopic endonasal skull base surgery is increasingly prevalent, with its scope expanding from pathogens in the midline region to those in the paramedian region. Maximizing anterior sphenoidectomy is important for the median approach, and lateralizing the pterygopalatine fossa is crucial for the paramedian approach. Maximizing the surgical corridor in the nasal cavity and minimizing damage to neurovascular structures are vital for establishing a surgical field with minimal bleeding, ensuring safe, precise, and gentle procedures. However, the relationship between the maxillofacial and skull b

[Treatment of fibrous dysplasia of the cranio-facial bones. Report of 25 cases].

Fibrous dysplasia accounts for approximately 2% of bone tumors. The ribs, proximal femurs and cranio-facial bones represent the majority of bone lesions. Surgery is the mainstay of treatment but the technique is controversial: conservative surgery or removal of dysplastic lesions followed by implantation of autogenous bone graft. The aim of this study was to assess the indications of each method. The medical records of 25 patients with fibrous dysplasia of the cranio-facial bones treated between January 1, 1980 and December 31, 1994 at the Department of Maxillofacial Surgery, Centre Hospitalie

Influence of Premedication and Dental Anxiety on Anesthetic Efficacy in Patients Undergoing Root Canal for Symptomatic Irreversible Pulpitis in Upper and Lower Molars: A Comparative Study of Articaine and Bupivacaine.

The use of local anesthetics in dentistry is crucial for pain control. Their efficacy may be related to multiple factors, including gender, the clinical status of the patient, anatomical factors, the type of anesthetic, premedication treatment, and the experience of the professional. Aim: The objective of this study was to analyze whether premedication or the degree of patient anxiety influences the anesthetic efficacy of 4% articaine with epinephrine 1:100,000 and 0.5% bupivacaine with epinephrine 1:100,000 in patients undergoing root canal treatment for symptomatic irreversible pulpitis in t

P2X4 receptor expression in a rat model of trigeminal neuropathic pain.

We investigated the P2X4 receptor (P2X4R) expression in the cervical spinal cord, trigeminal ganglion, and infraorbital nerve (ION), after a chronic constriction injury of unilateral ION and a treatment with selective serotonin reuptake inhibitor (SSRI). A recent study has showed that SSRI inhibits P2X4R expression. Injured rats had significantly lower pain thresholds. In injured and slightly injured ION, the P2X4R expression was significantly higher than in the naïve-rat ION. Injured animals with SSRI showed significantly higher pain thresholds than injured animals without the drug. Nonethele
